Predicting symptom progression in first-episode psychosis (FEP) is crucial for tailoring treatment and improving outcomes. Temporal lobe function, indicated by neurophysiological biomarkers like N100, predicts symptom progression and correlates with untreated psychosis. Our recent report showed that source-localized magnetoencephalography (MEG) M100 responses to tones in an oddball paradigm predicted recovery in FEP positive symptoms.

The N1 auditory evoked potential amplitude depends heavily on the inter-stimulus interval (ISI). Typically, shorter ISIs result in reduced N1 amplitudes, suggesting a decreased neural response with high stimulus presentation rates. However, an exception known as N1 facilitation occurs with very brief ISIs (∼150-500 ms), where the N1 amplitude increases.

Randomized controlled trials (RCTs) of youth with mania are very challenging to conduct, given the low base rate of bipolar disorder (BD) and the relative rarity of mania (vs bipolar depression, which tends to be much more common). Thus, many of the RCTs are relatively small, and it may be difficult to clinically interpret results. At the same time, findings about which anti-manic medications are most effective in youth are of critical importance, both because (1) poorly treated mania can lead to substantial negative psychosocial consequences, and (2) these medications can have significant adverse effects.

Anomalous Mismatch Negativity (MMN) in psychosis could be a consequence of disturbed neural oscillatory activity at sensory/perceptual stages of stimulus processing. This study investigated effective connectivity within and between the auditory regions during auditory odd-ball deviance tasks. The analyses were performed on two magnetoencephalography (MEG) datasets: one on duration MMN in a cohort with various diagnoses within the psychosis spectrum and neurotypical controls, and one on duration and pitch MMN in first-episode psychosis patients and matched neurotypical controls.

Background: Previous work indicates that polygenic risk scores (PRS) for bipolar disorder (BD) are elevated in adults and youth with BD, but whether BD-PRS can inform person-level diagnostic prediction is unknown. Here, we test whether BD-PRS improves performance of a previously published risk calculator (RC) for BD.
Methods: 156 parents with BD-I/II and their offspring ages 6-18 were recruited and evaluated with standardized diagnostic assessments every two years for >12 years.

Little is known about factors that contribute to attrition in clinical trials of the pharmacotherapy of psychotic depression. The purpose of this study was to identify factors associated with attrition during acute pharmacotherapy in the Study of the Pharmacotherapy of Psychotic Depression II (STOP-PD II) clinical trial. Sociodemographic and clinical variables were assessed at baseline in 269 men and women, aged 18-85 years, who were treated with up to 12 weeks of open-label sertraline plus olanzapine.

The most important predictor of bipolar disorder (BD) onset is a family history. Based on this premise, several offspring studies have recruited and followed offspring of parents with BD into early adulthood, including the Pittsburgh Bipolar Offspring Study, the Canadian Flourish Cohort, and the Dutch Bipolar Offspring Study (DBOS). These studies have shed important light on the incidence and prevalence of BD and other psychopathology in these offspring (11%-13% for BD-I/II), as well as the most important symptom-level and diagnostic predictors of BD in these youth.

There are currently no long-acting injectable antipsychotics (LAIAs) that are approved by the Food and Drug Administration for use in child and adolescent patients, however these agents are used off-label for the treatment of various psychiatric disorders. This study aims to describe the initiation and maintenance dosing strategies of LAIAs in child and adolescent psychiatry inpatients. This was a single-site retrospective chart review of patients less than 18 years of age initiated on an LAIA during an acute psychiatric hospitalization between October 1, 2015, and October 31, 2022.

Family Based Mental Health Services (FBMHS) with an embedded clinical model, Ecosystemic Structural Family Therapy, is an intervention designed for youth with a serious emotional disturbance (SED) who are at risk of out-of-home placement. The current evaluation examines the association between receipt of FBMHS and rates of out-of-home and community-based care during and after an episode of FBMHS. We identified 25,016 Medicaid-enrolled youth ages 3 to 17 years with receipt of a new FBMHS episode from 1/1/2015 to 6/30/2021.

Aims: For over 30 years, combined research and treatment settings in the US have been critical to conceptualizing care for first-episode psychosis (FEP). Here we describe an early example of such a context, the Services for the Treatment of Early Psychosis (STEP) clinic, which is affiliated with the University of Pittsburgh.
Methods: We describe STEP's historical roots and establishment in the early 1990s; STEP's research and treatment contributions, alongside its growth and ongoing leadership.
