Tympanostomy Tube Insertion With and Without Adenoidectomy.

JAMA Otolaryngol Head Neck Surg

January 2025

Department of Otolaryngology-Head & Neck Surgery, Stanford University School of Medicine, Stanford, California.

Importance: The benefit of adenoidectomy on otologic outcomes after tympanostomy tube (TT) insertion is unclear. Results from prior work are challenging to interpret due to small sample sizes, heterogeneous study designs, and varying outcome measures.

Objective: To evaluate the association between adenoidectomy and otologic outcomes using a US population-level sample of children who received TTs, producing generalizable results for widespread clinical application.

Transfer-RNA-derived fragments (tRFs) are a novel class of small non-coding RNAs that have been implicated in oncogenesis. tRFs may act as post-transcriptional regulators by recruiting AGO proteins and binding to highly complementary regions of mRNA at seed regions, resulting in the knockdown of the transcript. Therefore, tRFs may be critical to tumorigenesis and warrant investigation as potential biomarkers.

Background And Aims: The ongoing antimicrobial resistant crisis heralds the need for new therapeutics against enteric infection. In mouse models, colon epithelial peroxisome proliferator-activated receptor-γ (PPAR-γ) signaling limits oxygen and nitrate luminal bioavailability, thereby preventing bacterial pathogen colonization. However, whether this mechanism operates similarly in humans remains uncertain.

Introduction: With emerging evidence supporting the clinical efficacy and safety of mechanical thrombectomy (MT) for distal medium vessel occlusions (DMVOs), MT devices specifically designed to navigate through smaller caliber and more delicate tortuous distal cerebrovasculature are required. This study describes our single-center experience using the AXS Vecta 46 intermediate catheter for first-line thromboaspiration of DMVOs.

Methods: We identified all patients who underwent MT using the Vecta 46 for first-line thromboaspiration for primary or secondary DMVOs.

Article Synopsis
  • The study investigates the effects of neoadjuvant and adjuvant chemotherapy on survival rates in patients with Upper tract urothelial carcinoma undergoing Nephroureterectomy, using data from a multicenter registry.
  • A total of 1,994 patients were analyzed, revealing that neoadjuvant chemotherapy notably improved cancer-specific and overall survival, especially in patients with advanced tumors or positive nodes.
  • The results suggest a significant survival benefit from neoadjuvant chemotherapy, indicating a need for further research to explore its impact in this patient population.

Background: Previous studies have demonstrated that access to robotic surgery is influenced by socioeconomic factors, including insurance status. The 2010 Affordable Care Act established an avenue for states to expand Medicaid coverage, which has increased access to surgical care for many conditions. We hypothesized that socioeconomic disparities in access to robotic repair of non-elective emergency general surgery (EGS) hernias are less prevalent in California, a Medicaid expansion state, compared to Florida, which has not adopted Medicaid expansion.

Neoplastic Progression Risk in Females With Barrett's Esophagus: A Systematic Review and Meta-Analysis of Individual Patient Data.

Clin Gastroenterol Hepatol

February 2025

Division of Gastroenterology and Hepatology, Erasmus MC Cancer Institute, Erasmus University Medical Center, Rotterdam, the Netherlands. Electronic address:

Background And Aims: Females with Barrett's esophagus (BE) have a lower risk of neoplastic progression than males, but sufficiently powered risk analyses are lacking. This systematic review and meta-analysis of individual patient data (IPD) aimed to provide more robust evidence on neoplastic progression risk in females.

Methods: We conducted a systematic literature search of 3 electronic databases (Medline, Embase, Google Scholar) from inception until August 2023.

Influence of Th1 versus Th2 immune bias on viral, pathological, and immunological dynamics in SARS-CoV-2 variant-infected human ACE2 knock-in mice.

EBioMedicine

October 2024

Center for Vaccine Innovation, La Jolla Institute for Immunology, La Jolla, CA, 92037, USA; Division of Host-Microbe Systems and Therapeutics, Department of Pediatrics, UC San Diego School of Medicine, La Jolla, CA, 92037, USA. Electronic address:

Article Synopsis
  • Mouse models with humanized ACE2 and TMPRSS2 genes were created to study how Th1 and Th2 immune responses affect SARS-CoV-2 infection.
  • Mice infected with the Delta variant required ACE2 for lung infection, while Omicron didn't, with Omicron causing more severe disease in specific mouse models.
  • The study suggests that the genetic background of the host influences the immune response and severity of SARS-CoV-2 infections, highlighting the need for more research on this variability.

Article Synopsis
  • The study focuses on improving care transitions for heart failure patients by implementing an intervention called I-TRANSFER-HF, which combines early home health care (HHC) nurse visits with outpatient medical follow-up to reduce hospital readmissions.
  • It employs a Hybrid Type 1, stepped wedge randomized trial design, involving multiple hospital and home health agency (HHA) pairs across the US, to measure the effectiveness of the intervention on readmission rates and patient outcomes.
  • Additionally, the study aims to understand the factors affecting the implementation of I-TRANSFER-HF through qualitative interviews with key stakeholders, using the Consolidated Framework for Implementation Research 2.0 to guide the analysis.

Objective: Discrepancies in survival outcomes of various genitourinary tract malignancies have been documented across different racial and ethnic groups. Here we sought to examine long-term survival outcomes of patients with upper tract urothelial carcinoma (UTUC) following radical nephroureterectomy (RNU) when stratified by race.

Methods: A multicenter retrospective analysis using the ROBUUST (ROBotic surgery for Upper tract Urothelial cancer Study) registry identified patients undergoing RNU for UTUC between 2015 and 2022 at 12 centers across the United States, Europe, and Asia.

Lung ultrasonography underdiagnoses clinically significant pneumothorax.

Surgery

December 2024

Division of Trauma, Surgical Critical Care, Burns and Acute Care Surgery, Department of Surgery, UC San Diego School of Medicine, San Diego, CA.

Article Synopsis
  • A study was conducted to evaluate the effectiveness of chest ultrasonography in detecting pneumothorax in trauma patients, finding it had low sensitivity.
  • The analysis included 1,489 patients with blunt chest trauma who underwent various imaging techniques, revealing a high false negative rate of 72% for ultrasonography.
  • The findings suggest that relying solely on chest ultrasonography for screening pneumothorax might be risky due to the significant number of missed diagnoses.
