Rationale: Although patients with central disorders of hypersomnolence (CDH) exhibit characteristic symptoms of hypersomnia frequently, it takes 5 to 15 years from the onset for its diagnosis due to the lack of symptom recognition. Here, we present a case of idiopathic hypersomnia (IH), a CDH, wherein early diagnosis was aided by a video footage of a spontaneous sleep attack.

Patient Concerns: A 21-year-old man lost consciousness while driving and experienced an accident.

Article Synopsis
  • Hospitals that spend more on medical resources show lower mortality rates for sepsis patients than those that spend less, according to a study analyzing data from 997 hospitals between 2010 and 2017.
  • The study found that in the highest spending quartile, the in-hospital mortality rate was significantly lower (13.2%) compared to the lowest quartile (15.7%), indicating a strong relationship between spending and patient survival.
  • Additionally, hospitals with higher spending also had higher effective costs per survivor, suggesting that increased expenditure leads not only to better outcomes but also costs more per patient saved, especially in smaller or medium-sized hospitals.

The temporal pole (TP) is considered one of the major paralimbic cortical regions, and is involved in a variety of functions such as sensory perception, emotion, semantic processing, and social cognition. Based on differences in cytoarchitecture, the TP can be further subdivided into smaller regions (dorsal, ventrolateral and ventromedial), each forming key nodes of distinct functional networks. However, the brain structural connectivity profile of TP subregions is not fully clarified.

Inosine monophosphate dehydrogenase (IMPDH) is a rate-limiting enzyme in the de novo GTP biosynthesis pathway. Recent studies suggest that IMPDH2, an isoform of IMPDH, can localize to specific subcellular compartments under certain conditions and regulate site-specific GTP availability and small GTPase activity in invasive cancer cells. However, it is unclear whether IMPDH2 plays a site-specific regulatory role in subcellular functions in healthy cells.
