  • Cerebrovascular events (CVEs) can happen during or after a procedure called TAVR, which replaces a heart valve, and can lead to serious health problems.
  • Even though strokes have become less common since TAVR started, they’re still a major concern because TAVR is being used more often.
  • New devices and treatments are being tested to help prevent CVEs, but current options may not reduce stroke risk enough, and there are concerns about bleeding from the medication used.

Vitamin B (cobalamin; B) is an essential micronutrient, but deficiency is common. The prompt diagnosis and treatment of B deficiency protects against megaloblastic anaemia, neuropathy and neuropsychiatric changes. Biomarkers of B status include the measurement of serum B (also known as total B or serum cobalamin), holotranscobalamin (holoTC or 'active B12'), methylmalonic acid (MMA) and total plasma homocysteine (Hcy).

Article Synopsis
  • Cardiac resynchronization therapy (CRT) using biventricular (BIV) pacing is aimed at heart failure patients but some don't respond; MultiPoint Pacing (MPP) might offer better results for these non-responders.
  • In a study with 3724 patients treated with BIV, 1639 were identified as non-responders and some were randomized to MPP or continued BIV pacing.
  • Results showed MPP led to better outcomes with a 33% success rate in reducing heart failure events compared to 23.5% in BIV, and MPP also resulted in fewer hospitalizations for heart failure.

November 2024

Department of Gastroenterology, St Thomas' Hospital, London, UK.

Article Synopsis
  • The treatment of ulcerative colitis (UC) focuses on reducing inflammation, but some patients struggle with current therapies; mirikizumab, an anti-IL-23 antibody, has been approved as a new option for those with moderate to severe UC.
  • This review discusses the role of IL-23 in inflammation and details the pharmacokinetics and clinical evidence supporting mirikizumab's efficacy and safety through phase II and III trials.
  • Mirikizumab has shown to be more effective than placebo and is a viable option for both new and experienced patients, with flexible dosing options to help those who don't respond well to initial treatment.

Introduction: Pediatric patients have an increased risk of radiation-induced malignancies due to their ongoing development and long remaining life span. Thus, optimization of PET protocols is an important task in pediatric nuclear medicine. Long axial field-of-view (LAFOV) PET/CT has shown a significant increase in sensitivity, which provides an ideal opportunity for reduction of injected tracer activity in the pediatric population.

Renal artery occlusion is a rare but potentially catastrophic complication of paediatric endovascular renal artery intervention. Emergency auto-transplantation may be required to salvage the kidney; to date this has only been described in adults. We report our experience of performing emergency kidney auto-transplantation following acute renal artery thrombosis in a child undergoing redo renal artery angioplasty A 20-month-old boy presented with refractory hypertension and hypertensive cardiomyopathy secondary to multifocal fibromuscular dysplasia (FMD) with a single functioning kidney.

Background: Feeding intolerance is common in critically ill patients and can lead to malnutrition. Prokinetic agents may be used to enhance the uptake of nutrition. However, the evidence on the effectiveness and safety of prokinetic agents is sparse, and there is a lack of data on their use in intensive care units (ICU).

Electronic address:

Cyclodestruction is a technique reserved for glaucomas not controlled with medical and surgical treatment and poor visual potential. During the last century, new cyclodestructive techniques were developed, including cyclophotocoagulation, and the use of continuous-wave diode laser transescleral cyclophotocoagulation (CW-TS-CPC) has become widespread. In recent decades, micropulse diode laser transescleral cyclophotocoagulation (MP-TS-CPC) was introduced.

Effect of Fibroblast Growth Factor (FGF) 19 and 21 on Hip Geometry and Strength in Post-menopausal Osteoporosis (PMO).

Calcif Tissue Int

November 2024

Department of Chemical Pathology/Metabolic Medicine, North Wing, St Thomas' Hospital, Lambeth Palace Road, London, SE1 7EH, UK.

Article Synopsis
  • FGF receptors play a key role in bone development and metabolism, with FGF19 and FGF21 subfamily members impacting bone strength and mineral density, but their specific effects are still unclear.
  • A study of 374 post-menopausal women showed no significant links between serum levels of FGF19 and 21 with bone mineral density (BMD) or fracture prevalence after considering factors like age and BMI.
  • However, higher levels of FGF21 were associated with negative impacts on hip geometry and strength measurements, suggesting it could be harmful to bone health, warranting further research, especially as FGF21 may be explored for obesity treatments.

Sacral osteomyelitis is an infection of the bone that extends posterior to the pelvis at the base of the spine. This condition typically occurs in elderly or bedbound/immobile patients and is treated with wound debridement, wound care, surgery, and antibiotic therapy. In this report, a case is presented of a rare complication of sacral osteomyelitis where the infection extended through the bone into the spinal canal causing an ascending spinal canal infection.
