The aim of this study was to compare in-hospital mortality of three procedures -halo-vest immobilization, anterior spinal fixation (ASF), and posterior spinal fixation (PSF)- in the treatment of elderly patients with isolated C2 odontoid fracture. We extracted data for elderly patients who were admitted with C2 odontoid fracture and treated with at least one of the three procedures (halo-vest immobilization, ASF, or PSF) during hospitalization. We conducted a generalized propensity score-based matching weight analysis to compare in-hospital mortality among the three procedures.

Article Synopsis
  • The study assesses the effectiveness of antibiotics in treating uncomplicated diverticulitis, which is still debated among healthcare professionals.
  • A nationwide database was analyzed, comparing outcomes of patients receiving antibiotics versus those who did not within two days of admission, focusing on surgery rates, death, and medical costs.
  • Results showed that the antibiotic group had significantly lower rates of surgery, but higher medical costs, suggesting that early antibiotic treatment may help reduce severe complications in patients with diverticulitis.

J Antimicrob Chemother

December 2023

Department of Neurological Surgery, Graduate School of Medicine, Nippon Medical School, 1-1-5 Sendagi, Bunkyo-ku, Tokyo 113-8602, Japan.

Methods: The study used data from the Diagnosis Procedure Combination database in Japan.

J Intensive Care

October 2023

Department of Clinical Epidemiology and Health Economics, School of Public Health, The University of Tokyo, 7-3-1 Hongo, Bunkyo-ku, Tokyo, 113-0033, Japan.

Article Synopsis
  • A study investigated the practices and trends of massive transfusion for trauma in Japan using a national database that includes a majority of emergency hospitals from 2011 to 2020.
  • It found that only 0.15% of trauma patients received massive transfusions, and the incidence of these transfusions decreased significantly during the study period.
  • Additionally, higher ratios of fresh frozen plasma to red blood cells were linked to better survival rates, while lower ratios increased hospital mortality and adverse events.

Article Synopsis
  • The study introduces Days of Antibiotic Spectrum Coverage (DASC) as a metric for measuring inpatient antimicrobial consumption in Japan, comparing it to Days of Therapy (DOT).
  • It analyzed data from over 26 million hospital admissions from 2014 to 2018, finding significant increases in both DOT and DASC, but no significant change in the DASC/DOT ratio.
  • Results suggest that using DASC alone may not fully reflect antibiotic stewardship trends; combining DOT and DASC/DOT provides a better evaluation of antimicrobial practices in hospitals.

Article Synopsis
  • A study examined the effectiveness of proton-pump inhibitors (PPIs) and vonoprazan in preventing delayed bleeding after esophageal endoscopic submucosal dissection (ESD), as their efficacy in this context was previously unclear.
  • Researchers analyzed data from over 54,000 patients who underwent esophageal ESD between 2012 and 2020, comparing bleeding rates in those prescribed PPIs or vonoprazan to those with no acid suppression.
  • The results indicated no significant difference in delayed bleeding rates between the two groups, suggesting that prescribing PPIs or vonoprazan for this purpose may not be recommended.

Background: Cryptococcal meningitis (CM) is an invasive fungal infection with a poor prognosis that often occurs in both healthy individuals and compromised hosts, such as patients infected with human immunodeficiency virus (HIV). Unlike CM in HIV patients, evidence regarding CM in non-HIV patients is limited to small retrospective studies.

Objective: To identify the pretreatment prognostic factors for CM in non-HIV patients.

Introduction: Cyclosporine or infliximab (IFX) have been used to avoid surgery in patients with severe refractory ulcerative colitis (UC). Tacrolimus (Tac) is occasionally used as an alternative to cyclosporine; however, the comparative efficacy of Tac and IFX has not been reported. We aimed to compare the effectiveness of Tac and IFX in hospitalized patients with UC.

Objectives: Life-threatening antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) with rapidly progressive glomerulonephritis (RPGN) and/or alveolar haemorrhage (AH) has a poor prognosis. Rituximab (RTX) is as effective as cyclophosphamide (CY) in remission induction therapy; however, the effectiveness and safety of RTX have not been established in life-threatening AAV. This study aimed to investigate the short-term effectiveness and safety of RTX in life-threatening AAV with RPGN and/or AH.

Article Synopsis
  • Epilepsy can lead to significant psychological distress and anxiety, particularly due to seizures, but the connections between stress responses and anxiety in epilepsy patients are not well understood.
  • A study involving 159 epilepsy outpatients in Japan examined the links between seizure frequency, stress/arousal traits, and anxiety levels using specific questionnaires and analyses.
  • Results indicated strong correlations between stress/arousal characteristics and anxiety scores, with findings suggesting that epilepsy-specific stress and arousal responses mediate the relationship between different types of anxiety in these patients.

Objectives: To compare the prognosis of late elderly patients with spontaneous intracerebral hemorrhage (ICH) treated by endoscopic evacuation and craniotomy MATERIALS AND METHODS: Using the Diagnosis Procedure Combination database, we identified patients aged ≥ 75 years who underwent surgery for spontaneous ICH within 48 hours after admission between April 2014 and March 2018. Eligible patients were classified into two groups according to the type of surgery (endoscopic surgery and craniotomy). Propensity-score matching weight analysis was conducted to compare the good neurological outcome modified Rankin Scale (mRS) score (0-4) at discharge as the primary endpoint between the two groups.
