A simple and rapid method that is not based on race and ethnicity for classifying people with skin of color is of paramount importance in dermatology. The currently used Fitzpatrick classification of sun-reactive skin types is inadequate. Newer scales that have been used in immigration surveys and sociology studies are not applicable in the office setting.

Article Synopsis
  • The study aimed to investigate blood glucose outcomes in people with HIV (PWH) who have pre-diabetes and are starting treatment with dolutegravir compared to those with normal glucose levels.
  • Researchers conducted a matched cohort study with 44 PWH with pre-diabetes and 88 with normal glucose, measuring changes in fasting and 2-hour blood glucose levels at various points over 48 weeks.
  • Results showed that while those with normal glucose experienced an increase in fasting blood glucose, those with pre-diabetes saw a significant decrease, indicating improvement in glucose metrics, questioning the need for intensive glucose monitoring in the initial months of dolutegravir treatment.

Vitamin D deficiency appears to be more prevalent than previously considered in the adult critically ill population, and specifically burn-injured patients. No definitive regimen has been shown to restore vitamin D (25(OH)D) levels more effectively to therapeutic levels in the burn-injured population. The purpose of this study was to investigate the effects of either ergocalciferol (D2, 50,000 IU weekly) or cholecalciferol (D3, 6,000 IU daily) in adults with burns ≥ 10% TBSA.

Target-Driven Tissue-Agnostic Drug Approvals-A New Path of Drug Development.

Cancers (Basel)

July 2024

Division of Hematology & Medical Oncology, Center for Experimental Therapeutics, Knight Cancer Institute, Oregon Health & Science University, 3181 SW Sam Jackson Park Rd., Portland, OR 97239, USA.

The regulatory approvals of tumor-agnostic therapies have led to the re-evaluation of the drug development process. The conventional models of drug development are histology-based. On the other hand, the tumor-agnostic drug development of a new drug (or combination) focuses on targeting a common genomic biomarker in multiple cancers, regardless of histology.

Context: Heart failure (HF) is a chronic condition that affects the heart's functional capacity, resulting in symptoms such as fatigue, edema, and dyspnea. It affects millions of adults in the United States and presents challenges in optimizing treatment and coordinating care among clinicians. Additionally, the various classifications for HF and limited research on treatment outcomes in heart failure with midrange ejection fraction (HFmrEF) and heart failure with preserved ejection fraction (HFpEF) further complicate the pharmacological management of patients with this disease.

Background: Mobile Link is a mobile phone-based intervention to increase access to, and use of, health care services among female entertainment workers in Cambodia who face higher risks for specific diseases and gender-based violence. A multisite randomized controlled trial showed that Mobile Link connected female entertainment workers with outreach workers for information and escorted referrals after 6 months but did not lead to statistically significant improvements in HIV and sexually transmitted infection testing, contraceptive use, and condom use.

Objective: This study aims to conduct a 3-part economic evaluation of Mobile Link to understand its costs, value, and affordability.

As psychedelics are being investigated for more medical indications, it has become important to characterize the adverse effects and pharmacological interactions with these medications. This chapter will summarize what is known about the toxicology and drug-drug interactions for classic psychedelics, such as LSD, psilocybin, DMT, 5-MeO-DMT, mescaline, 2C-B, Bromo-DragonFLY, and 25X-NBOMe.

Background: Current assessments on topical treatment attributes in actinic keratosis (AK) do not evaluate safety, effectiveness, and satisfaction from both clinician and patient perspectives, creating an unmet need for more comprehensive AK-specific measures that fully capture the patient experience.

Objective: To develop an actinic keratosis-specific expert panel questionnaire (AK-EPQ) of patient-reported outcomes and clinician-reported outcomes for use in research studies.

Methods: Using interviews of patients with AK and targeted literature reviews, a 9-person consensus panel of dermatologists with expertise in AK treatment was convened to develop the AK-EPQ to assess AK-specific patient-reported outcomes and clinician-reported outcomes.

Background: Large osteochondral lesions of the humeral head can result from locked posterior dislocations, avascular necrosis, and osteochondritis dissecans. Fresh osteochondral allograft (OCA) transplantation is a treatment option for young patients with focal osteochondral defects of the humeral head. The purpose of this case series was to assess graft survivorship, subjective patient-reported outcomes, and satisfaction among 7 patients who underwent OCA transplantation of the humeral head.

Background: Physical therapy educators strive to achieve excellence in education and develop clinicians who are prepared to meet the health needs of society. Previous research has focused on describing the traits, beliefs, and experiences of exemplary educators and clinicians to inform didactic and clinical curricula and serve as a model for expert practice. However, no studies have explored the experiences and perspectives of exemplary students.

Due to the challenges associated with accurately identifying as the causative agent in urinary tract infections (UTIs), coupled with limited guidance on treatment protocols, reports of similar cases still need to be made publicly available because of their increasing emergence. In this article, we present the first documented case of a UTI caused by ornithinolytica in a patient with triple-negative breast cancer undergoing neoadjuvant chemotherapy. This case report highlights as an uncommon yet significant pathogen, particularly in immunocompromised patients.

Article Synopsis
  • Anal cancer is primarily caused by HPV, especially HPV-16, and often develops from anal high-grade squamous intraepithelial lesions (HSILs), with the highest incidence seen in men who have sex with men (MSM) living with HIV, particularly those over 50 years old.
  • A study conducted in San Francisco between 2018 and 2022 found high rates of anal HSIL and oncogenic HPV among older MSM, regardless of HIV status; 47% of MSM living with HIV and 37% of MSM not living with HIV had anal HSIL.
  • The results suggest that there is a significant prevalence of high-risk anal HPV types in this age group, indicating a need for anal cancer screening

Glucocorticoids are ubiquitously used by physicians for a myriad of diseases. Though powerful and potentially lifesaving, sometimes the dangerous side effects are not at the forefront of our medical decision-making. By immunosuppressing patients, glucocorticoids can place patients at increased risk for not only the metabolic effects of chronic glucocorticoid use but also increased risk for opportunistic infections.

The current study analyzed the intersecting biophysical, biochemical, and functional properties of extracellular particles (EPs) with the human immunodeficiency virus type-1 (HIV-1) beyond the currently accepted size range for HIV-1. We isolated five fractions (Frac-A through Frac-E) from HIV-infected cells by sequential differential ultracentrifugation (DUC). All fractions showed a heterogeneous size distribution with median particle sizes greater than 100 nm for Frac-A through Frac-D but not for Frac-E, which contained small EPs with an average size well below 50 nm.

Non-ST segment elevation myocardial infarction (NSTEMI) is a relatively unknown complication of injecting sublingual Suboxone (buprenorphine/naloxone). Buprenorphine/naloxone should be taken as a sublingual tablet or a buccal film and not injected, so its effects from this mode of administration are not well known. While the differential diagnosis for chest pain is very broad, many practitioners do not associate chest pain with the use of buprenorphine/naloxone.

We recently identified TMEM230 as a master regulator of the endomembrane system of cells. TMEM230 expression is necessary for promoting motor protein dependent intracellular trafficking of metalloproteins for cellular energy production in mitochondria. TMEM230 is also required for transport and secretion of metalloproteinases for autophagy and phagosome dependent clearance of misfolded proteins, defective RNAs and damaged cells, activities that decline with aging.
