Background: More than 65% of patients with end-stage renal disease (ESRD) use arteriovenous fistulas (AVFs) for hemodialysis. The increasing incidence of comorbid ESRD and obesity (body mass index, >35 kg/m) precludes patients from kidney transplantation, resulting in a need for long-term, durable AVF access. Compared with traditional superficialization techniques for overlying adiposity, liposuction is minimally invasive and well-tolerated, allowing for earlier fistula use with lower complications.

Background: Liposuction is the most common cosmetic plastic surgery procedure in the United States, and lipo-aspiration for fat grafting is gaining in popularity. The results are effective, but complications include seroma, contour irregularities, skin necrosis, and even death. Scarring, dehiscence, and infection at the port site, although minor, are a common and less-discussed problem.

Background: Liposuction has become the most sought-after aesthetic surgery across the globe, increasing the demand for more developments in patient safety protocols. Evidence of the efficacy of tranexamic acid (TXA) in reducing intraoperative blood loss with minimal complications have led to its widespread applications across various surgical operations. However, the role and effectiveness of TXA in liposuction is not fully understood.

Successful Outcome of VASER Liposuction Followed by Keto Diet in a 35-year-old Man with Class III Obesity.

Plast Reconstr Surg Glob Open

August 2024

From the Hasan Surgery FZ LLC, Dubai, UAE.

Article Synopsis
  • Obesity is a major health issue globally, requiring various treatments, especially for individuals struggling with morbid obesity, who often find traditional weight loss methods ineffective.
  • A case study on a 35-year-old man with a BMI of 40 demonstrated significant weight loss through tumescent VASER liposuction (removing 5L of fat) followed by a 6-month ketogenic diet, reducing his weight from 127 kg to 97 kg.
  • The combined treatments led to improved body contour, enhanced skin tightening with minimal loose skin, effective pain management, and better energy levels, suggesting this approach could be effective for similar cases of severe obesity.

Article Synopsis
  • Lipoedema is a painful condition primarily affecting women's limbs, which does not respond well to diet and exercise.
  • A study evaluated the health-related quality of life (HRQoL) outcomes for 47 patients who underwent tumescent liposuction (TL) for lower limb lipoedema, using validated questionnaires before and after the procedure.
  • Results indicated significant HRQoL improvements at an average of 12 months post-surgery, especially among patients with stage 3 lipoedema, but more extensive and long-term research is needed to confirm the lasting benefits of TL.

  • High-definition liposculpture (HDL) improves muscular definition but challenges like skin laxity and edema still exist post-surgery.
  • A new technique involving a continuous suture-splint was tested on 52 patients undergoing HDL to address these issues, using ultrasound for safety.
  • Results showed minimal complications, high patient satisfaction, and effective reduction in edema and skin laxity, suggesting the suture-splint method enhances recovery after body contouring.

Article Synopsis
  • Tranexamic acid (TXA) is commonly used in surgeries to reduce blood loss, but its potential link to venous thromboembolism (VTE) has not been previously documented in plastic surgery literature.
  • This study presents 5 cases of VTE occurring in patients who received TXA during various plastic surgeries, particularly abdominoplasty, where VTE rates are notably high.
  • Although there was a noticeable increase in VTE cases after using TXA compared to prior data, the study does not establish a direct causal relationship between TXA and the incidence of VTE.

Article Synopsis
  • Aesthetic surgeries, particularly liposuction, have grown in popularity over the last four years, with a focus on tumescent liposuction, including true and semi-tumescent variants; the latter’s safety under deep sedation using a specific protocol (propofol-ketamine) is underexplored.
  • A study analyzing 3094 patients over eight years found no fatalities, though there were 43 instances of mask ventilation mostly in the first 10 minutes, and some patients experienced cancellations or needed higher-level care, highlighting vigilance during procedures.
  • The findings suggest that the propofol-ketamine protocol is a safe option for tumescent liposuction, underscoring the importance of careful patient evaluation,

Article Synopsis
  • Tranexamic acid (TXA) is being evaluated for its effectiveness in reducing blood loss during liposuction procedures, although its blood-sparing benefits are not well established in this area.
  • A systematic review identified 711 articles, narrowing down to 5 relevant studies, which found that TXA significantly reduced blood loss and related complications when used in liposuction.
  • The results suggest that TXA could enhance patient safety during liposuction by minimizing adverse effects, prompting a call for further research to establish the best methods for its application.

Adipose biopsy techniques for studies in human exercise physiology.

Am J Physiol Regul Integr Comp Physiol

March 2024

Human Performance Laboratory, Ball State University, Muncie, Indiana, United States.

Article Synopsis
  • - The study investigated the effects of two adipose biopsy techniques on tissue quality, focusing on metrics like adipocyte cell size and levels of inflammatory cytokines in normal-weight individuals.
  • - Thirteen participants underwent biopsies using either a 6-mm Bergström needle or a 4-mm Mercedes needle with a 'wet' technique, and both methods produced high-quality tissue suitable for analysis.
  • - Results showed no significant differences in adipocyte size or cytokine levels between the two techniques, indicating both methods are valid for exercise physiology research related to adipose tissue in nonobese subjects.

Perfusion Dynamics during Secondary Flap Debulking with Liposuction.

J Reconstr Microsurg

July 2024

Department of Plastic and Reconstructive Surgery, Chang Gung Memorial Hospital, Chang Gung Medical College and Chang Gung University, Taoyuan, Taiwan.

Article Synopsis
  • Lower extremity trauma often requires flap transfer for reconstruction, with the goal of achieving thin tissue coverage, which usually involves secondary debulking of the flap.
  • The study compares two debulking methods: conventional lipectomy and liposuction followed by excision of excess flap tissue, analyzing patient demographics, surgical details, and outcomes.
  • Results indicate similar postoperative complication rates for both methods, though liposuction leads to significant initial decreases in flap perfusion, which may not predict actual flap necrosis or complications accurately.

Effects of epinephrine, lidocaine, and prilocaine on viability and differentiation capacity of human adipose stem cells.

J Plast Reconstr Aesthet Surg

December 2023

Department of Plastic Surgery, Hand Surgery, Burn Center, University Hospital RWTH Aachen, Pauwelsstraße 30, 52074 Aachen, NRW, Germany.

Article Synopsis
  • Local anesthetics (LAs) like prilocaine and lidocaine are commonly used in plastic and reconstructive surgery, and recent studies suggest they aren’t as harmful to adipose stem cells (ASCs) as previously thought.
  • The study evaluated the effects of prilocaine and lidocaine combined with epinephrine on the viability and differentiation of ASCs, showing that while prilocaine decreased metabolic activity and cell numbers, lidocaine had a milder effect.
  • The findings suggest that using these anesthetics in clinically relevant concentrations does not significantly harm ASCs or their ability to differentiate into other cell types, which is important for surgical outcomes.

Article Synopsis
  • The project aimed to enhance perioperative care for patients undergoing tumescent liposuction by creating and implementing an evidence-based protocol, an educational course for staff, and clear discharge instructions for patients.
  • The project's design involved using expert validation through the modified Delphi process and assessing the effectiveness of the educational course and discharge instructions.
  • Despite some positive trends in provider knowledge and patient satisfaction, the results did not show statistically significant improvements, likely due to a small sample size.
