The finding of pupil-sparing third nerve palsy is synonymous with diabetic third nerve palsy in the minds of many clinicians. While this is the most common cause of a third nerve palsy with normal pupillary response, it is not the only cause. We present the case of an elderly diabetic gentleman who presented with pupil-sparing third nerve palsy and gait abnormalities without any weakness or incoordination in the extremities.

Current guidelines tend to focus on a p-value threshold of a pre-specified primary endpoint tested in randomized controlled clinical trials to determine a treatment effect for a specific drug. However, a p-value does not always provide evidence on the treatment effect of a drug, especially when stratification of the data does not account for unforeseen variables introduced into the analysis. We report and discuss a rare case in which investigational site stratification in the pre-specified analysis method of a primary endpoint results in a loss of statistical power in the evaluation of the treatment effect due to data attrition of almost 17% of outcome data in the phase III randomized, controlled PANAMO study in critically ill COVID-19 patients.

Introduction: Congenital complete heart block is a condition where there is a risk of Stokes Adam's attacks and sudden death may occur. Once the escape rate is too low, or other high-risk factors occur, these patients ultimately need pacemakers placed. Epicardial or transvenous pacemakers have typically been in employed dependent on size of the patient and other circumstances.

Purpose: The new CAP guideline published in August 2022 recommends using immunohistochemistry (IHC) to test for mismatch repair defects in gastroesophageal (GE), small bowel (SB), or endometrial carcinoma (EC) cancers over next-generation sequencing assessment of microsatellite instability (NGS-MSI) for immune checkpoint inhibitor (ICI) therapy eligibility and states there is a preference to use IHC over NGS-MSI in colorectal carcinoma (CRC).

Methods: We assessed the concordance of NGS-MSI and IHC-MMR from a very large cohort across the spectrum of solid tumors.

Results: Of the over 190,000 samples with both NGS-MSI and IHC-MMR about 1,160 were initially flagged as discordant.

Br J Anaesth

November 2024

Department of Anesthesiology and Pain Management, University of Texas Southwestern and Children's Health, Dallas, TX, USA; Department of Outcomes Research, Cleveland Clinic, Cleveland, OH, USA.

Background: The incidence of severe adverse events in children with SARS-CoV-2 undergoing anaesthesia has not been well established. We examined the relationship between SARS-CoV-2 infection and severe perioperative adverse events in children.

Methods: This multicentre (21 North American institutions), retrospective cohort study included children <18 years old, with American Society of Anesthesiologists physical status (ASA PS) of 1-4 and non-severe SARS-CoV-2, who underwent general anaesthesia between April 1, 2020, and March 31, 2021.

Many central nervous system (CNS) disorders lack approved treatment options. Previous research demonstrated that peptide CAQK can bind to chondroitin sulfate proteoglycans (CSPGs) in the extracellular matrix of the CNS. In vivo studies have investigated CAQK conjugated to nanoparticles containing therapeutic agents with varying methodologies/outcomes.

Department of Pediatric Cardiology, UC Davis Medical Center, Sacramento, CA 95817, USA.

Article Synopsis
  • Implantable loop recorders enable ongoing heart rhythm tracking that can be monitored remotely, which is particularly beneficial for certain medical conditions.
  • Their use in children is limited due to uncertainty about when they are appropriate and suitable for implantation.
  • A case study of a 2.7kg female patient with Long QT3 shows that the device effectively identified heart issues and facilitated her care, with successful placement and no complications.

  • Since January 2020, COVID-19 impacted over 100 million people in the U.S., and there is a need for better understanding of racial and ethnic disparities in hospitalization data related to the virus.
  • A study analyzed data from the National Inpatient Sample to quantify these disparities, revealing significant differences in hospitalization rates and outcomes among different racial and ethnic groups.
  • The findings showed that Black, Hispanic, and Native American patients experienced hospitalization rates over twice that of White patients, with higher mortality rates linked to factors like age, sex, and certain health conditions.
