Introduction: Microsurgical breast reconstruction has become popular over the past twenty years and allows a tailor-tuck approach to each patient. However, smoking or coagulation disorders may switch surgeon's choice towards alternative options. When facing these risk factors, we performed pedicled latissimus dorsi (LD) flap and thoracodorsal artery perforator (TDAP) flap reconstruction with immediate fat transfer (LIFT and TIFT), achieving satisfactory surgical outcomes.

Introduction: Massive weight loss (MWL) patients present skin and soft-tissue laxity and ptosis involving inner thighs. Previous efforts were focused on the upper two thirds of the thigh, while literature has never dealt specifically with lower thigh contouring. We present an original approach to the lower inner thigh, intended for patients who already had upper thigh lift, discussing outcomes, advantages, and limits of our technique.

Approximately a half of breast tumors classified as HER2-negative exhibit HER2-low-positive expression. We recently described a high instability of HER2-low-positive expression from primary breast cancer (BC) to relapse. Previous studies reporting discordance in HER2 status between baseline biopsy and residual disease (RD) in patients undergoing neoadjuvant treatment did not include the HER2-low-positive category.

Bone sarcoma are infrequent diseases, representing < 0.2% of all adult neoplasms. A multidisciplinary management within reference centers for sarcoma, with discussion of the diagnostic and therapeutic strategies within an expert multidisciplinary tumour board, is essential for these patients, given its heterogeneity and low frequency.

Evolution of HER2-low expression from primary to recurrent breast cancer.

NPJ Breast Cancer

October 2021

Department of Surgery, Oncology and Gastroenterology (DISCOG), University of Padova, Padova, Italy.

Article Synopsis
  • - About 34.2% of primary HER2-negative breast cancer cases show low HER2 expression, which can be targeted with new therapies, and this increases to 37.3% during relapse.
  • - HER2-low status is more common in hormone receptor-positive tumors compared to triple-negative tumors, with rates of 47.3% and 35.4%, respectively, in primary samples.
  • - There is a significant rate of HER2 discordance (38.0%) between primary and relapse samples, suggesting that a primary HER2-0 tumor may evolve to HER2-low, potentially opening new treatment options for these patients.

MRP-1 is implicated in multidrug resistance and was described as prognostic in high-risk patients with soft-tissue sarcoma (STS) in a previous study. The current research aimed to validate MRP-1 prognostic/predictive value in localized sarcomas treated with anthracyclines plus ifosfamide within the ISG-1001 phase III study. In addition, the inhibitory activity on MRP-1 was investigated in preclinical studies to identify new combinations able to increase the efficacy of standard chemotherapy in STS.

Tumor genotype, location, and malignant potential shape the immunogenicity of primary untreated gastrointestinal stromal tumors.

JCI Insight

November 2020

Unit of Oncogenetics and Functional Oncogenomics, Centro di Riferimento Oncologico di Aviano (CRO Aviano) IRCCS, National Cancer Institute, Aviano, Italy.

Article Synopsis
  • Recent research has uncovered the presence of immune cells within gastrointestinal stromal tumors (GISTs), but the specific factors that influence how GISTs provoke immune responses remain unclear.
  • In a study involving 82 untreated primary GIST samples, factors such as genetic mutations and tumor location were found to influence the level of immune cell infiltration, with more aggressive tumors showing greater immune presence.
  • The study also identified potential therapeutic targets by noting that certain pathways (Hedgehog and WNT/β-catenin) might both promote cancer growth and suppress immune responses, suggesting that drugs targeting these pathways could enhance treatment effectiveness and activate anti-tumor immunity.

Background: Uterine sarcomas are very rare tumours with different histotypes, molecular features and clinical outcomes; therefore, it is difficult to carry out prospective clinical trials, and this often results in heterogeneous management of patients in the clinical practice.

Aim: We planned to set up an Italian consensus conference on these diseases in order to provide recommendations on treatments and quality of care in our country.

Results: Early-stage uterine sarcomas are managed by hysterectomy + bilateral salpingo-oophorectomy according to menopausal status and histology; lymphadenectomy is not indicated in patients without bulky nodes, and morcellation must be avoided.
