Recent development of effective regimen on the cancer chemotherapy and the improved management for the side effects of chemotherapy will give a safety management for outpatients with cancer. The chemotherapy for outpatients will produce their prolonged and better life at home and improve their quality of life. In the ambulatory center for the chemotherapy at Tokai University Oiso Hospital, the role of the pharmacists is important for the safety and comfortable management of patients.

Background: Occult neoplastic cells (ONCs) are the tumor cells floating in the lymph node sinuses, distant from the primary tumor, and supposed to be one of most reliable marker of prognosis.

Methods: We report here the case of a 52-year-old woman with a gastric cancer associated by numerous ONCs.

Results: Postoperative examination of the stomach disclosed an advanced, poorly differentiated adenocarcinoma with frequent lymph node metastases.

Paradoxical shortening of sympathetic skin response latency at distal recording sites.

Clin Neurophysiol

January 2009

Department of Rehabilitation Medicine, Tokai University Oiso Hospital, 21-1 Gakkyo, Oisomachi, Nakagun, Kanagawa 259-0198, Japan.

Objective: The purpose of this study was to investigate and to discuss the neurophysiological mechanism of paradoxical shortening of the sympathetic skin response (SSR) latency at distal recording sites.

Methods: The latency and peak-to-peak amplitude of SSRs evoked by magnetic stimuli were analyzed. Eight active electrodes were placed on the palmar (anterior) and dorsal (posterior) sides of the hand (forearm) proximal to the distal arrangement.

This study aimed to retrospectively assess the efficacy of postoperative adjuvant chemotherapy in 77 patients who underwent curative resection for stage III colorectal cancer. They were treated by intravenous administration of 5FU + LV (FL-IV group, 38) or oral administration of UFT + PSK (oral group, 39). The 3-year relapse-free (3Y-RFS), 5-year relapse-free (5Y-RFS) and 5-year overall survival (5Y-OS) were calculated for each group, and clinical results and adverse events (AEs) were compared between the two groups.

We report a patient with superior mesenteric venous thrombosis presenting as diabetic ketonuria and bacteremia. The patient was a 65-year-old man with a history of diabetes mellitus, and was admitted to our hospital due to high fever. Tests revealed diabetic ketonuria and Bacteroides fragilis bacteremia.

The present study was designed to retrospectively examine the efficacy of postoperative adjuvant chemotherapy in 107 patients with stage II primary colorectal cancer who underwent curative resection. The chemotherapy regimen was intravenous 5FU/LV in 30 patients (FL-IV group) and oral UFT/PSK in 77 patients (oral group). There were no significant differences between the FL-IV and the oral group with respect to the 3-year relapse-free survival rate, 5-year relapse-free survival rate, and 5-year overall survival rate, which were 82.

We reported a 23-year-old female who was treated for rash due to hand, foot, and mouth disease (HFMD). On day 4 of hospitalization, the patient showed opsoclonus (jerky eye movements in all directions), myoclonus of the neck, trunk, and extremities, and cerebellar ataxia. Based on the changes in serum viral antibody titer, the patient was diagnosed as enterovirus 71 infection.

The present case is a 5-month-old female with atopic dermatitis who was brought to hospital for growth failure noted upon regular health examination. Laboratory examinations revealed hyponatremia, hyperkalemia, hypoproteinemia, hypogammaglobulinemia, elevated plasma renin activity and hyperaldosteronemia. Immune function was normal.

We report a case with locking of the metacarpophalangeal (MP) joint of the thumb in a 15-year-old high school baseball catcher, which was caused by an intra-articular loose body arising from osteochondritis dissecans. The loose body was removed arthroscopically, enabling early return to full MP joint function.

Background: Satisfactory internal fixation of comminuted radial head fractures is often difficult to achieve, and radial head resection has been the accepted treatment. In this study, we compared the results of radial head resection with those of open reduction and internal fixation in patients with a comminuted radial head fracture.

Methods: Twenty-eight patients with a Mason type-III radial head fracture (some with associated injuries) were enrolled in the study.

Sympathetic skin responses: the influence of electrical stimulus intensity and habituation on the waveform.

Clin Auton Res

Dept. of Rehabilitation Medicine, Tokai University Oiso Hospital, 21-1 Gakkyo, Oisomachi, Nakagun, Kanagawa, 259-0198, Japan.

Repeated stimulation of the sympathetic skin response (SSR) causes habituation. The aim of this study was to determine the effects of electrical stimulus intensity on SSR in 40 healthy controls (mean age+/-SD, 28.0+/-6.

Attentional disturbance following brain damage has been commonly evaluated by having the patients perform neuropsychological tasks. In rehabilitation settings, however, the primary concern is not task performance, but functional real-world behaviors. The aim of this study was to investigate the reliability and validity of a newly developed system for rating attentional behavior (Behavior Assessment of Attentional Disturbance, BAAD).

Oxidized low-density lipoprotein cholesterol (Ox-LDL) is considered to play a critical role in the pathogenesis of atherosclerosis. We investigated the role of malondialdehyde-modified LDL (MDA-LDL), an indicator of Ox-LDL, in cerebral infarction. We also investigated the relationship between MDA-LDL and atherosclerotic change of the carotid artery.

Objective: Using magnetic resonance imaging (MRI), sequential information regarding the dynamic movement of the cerebrospinal fluid (CSF) from the subarachnoid space and the ventricles to the drainage pathway in response to the CSF pressure was obtained in guinea pigs. In this study, a new mechanical hydrocephalus model in a guinea pig was developed to investigate the CSF kinetics in an acute hydrocephalic brain.

Materials And Methods: A total of 18 adult male guinea pigs were studied.

April 2005

Department of Medicine, Tokai University Oiso Hospital, 21-1 Gakkyo, Oiso-machi, Naka-gun, Kanagawa 259-0198, Japan.

The objective of this study was to compare respiratory motions of the chest wall in the healthy elder subjects (N = 5; mean age, 71 years old) with those in healthy young subjects (n = 9; mean age, 29 years old). Thirty sequential images (scanning time, 0.4s per image) of dynamic MRI on sagittal and coronal planes were obtained while the subjects were deeply breathing.

Development of a new mouth opening force test using an indirect cervical traction device.

Tokai J Exp Clin Med

April 2005

Department of Rehabilitation Medicine, Tokai University Oiso Hospital, 21-2 Gakkyo, Oiso-machi, Naka-gun, Kanagawa 259-0198, Japan.

Background And Aims: There have been few reports of objective jaw opening tests, and such studies have involved devices specifically designed for research. In the present study, in order to conveniently and objectively assess mouth opening movements, we replaced the manual resistance used in Daniels and Worthingham's muscle test (DMT) with an indirect cervical traction device. We examined whether the maximum mouth opening force (MOF) could be reliably quantified using this device.

Many studies have shown a close relationship between the detection of occult neoplastic cells (ONCs) freely floating in lymph node sinuses remote from the primary tumor and the recurrence/metastasis of various malignancies. If 5 to 10 circulating ONCs form an aggregate of tumor cells, it may easily escape attack by the host immune system. Since such malignant micro-aggregates cannot pass through the circulation in the liver and lungs, which filter tumor cells from the blood, the aggregates may become anchored in these organs and cause metastasis.
