Article Synopsis
  • This study investigated the changes in high-energy phosphate compounds in heart grafts stored under cold conditions, using the Celsior solution for preservation.
  • Seven discarded heart grafts were monitored over 9 hours with Magnetic Resonance Spectroscopy to track levels of phosphocreatine (PCr), adenosine triphosphate (ATP), inorganic phosphate (Pi), and intracellular pH at regular intervals.
  • The findings indicated that while ATP levels remained stable, both PCr and Pi concentrations decreased over time, suggesting that monitoring these metabolites could help improve strategies for extending the ischemic time of heart grafts before transplantation.

Article Synopsis
  • The study investigates the effects of immediate and delayed increases in mean gradient elevation after transcatheter aortic valve implantation (TAVI), using data from the FRANCE 2 registry involving 4201 patients with severe aortic stenosis.
  • Three patient groups were analyzed based on their post-procedural mean transvalvular gradient (PPMG) and delayed mean gradient increase (DMGI): control (normal gradients), Group 1 (normal with delayed increase), and Group 2 (elevated gradients).
  • Findings revealed that patients with PPMG ≥ 20 mmHg experienced significantly higher 4-year mortality compared to controls, while those with PPMG < 20 mmHg developed DMGI showed similar outcomes

Ischemic spinal cord injury remains a major complication of both open and endovascular repairs of extensive lesions of the thoracic or thoracoabdominal aorta. Patients undergoing endovascular treatment cannot benefit from direct revascularization of the Adamkiewicz artery (AA). Primary revascularization of the intercostal artery (ICA) giving rise to the AA using the internal mammary artery (IMA) could ensure uninterrupted flow in the AA even if the origin of the feeding ICA was obstructed.

Comparison between PET and PET/CT in recurrent head and neck cancer and clinical implications.

Eur Arch Otorhinolaryngol

May 2007

Department of Head and Neck Surgery and Federation of Otolaryngology, La Timone University Hospital Center, Marseille Cedex 05, France.

The aim of this retrospective study was to compare the diagnostic accuracy between positron emission tomography (PET) and combined PET/computed tomography (PET/CT) in the detection of recurrent head and neck squamous cell carcinoma (HNSCC) and to evaluate the degree of interobserver agreement. Thirty-two patients who had undergone curative treatment for HNSCC and who presented with a suspicion of recurrent local disease were studied with fluoro-2-deoxy-D-glucose (FDG)-PET imaging. All patients had undergone an inconclusive conventional workup (nasofibroscopy, CT scan and/or MRI).

Cervical paragangliomas: neurovascular surgical risk and therapeutic management.

Eur Arch Otorhinolaryngol

September 2006

Department of Head and Neck Surgery, Federation of Otolaryngology, La Timone University Hospital Center, 264, rue Saint-Pierre, 13385 Marseille Cedex 05, France.

Paragangliomas of the neck region arise most commonly in the carotid and vagal bodies. The goal of this retrospective study is to evaluate intraoperative vascular and neurological morbidity and to define the therapeutic strategy. During the period 1990-2004, 32 patients with 42 neck paragangliomas were referred to our institution (Head and Neck Service, Otolaryngology Federation, CHU La Timone, Marseilles, France).

Objective: Brainstem metastasis is an uncommon complication of systemic cancer, generally considered to have a highly unfavorable prognosis. Surgical risks are high and standard radiation or chemotherapy have little effect. The purpose of this study is to evaluate our experience using Gamma Knife radiosurgery (GKRS) for the management of brainstem metastasis.

The purpose of this study was to demonstrate that severe multipedicular lesions involving supraaortic trunks cause compromised cerebral hemodynamics with nonhemispheric symptoms (NHS) that can be relieved by surgical treatment. A total of 11 patients were prospectively included in the study. Regional cerebral blood flow (rCBF) and cerebral blood flow reactivity (CBFR) were measured by acetazolamide single photon emission computed tomoscintigraphy scans (SPECT) before and after surgery.

Recurrence is a major problem after release of temporomandibular joint ankylosis. Early physiotherapy and choice of interpositional material are important in preventing recurrence. Currently, the most used technique is gap arthroplasty associated with coronoidectomy, temporalis muscle flap interposition and reconstruction of the condylar unit with a costochondral graft.

Silent myocardial ischemia in patients with diabetes: who to screen.

Diabetes Care

September 1999

Nutrition-Endocrinology-Metabolic Disease Department, Timone University Hospital Center, Marseille, France.

Objective: Silent myocardial ischemia (SMI) is more common in diabetic patients than in the general population. However, the exact prevalence of SMI is not known, and routine screening is costly. The purpose of this 1-year study was to estimate the prevalence of SMI and define a high-risk diabetic population by systematically testing patients with no symptoms of coronary artery disease (CAD).
