Article Synopsis
  • Olaparib is a drug used for treating ovarian cancer, specifically as maintenance therapy after chemotherapy, but it can cause anemia in some patients due to its varying effects on drug concentration in the body.
  • A new assay using high-performance liquid chromatography with ultraviolet light (HPLC-UV) has been developed to monitor olaparib concentrations in patients, ensuring effective drug dosage and safety.
  • The HPLC-UV method demonstrated high accuracy and reliability, making it a promising tool for clinical use in managing olaparib therapy for ovarian cancer patients.

  • The study investigates the link between pharmacokinetic parameters of direct oral anticoagulants (DOACs) and the risk of post-endoscopic submucosal dissection (ESD) bleeding, noting that GI bleeding is a significant concern.
  • It found a 12.8% incidence of post-ESD bleeding in patients on DOACs, with factors such as age and FXa activity correlating significantly with bleeding risk.
  • The research suggests that measuring drug levels and anticoagulant effectiveness could help identify patients at higher risk for bleeding after ESD procedures.

Background: Intensity-modulated radiation therapy (IMRT) has been increasingly used as a new radiation modality for unresectable non-small cell lung cancer (NSCLC). The risk factors for radiation pneumonitis (RP) during consolidation durvalumab following concurrent chemoradiotherapy (CCRT) using IMRT have not been thoroughly investigated.

Methods: This retrospective study analyzed medical record data from consecutive patients diagnosed with NSCLC who underwent CCRT and consolidation durvalumab at our institution between April 2018 and September 2022.

  • A study analyzed data from 10,342 patients with acute lower gastrointestinal bleeding (ALGIB) to better understand surgical rates, procedures, and outcomes.
  • Surgery was performed in only 1.3% of patients, with common indications being diverticular bleeding, colorectal cancer, and small bowel bleeding, and the overall mortality rate was 1.5% for those who underwent surgery.
  • The findings suggest that identifying the source of bleeding and using endoscopic techniques may lower the need for surgery and enhance treatment for ALGIB.

Deep learning reconstruction for improving the visualization of acute brain infarct on computed tomography.

Neuroradiology

January 2024

Department of Radiology, Graduate School of Medicine, The University of Tokyo, 7-3-1 Hongo, Bunkyo-Ku, Tokyo, 113-8655, Japan.

Article Synopsis
  • The study examined how deep learning reconstruction (DLR) affects the identification of acute infarcts in CT images compared to hybrid iterative reconstruction (Hybrid IR).
  • It included analyses from 29 patients with acute infarction and 26 without, using both DLR and Hybrid IR for unenhanced head CT images.
  • Results showed that DLR significantly improved the visibility of acute infarcts, especially within 24 hours of onset, and reduced image noise compared to Hybrid IR.

Association between transfusion volume and survival outcome following trauma: Insight into the limit of transfusion from an analysis of nationwide trauma registry in Japan.

J Trauma Acute Care Surg

May 2024

From the Tertiary Emergency Medical Center (K. Shibahashi, M.H., K. Sugiyama), Tokyo Metropolitan Bokutoh Hospital, Tokyo; Advanced Medical Emergency Department and Critical Care Center (M.A.), Japan Red Cross Maebashi Hospital, Maebashi, Japan.

Background: Whether and how the transfusion volume should be limited in resuscitation after trauma remains unclear. We investigated the association between transfusion volume and survival outcome following trauma.

Methods: Using the Japan Trauma Data Bank (2019-2021), we identified patients 18 years or older who received balanced blood transfusion within the first 24 hours of injury.

Objectives: Signs of life (SOLs) during cardiac arrest (gasping, pupillary light reaction, or any form of body movement) are suggested to be associated with favorable neurologic outcomes in out-of-hospital cardiac arrest (OHCA). While data has demonstrated that extracorporeal cardiopulmonary resuscitation (ECPR) can improve outcomes in cases of refractory cardiac arrest, it is expected that other contributing factors lead to positive outcomes. This study aimed to investigate whether SOL on arrival is associated with neurologic outcomes in patients with OHCA who have undergone ECPR.

Systemic Capillary Leak Syndrome (SCLS) is a rare disease that causes severe distributive shock provoked by infection or vaccination. SCLS is clinically diagnosed by a triad of distributive shock, paradoxical hemoconcentration, and hypoalbuminemia. SCLS associated with coronavirus disease (COVID-19) in adults has not been reported yet in Japan.

  • This study compared the effects of early versus delayed feeding in patients with acute lower gastrointestinal bleeding (ALGIB) after their bleeding was controlled.
  • Researchers analyzed data from 5,910 patients across Japan, dividing them into early (feeding within 1 day) and delayed (feeding after 2-3 days) groups.
  • Results showed no significant differences in rebleeding rates or need for further treatments between the groups, but the early feeding group had a shorter hospital stay, suggesting it is a safe and beneficial practice.

Radical cystectomy is a gold-standard treatment for muscle-invasive bladder cancer. We recently introduced robot-assisted radical cystectomy (RARC) with perioperative enhanced recovery after surgery (ERAS). The medical records of patients with bladder cancer who underwent open radical cystectomy (ORC) or RARC/ERAS at NTT Medical Center Tokyo were retrospectively reviewed to compare the surgical outcomes, hospital stay, and medical costs between groups.

Article Synopsis
  • A study analyzed the impact of weekend admissions on outcomes for patients with acute lower gastrointestinal bleeding in 49 Japanese hospitals from 2010 to 2019.
  • The findings showed no significant difference in mortality rates between weekend and weekday admissions, although weekend admissions resulted in a higher need for blood transfusions.
  • Additionally, weekend admissions led to delays in early colonoscopy and an increase in urgent CT scans, but overall mortality and other outcomes remained unaffected.

Background & Aims: The QUASAR Phase 2b Induction Study evaluated the efficacy and safety of guselkumab, an interleukin-23p19 subunit antagonist, in patients with moderately to severely active ulcerative colitis (UC) with prior inadequate response and/or intolerance to corticosteroids, immunosuppressants, and/or advanced therapy.

Methods: In this double-blind, placebo-controlled, dose-ranging, induction study, patients were randomized (1:1:1) to receive intravenous guselkumab 200 or 400 mg or placebo at weeks 0/4/8. The primary endpoint was clinical response (compared with baseline, modified Mayo score decrease ≥30% and ≥2 points, rectal bleeding subscore ≥1-point decrease or subscore of 0/1) at week 12.

  • Left ventricular free wall rupture is a severe complication of heart attacks that can lead to cardiac tamponade, which is life-threatening.
  • ECMO is often used to manage heart instability, but increased pressure can cause problems with blood flow and brain perfusion.
  • A case study of a 65-year-old man highlights how combining ECMO with pericardial drainage helped stabilize his condition temporarily, though he ultimately faced a poor neurological outcome and passed away.
