Fœtal sacrococcygeal teratoma type I: A case report.

Radiol Case Rep

February 2025

Pediatric Surgery Department, Tunis Faculty of Medicine El Manar University, Béchir Hamza Children's Hospital, Tunis, Tunisia.

Sacrococcygeal teratoma (SCT) is a rare congenital tumor typically diagnosed in neonates, with management challenges arising from the size of the tumor and associated delivery complications. In this case, a 32-year-old gravida 5 para 5 woman with a history of three prior cesarean sections was diagnosed with a giant type I SCT at 30 weeks of gestation through prenatal ultrasound, confirmed by fetal MRI. At 34 weeks, an emergency cesarean section was performed due to acute fetal distress, resulting in a newborn with transient respiratory distress.

Faculty of Medicine of Tunis, Tunis Maternity and Neonatology Center, Department 'C' of Gynecology and Obstetrics, University of Tunis El Manar, Tunis, Tunisia (Frikha, Aloui, Karoui, Hamami, Menjli, Abouda, and Chanoufi).

We present a rare case of uterine artery pseudoaneurysm (UAP) following an emergency cesarean section, which led to severe vaginal bleeding and subcutaneous hematoma. The patient, a 40-year-old woman with no history of hemophilia or hemostasis disorders, presented with sudden profuse vaginal bleeding and multiple subcutaneous hematomas at the site of the cesarean scar ten days postoperation. Ultrasound and CT scan confirmed the presence of a pseudoaneurysm in the right uterine artery.

Article Synopsis
  • The increasing rates of obstetric complications, like post-partum hemorrhage from placenta accreta spectrum, necessitate new treatment methods, highlighting the effectiveness of internal iliac artery embolization for managing obstetric bleeding.
  • Two case studies demonstrate successful outcomes: a 38-year-old patient experienced persistent bleeding after a subtotal hysterectomy and was saved by arterial embolization, while a 35-year-old patient with placenta accreta after a hysterectomy also benefited from the procedure.
  • The discussion emphasizes the importance of interventional radiology in treating postpartum hemorrhage and suggests that consulting a specialized radiologist can be crucial for improving patient outcomes post-surgery.

Background: The placenta accreta spectrum is a complex disorder characterized by abnormal invasion of the placenta into the uterine wall, posing a significant risk of life-threatening haemorrhage for patients. Its incidence is on the rise, largely attributed to the increasing rates of caesarean sections. Management of this spectrum involves a multidisciplinary approach, although standardized protocols are not yet established.

Human Leukocyte Antigen-G (HLA-G) is known as an immune suppressive molecule; it interacts with several immune cells and inhibits their functions. HLA-G molecule is highly represented in pathological conditions including malignant transformation. To the best of our knowledge this is the first study that focuses on the expression of soluble HLA-G (sHLA-G) in endometrial cancer (EC).

Pelvic Packing for Intractable Obstetric Hemorrhage After Emergency Peripartum Hysterectomy: A Review.

Obstet Gynecol Surv

February 2018

Professor and Head, Anesthesiology and Reanimation Department, Tunis Maternity and Neonatology Center, Tunis El Manar University, Tunis, Tunisia.

Importance: Postpartum hemorrhage (PPH) is the leading cause of maternal mortality worldwide. Even after emergency peripartum hysterectomy (EPH), bleeding may occur in the setting of acquired coagulopathy. This type of bleeding resistant to clipping, ligating, or suturing could be successfully controlled with a pelvic packing.

Are the Wells Score and the Revised Geneva Score valuable for the diagnosis of pulmonary embolism in pregnancy?

Eur J Obstet Gynecol Reprod Biol

February 2018

Anesthesiology and Reanimation Department, Tunis Maternity and Neonatology Center, Tunis El Manar University, Tunisia.

Objectives: To evaluate and to compare the predictive accuracy of the Wells score and the revised Geneva scores for the diagnosis of pulmonary embolism in the pregnant and postpartum population.

Study Design: All pregnant or post-partum patients with a suspected PE and for whom a diagnostic imaging testing was performed (VQ scintigraphy or computed tomography pulmonary angiography) over a 3-year period were included in the study. The Wells and Revised Geneva Scores were calculated on the same cohort of patients and dichotomized into low and intermediate/high probability groups.

The utility of sentinel lymph node mapping in the management of endometrial atypical hyperplasia.

Gynecol Oncol

March 2018

Gynecologic Oncology Division, L'Hôtel-Dieu de Québec, Centre Hospitalier Universitaire de Québec, Laval University, Quebec City, Canada. Electronic address:

Objectives: To determine the risk of endometrial cancer (EC) and lymph node involvement in patients with a preoperative diagnosis of "AH-only" versus "AH - cannot rule out carcinoma" and to study the value of SLN mapping.

Methods: We reviewed all patients with a preoperative diagnosis of atypical hyperplasia, who underwent primary surgery with SLN mapping followed by pelvic lymphadenectomy. Sensitivity and negative predictive value (NPV) of SLN and rates of endometrial cancer were calculated.

Objective: While the accuracy of the SLN procedure has been validated in patients with low risk EC, its relevance for high-risk EC remains debated. The aim of this study was to evaluate the accuracy of SLN mapping in patients with high-risk EC.

Study Method: We reviewed all patients with high risk EC (grade 3 endometrioid, serous, carcinosarcoma, clear cell and undifferentiated) who underwent primary surgery with SLN mapping followed by pelvic +/- paraaortic lymphadenectomy, between November 2010 and November 2016.

Ultrasound cervical length measurement in prediction of labor induction outcome.

J Neonatal Perinatal Med

May 2016

C Department, Tunis Maternity and Neonatology Center, Tunis El Manar University, Tunis, Tunisia.

Induction of labor is one of the most common procedures in modern obstetrics, with an incidence of approximately 20% of all deliveries. Not all of these inductions result in vaginal delivery; some lead to cesarean sections, either for emergency reasons or for failed induction. That's why, It seems necessary to outline strategies for the improvement of the success rate of induced deliveries.

Objectives: To study the safety and effectiveness of pelvic packing in the control of post emergency peripartum hysterectomy (EPH) bleeding in a postpartum hemorrhage (PPH) setting.

Study Design: From 39 patients with a severe PPH leading to an EPH (January 2010-December 2013), we identified a group of 17 patients requiring a pelvic packing (packing group) and a second group of 22 patients not requiring a pelvic packing (non-packing group). For each group, transfusion requirements were recorded from time of PPH diagnosis to end of the surgical procedure (P1: Period 1) and from that point to the end management in the SICU (P2: Period 2).
