Article Synopsis
  • The study reviews existing tools (PSI, A-DROP, I-ROAD, CURB-65) used to predict mortality in patients with healthcare-associated pneumonia (HCAP) and their effectiveness is debated.
  • Researchers conducted a systematic review and meta-analysis of 21 articles, analyzing various statistical measures (like sensitivity and specificity) for these tools.
  • Findings indicate that none of the tools have strong predictive abilities for mortality in HCAP, and there are no significant differences in their predictive performance.

  • The study aimed to investigate how biological and targeted synthetic DMARDs (b/tsDMARDs) affect bone metabolism in patients with rheumatoid arthritis (RA) over a 52-week period.
  • A total of 1164 RA patients were monitored, revealing that those not on anti-osteoporotic agents (anti-OP) experienced a significant decline in bone mineral density (BMD) despite improved disease activity, while those on anti-OP maintained stable BMD.
  • The findings suggest that osteoporosis may worsen in RA patients not receiving anti-OP during b/tsDMARD treatment, indicating the need for regular BMD monitoring for early osteoporosis management.

We herein report a case of a patient with gastric cancer-associated microscopic polyangiitis (MPA) who was treated with combination glucocorticoids and rituximab (RTX) for remission induction and maintenance, and finally to discontinue glucocorticoids without recurrence of gastric cancer or MPA in a year. A 69-year-old man was suspected of having MPA because of fever, high C-reactive protein levels, neuritis, and a high titer of myeloperoxidase-anti-neutrophil cytoplasmic antibody (MPO-ANCA). Upper gastrointestinal endoscopy indicated early-stage gastric cancer, for which he underwent surgery preceded by immunosuppressive therapy for vasculitis.

The First Department of Internal Medicine, School of Medicine, University of Occupational and Environmental Health, 1-1 Iseigaoka, Yahata-nishi-ku, Kitakyushu, Fukuoka, 807-8555, Japan.

Background: Improvements in the treatment of rheumatoid arthritis (RA) have made it possible to achieve treatment goals. It has been reported that both residual synovitis caused by RA and the patients' subjective symptoms remain even after achieving the treatment goals; however, there are limited reports showing a relationship between them. Furthermore, no studies have evaluated the relationship between patient-reported outcomes (PROs) and subclinical synovitis measured by musculoskeletal ultrasonography (MSUS) in the treatment of RA.

Article Synopsis
  • A study in Japan looked at the effectiveness of voluntary vaccines for rotavirus and mumps, but faced challenges due to lack of government data on vaccination rates.
  • The researchers found that vaccine coverage increased from 2015 to 2018, with rotavirus-related hospitalizations dropping significantly, while mumps cases stayed the same.
  • The study concluded that higher vaccination rates are needed to control infections, suggesting that these vaccines should be added to the national immunization program to improve coverage.

Psoriatic arthritis (PsA) manifests not only skin lesion but also peripheral and axial joint involvements, thus the disease is considered to be part of spondyloarthritis. In patients with active spondyloarthritis, tumour necrosis factor inhibitors biologics are often considered, but there is not sufficient evidence about other type of biologics. Ixekizumab (IXE) is a humanised monoclonal antibody, which acts against interleukin-17A, a cytokine involved in psoriasis pathogenesis.
