January 2025

Sheffield Centre for Health and Related Research, School of Medicine and Population Health, University of Sheffield, Sheffield, UK.

Background: Tuberculous meningitis is fatal if untreated and can lead to lifelong neurological sequelae. However, to our knowledge, there are no data on the number of children affected by this disease. We aimed to estimate the global disease burden and attributable mortality of childhood tuberculous meningitis by WHO regions, age groups, treatment status, and HIV status in 2019.

Article Synopsis
  • - This study aimed to explore metabolic markers associated with Mycobacterium tuberculosis (M. tb) in the cerebrospinal fluid (CSF) of South African children suffering from tuberculous meningitis (TBM) using advanced metabolomics techniques.
  • - Four key metabolites were found to distinguish TBM patients from controls: lower levels of mannose and arabinose, and higher levels of nonanoic acid and propanoic acid in the TBM group.
  • - The research suggests that nonanoic acid may be a novel M. tb-linked marker in TBM, while propanoic acid could indicate brain energy disruptions and inflammation; however, mannose and arabinose were not effective markers for M.

Background: Hyponatremia is common in tuberculous meningitis (TBM), but its impact on disease severity and outcomes is unclear.

Methods: In a cohort of 864 adult patients with TBM in Indonesia, we assessed the prevalence and prognostic significance of hyponatremia, classified as moderate (120-130 mEq/L) or severe (<120 mEq/L). Patients received standard antituberculous therapy and corticosteroids and were followed for 1-year mortality.

For the effective treatment of tuberculosis with first-line anti-tubercular drugs, drug concentrations need to be measured at the site of infection to determine drug exposure. To enable the measurement of the anti-tuberculosis drugs isoniazid and pyrazinamide in the nervous system of patients with tuberculous meningitis, an analytical method was developed and validated for the quantification of these drugs in human cerebrospinal fluid. Samples were prepared by solid phase extraction using Strata-X polymeric extraction plates.

Paradoxical Deterioration During Treatment of Tuberculous Meningitis Responded to Infliximab But Not to Steroids.

Eur J Case Rep Intern Med

November 2024

Department of Internal Medicine, University Center of Internal Medicine, Cantonal Hospital Baselland, Liestal, Switzerland.

Unlabelled: Paradoxical deterioration in patients with tuberculous meningitis is a significant diagnostic and treatment challenge. We present the case of a 55-year-old previously healthy male, human immunodeficiency virus (HIV)-negative, presenting with headache, fever, agitation and slurred speech, who was ultimately diagnosed with tuberculous meningoencephalitis confirmed by cerebrospinal fluid polymerase chain reaction positive for complex. After initiation of a standard antitubercular therapy combined with glucocorticoids, the patient experienced an initial neurological improvement.

We report a case of tuberculous meningitis without pleocytosis of the cerebrospinal fluid (CSF) in a 27-year-old patient admitted for a meningeal syndrome with signs of basilar involvement and an infectious syndrome associated with a hacking cough with whitish sputum and night sweats, evolving for 15 days before her admission, in a context of weight loss of 2 kg, asthenia, and anorexia. Cytobacteriological and chemical analysis of the CSF revealed less than 3 cells/mm white blood cells, high protein levels of 2.54 g/l, and low glucose levels of 0.

  • Assisted reproductive technologies like IVF-ET can help infertile patients achieve pregnancy, but they may also lead to changes in the body that lower immune defenses, increasing the risk of tuberculosis (TB) infection.
  • A case report highlights a patient with normal immune function who developed severe pulmonary tuberculosis following IVF-ET, emphasizing the potential dangers.
  • The authors recommend screening for genital and latent TB before IVF-ET and advise healthcare providers to be alert for active TB symptoms during pregnancy, especially in the presence of unexplained fever, cough, or bleeding.

  • - The study aimed to evaluate how effective nanopore sequencing technology is for diagnosing tuberculous meningitis (TBM) using cerebrospinal fluid samples from patients.
  • - Compared to traditional methods like acid-fast staining and culture, nanopore sequencing had a sensitivity of 43.30% and an area under the ROC curve of 0.661, indicating better performance overall.
  • - When nanopore sequencing was used alongside imaging techniques, it achieved the best diagnostic results with increased sensitivity, specificity, and predictive values, showing its potential as a reliable tool for TBM diagnosis.

Neuro-meningeal tuberculosis: A pediatric case report and review of the literature.

Radiol Case Rep

January 2025

Pediatric Radiology Department, Children's Hospital, Faculty of Medicine and Pharmacy, Mohammed V University, Rabat, Morocco.

Pediatric neuro-meningeal tuberculosis leads to high rates of mortality and morbidity. Prompt diagnosis and initiation of treatment are challenging; imaging findings play a key role in establishing the presumptive diagnosis. General brain imaging findings are well reported; however, specific data on cerebral vascular and spinal involvement in children are sparse.

Paediatric cerebral palsy in South Africa: Prevention and care gaps at hospital level.

Afr J Disabil

October 2024

Department of Pediatrics and Child Health, Faculty of Health Sciences, University of Cape Town, Cape Town, South Africa.

Background: Population-based data show high proportions of severe cases of cerebral palsy (CP) in resource-poor regions such as sub-Saharan Africa, where most children have potentially preventable risk factors (factors that may increase the likelihood of CP occurrence but can be mitigated through medical interventions).

Objectives: This study aimed to describe the demographic and clinical profile of children living with CP accessing services at Tygerberg Hospital over a period of 10 years (2010-2020), identify the potential gaps in care (proportion of individuals in a country requiring but not receiving suboptimal or inadequate care), and comparison with a similar study at the same centre two decades ago.

Method: This 10-year retrospective study investigated causes and morbidities in children with CP, attending a central hospital in the Western Cape, South Africa.
