Background: The health of young carers is poorer, on average, than their peers. The timing and persistence of health and wellbeing changes around becoming a young carer are unknown. We investigated how health and wellbeing change before, during and after becoming a young carer in the UK and whether this varies by caring intensity, age, gender, ethnicity, or household income.

Integrating Patient-Generated Health Data from Mobile Devices into Electronic Health Records: Best Practice Recommendations by the IFCC Committee on Mobile Health and Bioengineering in Laboratory Medicine (C-MHBLM).

Background: An increasing number of wearable medical devices are being used for personal monitoring and professional health care purposes. These mobile health devices collect a variety of biometric and health data but do not routinely connect to a patient's electronic health record (EHR) or electronic medical record (EMR) for access by a patient's health care team.

Methods: The International Federation of Clinical Chemistry and Laboratory Medicine (IFCC) Committee on Mobile Health and Bioengineering in Laboratory Medicine (C-MHBLM) developed consensus recommendations for consideration when interfacing mobile health devices to an EHR/EMR.

Background: Surgical site infections (SSIs) have been shown to increase patient morbidity and mortality, impact on quality of life and place a significant economic burden on healthcare systems worldwide. Irrigation using wound cleansing and antiseptic effective solutions during surgical procedures is a key part of SSI prevention. The optimal solution would have minimal cytotoxicity to the patient while maintaining a minimum concentration required for antimicrobial activity necessary to prevent opportunistic pathogens and biofilm formation.

Targeting the Type I Interferon Pathway in Glomerular Kidney Disease: Rationale and Therapeutic Opportunities.

Type I interferons (IFNs) are immunostimulatory molecules that can activate the innate and adaptive immune systems. In cases of immune dysfunction, prolonged activation of the type I IFN pathway has been correlated with kidney tissue damage in a wide range of kidney disorders, such as lupus nephritis (LN) and focal segmental glomerulosclerosis (FSGS). Genetic mutations, such as risk variants in conjunction with elevated type I IFN expression, are also associated with higher rates of chronic kidney disease in patients with LN and collapsing FSGS.

Introduction: Endothelin A (ETA) receptor activation is a driver of proteinuria, kidney inflammation, and fibrosis in IgA nephropathy (IgAN). Atrasentan, a selective ETA receptor antagonist, has potential to reduce proteinuria and preserve kidney function in IgAN. ALIGN (NCT04573478) is a phase 3, randomized, double-blind, placebo-controlled clinical trial of atrasentan in patients with IgAN at high risk of kidney function loss.

Efficacy and Safety of Pegcetacoplan in Kidney Transplant Recipients With Recurrent Complement 3 Glomerulopathy or Primary Immune Complex Membranoproliferative Glomerulonephritis.

Introduction: Complement 3 glomerulopathy (C3G) and primary immune complex membranoproliferative glomerulonephritis (IC-MPGN) have high risks for disease recurrence and allograft loss in transplant kidneys. Pegcetacoplan (targeted complement 3 [C3]/C3b inhibitor) may prevent excessive deposition of C3 and complement 5 [C5] breakdown products and associated renal damage.

Methods: NOBLE (NCT04572854) is a prospective, phase 2, multicenter, open-label, randomized controlled trial evaluating the efficacy and safety of pegcetacoplan in posttransplant patients with recurrent C3G or IC-MPGN.

Seroprevalence of SARS-CoV-2 in wet market workers in Dhaka, Bangladesh in 2022.

Objectives: We assessed the transmission of SARS-CoV-2 and vaccine receipt in a representative sample of wet market workers in a highly dense, low-income setting. Wet markets are key in many Asian settings, including Dhaka, Bangladesh, for fresh food, including animal protein.

Methods: During early 2022, we assessed the prevalence of anti-SARS-CoV-2 antibodies in a random sample of poultry and vegetable workers in 15 wet markets, and investigated associations with socio-demographic characteristics and COVID-19 vaccination.

In this longitudinal study, we compare the age of reaching early developmental milestones in bilingual and monolingual children and between the bilinguals' two languages. We present data from 302 Polish bilinguals (living outside of Poland with various majority languages) and 302 Polish monolinguals, aged = 12.78 months on study entry (range: 024 months), matched on sex, age at study entry, duration of parental reporting, and parental education.

