Background: Acute respiratory infections (ARIs) result in millions of illnesses and hundreds of thousands of hospitalizations annually in the United States. The responsible viruses include influenza, parainfluenza, human metapneumovirus, coronaviruses, respiratory syncytial virus (RSV), and human rhinoviruses. A clinical practice guideline on total knee arthroplasty was developed by an American Physical Therapy (APTA) volunteer guideline development group that consisted of physical therapists, an orthopedic surgeon, a nurse, and a consumer. A common method to address staffing shortages is the use of float nurses. This short-term resolution is not without its issues. When float nurses are unfamiliar with the assigned patient population or when there is little time for unit orientation, unnecessary anxiety and undue stress can affect the staff, healthcare providers, and, most importantly, the patients.

Purpose: This exploratory, retrospective study aimed at examining whether patients experience orthostatic hypotension unnecessarily when antihypertensive medication(s) is simultaneously resumed following total joint replacement surgery.

Sample: The study sample consisted of 285 patients who underwent either total knee or total hip surgery and had a dual medical diagnosis of hypertension requiring treatment with more than 1 type of antihypertensive medication.

Method: Medical record reviews of all patients admitted to the orthopaedic unit from March 1, 2006, through June 30, 2006, at University of Pittsburgh Medical Center were analyzed to determine whether patients experienced any hypotensive event within 48 hr of their total joint replacement surgery following the resumption of their antihypertensive medications.

Establishing the role of tigecycline in an era of antimicrobial resistance.

Tigecycline is a glycylcycline antibacterial agent derived from minocycline with a broad spectrum of activity. Like tetracycline agents, it inhibits bacterial protein synthesis but is able to bind to target sites with higher affinity and overcome tetracycline-resistance mechanisms due to structural modifications. The safety and efficacy of tigecycline has been demonstrated in clinical trials for complicated intra-abdominal and complicated skin and skin-structure infections.

Although published reports describe specific handheld computer applications in medical training, we know very little yet about how, and how well, handheld computing fits into the spectrum of information resources available for patient care and physician training. This paper reports preliminary quantitative and qualitative results from an evaluation study designed to track changes in computer usage patterns and computer-related attitudes before and after introduction of handheld computing. Pre-implementation differences between residents and faculty s usage patterns are interpreted in terms of a "work role" construct.
