Since 2010, medical toxicology physicians from the American College of Medical Toxicology (ACMT) Toxicology Investigators Consortium (ToxIC) have provided reports on their in-hospital and clinic patient consultations to a national case registry, known as the ToxIC Core Registry. De-identified patient data entered into the registry includes patient demographics, reason for medical toxicology evaluation, exposure agents, clinical signs and symptoms, treatments and antidotes administered, and mortality. This thirteenth annual report provides data from 7206 patients entered into the Core Registry in 2022 by 35 participating sites comprising 52 distinct healthcare facilities, bringing the total case count to 94,939.

Article Synopsis
  • The study evaluated the quality and readability of online resources related to hypoplastic left heart syndrome (HLHS) to ensure they are accessible for patients and families, given the average reading level of US citizens is around 8th grade.
  • A total of 52 health websites were analyzed for their accountability, content quality, interactivity, and structure, with only 32.7% disclosing authorship and 26.9% citing sources, indicating low accountability.
  • Open-access websites scored the highest on overall quality and accountability compared to hospital or healthcare organization sites, suggesting the need for improvements in the latter to enhance patient education.

Article Synopsis
  • The study investigates the rise in pediatric cannabis intoxications as more areas legalize recreational use, looking specifically at severe outcomes in emergency department cases from 2017 to 2020.
  • Researchers found that 38% of the 138 children studied were admitted to intensive care, with polysubstance use and ingestion of cannabis edibles identified as significant predictors of severe outcomes.
  • For children older than 10, polysubstance ingestion was the main risk factor for severe outcomes, while all younger children primarily ingested edibles, highlighting differences in risks based on age.

This study aimed to assess the implementation and access to telehealth-delivered pediatric weight management (PWM) during the initial phase of the COVID-19 pandemic at six US PWM programs (PWMP) using the RE-AIM (Reach, Effectiveness, Adoption, Implementation, Maintenance) framework. The COVID-19 period (COVID) was defined in this retrospective, multisite study as the time when each site closed in-person care during 2020. The Pre-COVID period (Pre-COVID) was an equivalent time frame in 2019.

Over the past 2 decades, several categorizations have been proposed for the abnormalities of the aortic root. These schemes have mostly been devoid of input from specialists of congenital cardiac disease. The aim of this review is to provide a classification, from the perspective of these specialists, based on an understanding of normal and abnormal morphogenesis and anatomy, with emphasis placed on the features of clinical and surgical relevance.

Congenital Heart Center, UF Health Shands Hospital, Division of Cardiovascular Surgery, Departments of Surgery and Pediatrics, University of Florida, Gainesville, FL, USA.

Article Synopsis
  • * A survey was conducted among 37 students who participated, revealing a significant increase in interest in medicine and cardiothoracic surgery after their shadowing experience.
  • * Findings indicated that students with family members in medicine had more prior clinical exposure, while those without such connections could benefit more from these shadowing opportunities to enhance their career understanding.

Article Synopsis
  • - The text discusses the current treatment landscape for cholangiocarcinoma (CCA), emphasizing that many patients are not surgical candidates and survival with traditional chemotherapy is under 12 months.
  • - A systematic review identified four FDA-approved targeted therapies for advanced CCA, including ivosidenib and three FGFR2 inhibitors, which have created new options for treatment and improved patient outcomes.
  • - The emergence of these targeted therapies has prompted new research into combination treatments, such as integrating chemotherapy and immunotherapy, enhancing the overall approach to managing CCA.

Therapeutic drug monitoring in cystic fibrosis and associations with pulmonary exacerbations and lung function.

Respir Med

June 2023

Department of Internal Medicine, Pulmonary and Critical Care Division, UF Health-Shands Hospital, University of Florida, Gainesville, FL, 32611, USA. Electronic address:

Background: Cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy have resulted in longer life expectancies, yet pulmonary exacerbations remain a leading cause of morbidity. Intravenous antibiotics is the mainstay treatment, however achieving adequate concentrations remains challenging. The effect of therapeutic drug monitoring (TDM) of beta-lactams on exacerbations and lung function has not been studied.

The ventricular components of the conduction axis remain vulnerable following transcatheter aortic valvar replacement. We aimed to describe features which may be used accurately by interventionalists to predict the precise location of the conduction axis, hoping better to avoid conduction disturbances. We scanned eight normal adult heart specimens by 3T magnetic resonance, using the images to simulate histological sections in order accurately to place the conduction axis back within the heart.

Purpose: To investigate the impact of the following three methodological pitfalls on model generalizability: () violation of the independence assumption, () model evaluation with an inappropriate performance indicator or baseline for comparison, and () batch effect.

Materials And Methods: The authors used retrospective CT, histopathologic analysis, and radiography datasets to develop machine learning models with and without the three methodological pitfalls to quantitatively illustrate their effect on model performance and generalizability. F1 score was used to measure performance, and differences in performance between models developed with and without errors were assessed using the Wilcoxon rank sum test when applicable.

Beta-lactam target attainment and associated outcomes in patients with bloodstream infections.

Int J Antimicrob Agents

March 2023

Infectious Disease Pharmacokinetics Lab, Emerging Pathogens Institute, University of Florida, Gainesville, FL, USA; Department of Pharmacotherapy and Translational Research, College of Pharmacy, University of Florida, Gainesville, FL, USA.

Objectives: To evaluate the association between early and cumulative beta-lactam pharmacokinetic/pharmacodynamic (PK/PD) parameters and therapy outcomes in bloodstream infection (BSI).

Methods: Adult patients who received cefepime, meropenem, or piperacillin/tazobactam for BSI and had concentrations measured were included. Beta-lactam exposure was generated and the time that free concentration remained above the minimum inhibitory concentration (fT) and four multiples of MIC (fT) were calculated for times 0-24 h and 0-7 days of therapy.

Goal: Retention of advanced practice providers (APPs) is an issue of critical importance to healthcare leaders. High APP turnover negatively affects an organization's ability to meet patient needs, maintain quality of care, and control costs. Our goal was to identify any association between intent to leave and specific survey factors; analyze the distribution of scale factors from the survey; and describe differences among the APP roles, specialties, and length of employment.

  • A phase 3 clinical trial assessed the safety and effectiveness of rivipansel, an E-selectin antagonist, in 345 patients (adults and children) with vaso-occlusive crises (VOC) requiring hospitalization.
  • The study found that rivipansel did not significantly improve the primary endpoint of time to readiness for discharge compared to placebo, although it did notably reduce soluble E-selectin levels in the treatment group.
  • A post hoc analysis suggested that administering rivipansel early after VOC pain onset could significantly decrease the time to discharge and reduce opioid use, indicating that the timing of treatment may be key for better outcomes.