Purpose: Revision knee replacement (RevKR) for infection is rare but increasing. It is hypothesised that higher hospital volume reduces adverse outcomes. The aim was to estimate the association of surgical unit volume with outcomes following first, single-stage RevKR for infection.

Quantitative approaches for studying G protein-coupled receptor signalling and pharmacology.

G protein-coupled receptor (GPCR) signalling pathways underlie numerous physiological processes, are implicated in many diseases and are major targets for therapeutics. There are more than 800 GPCRs, which together transduce a vast array of extracellular stimuli into a variety of intracellular signals via heterotrimeric G protein activation and multiple downstream effectors. A key challenge in cell biology research and the pharmaceutical industry is developing tools that enable the quantitative investigation of GPCR signalling pathways to gain mechanistic insights into the varied cellular functions and pharmacology of GPCRs.

In this article, I argue that the short-term approach of corporate leaders being brought into healthcare for 'transformation' has led to a trail of service dismemberment with no discernible clinical gain for those we seek to serve.

Long-Term Clinical Outcomes After Autologous Hematopoietic Stem Cell Transplantation in 49 Individuals Living With HIV (PLWH) and Affected by High-Risk or Relapsed Lymphoma: A European Experience of Continued Relevance for PLWH.

Previous reports have indicated that during the era of combination antiretroviral therapy, the major causes of morbidity and mortality in people living with HIV (PLWH) were not solely linked to HIV-related opportunistic infections but also to cancers that were difficult to manage due to HIV-related immunodeficiency. We investigated whether PLWH who underwent autologous hematopoietic stem cell transplantation (ASCT) for lymphomas experienced significant morbidity over the past thirty years following HIV infection. We conducted a retrospective follow-up study of 49 PLWH over a 10-year period following ASCT.

Diversity in psychiatry education and patient and public involvement: roundtable analysis.

Patient involvement in psychiatry education struggles to be representative of the patients that doctors will treat once qualified. The issues of mental health stigma, cultural perspectives of mental health and the unique role of teaching, required exploring to establish the barriers and facilitators to increasing the diversity of patients involved in psychiatry education. To explore the causes of this lack of representation, a roundtable event with 34 delegates composed of people with lived experience of mental health issues, people from underserved communities, academics, mental health professionals and charity representatives met to discuss the barriers to involvement in psychiatry education and possible solutions.

C-low threshold mechanoreceptors (C-LTMRs) in animals (termed C-tactile (CT) fibres in humans) are a subgroup of C-fibre primary afferents, which innervate hairy skin and respond to low-threshold punctate indentations and brush stimuli. These afferents respond to gentle touch stimuli and are implicated in mediating pleasant/affective touch. These afferents have traditionally been studied using low-throughput, technically challenging approaches, including microneurography in humans and teased fibre electrophysiology in other mammals.

Reimagining psychosis prevention: responding to the accessibility issues of At-Risk Mental State (ARMS) services through a selective public health approach.

At-Risk Mental State (ARMS) services aim to prevent the onset of first-episode psychosis (FEP) in those with specific clinical or genetic risk markers. In England, ARMS services are currently expanding, but the accessibility of this preventative approach remains questionable, especially for a subgroup of FEP patients and those from specific ethnic minority communities. This commentary outlines the key debates about why a complimentary approach to psychosis prevention is necessary, and gives details for an innovative public health strategy, drawing on existing research and health prevention theory.

Effectiveness of a stepped-care programme of WHO psychological interventions in a population of migrants: results from the RESPOND randomized controlled trial.

Migrant populations - including labour migrants, undocumented migrants, asylum seekers, refugees, internationally displaced persons, and other populations on the move - are exposed to a variety of stressors that affect their mental health. We designed and tested the effectiveness of a stepped-care programme consisting of two scalable psychological interventions developed by the World Health Organization (WHO) and locally adapted for migrant populations. A parallel-group randomized controlled trial was conducted in Italy.

Feeding and eating disorders (FEDs) are a heterogeneous grouping of disorders at the mind-body interface, with typical onset from childhood into emerging adulthood. They occur along a spectrum of disordered eating and compensatory weight management behaviors, and from low to high body weight. Psychiatric comorbidities are the norm.
