Background: Acute psychological stress can provoke mental stress-induced myocardial ischemia (MSIMI) in coronary artery disease (CAD). Stromal cell-derived factor 1 (SDF1) is released in response to hypoxia, and higher levels of SDF1 are associated with adverse outcomes. We examined whether an increase in SDF1 level in response to mental stress predicts adverse outcomes in CAD patients.

Chronic lymphocytic leukemia (CLL) is a lymphoproliferative malignancy that is categorized by the production and accumulation of CD5+ monoclonal B cell lymphocytes, commonly in the spleen, bone marrow, and peripheral blood; these are morphologically mature lymphocytes with abnormal immune function. Ascites, although common in solid organ malignancies such as ovarian, breast, and gastrointestinal, is a rare clinical manifestation in hematological malignancies. The case presented herein describes an elderly male patient with CLL who presented with transudative ascites 7 years after the completion of chemotherapy.

Article Synopsis
  • - Mutations in the VAPB gene, linked to familial ALS, induce changes in the structure of motor neuron connections, leading to larger but fewer presynaptic boutons at the neuromuscular junction (NMJ).
  • - The morphological changes are tied to reduced microtubule stability due to hyperactivation of CaMKII caused by elevated calcium levels, which result from impaired calcium extrusion in neurons due to insufficient energy production.
  • - The study highlights a potential bioenergetic dysfunction in ALS-related motor neurons, where disrupted mitochondrial ATP production leads to an inability to meet the energy demands of neuronal activity, contributing to synaptic defects.

Shoulder Dystocia: Challenging Basic Assumptions.

Obstet Gynecol Clin North Am

September 2022

Department of OB/GYN, Division of Maternal/Fetal Medicine, Wellspan Health System York PA, 21636 Ripplemead Drive, Laytonsville, MD 20882, USA. Electronic address:

Most of our knowledge pertaining to this obstetric emergency has emanated from case reports and retrospective studies that have subsequently resulted in empirical management protocols. This article has identified the existence of large gaps in our clinical knowledge base regarding the prevention and resolution of shoulder dystocia, as well as its long-term sequelae. We have attempted to challenge current recommendations regarding whether prophylactic cesarean delivery should be performed based on estimated fetal weight alone or a prior history of shoulder dystocia, shoulder dystocia management techniques, what defines "excessive" traction, and the role of simulation training for all clinicians.

Adipocyte mesenchymal transition contributes to mammary tumor progression.

Cell Rep

September 2022

Touchstone Diabetes Center, Department of Internal Medicine, The University of Texas Southwestern Medical Center, Dallas, TX 75390, USA. Electronic address:

Obesity is associated with increased cancer incidence and progression. However, the relationship between adiposity and cancer remains poorly understood at the mechanistic level. Here, we report that adipocytes from tumor-invasive mammary fat undergo de-differentiation to fibroblast-like precursor cells during tumor progression and integrate into the tumor microenvironment.

Obesity is associated with increased prostate cancer (PCa) progression and higher mortality, however, the mechanism(s) remain still unclear. Here, we investigated signaling by the ASC-secreted chemokine CXCL12 in a mouse allograft model of PCa and in HiMyc mice in the context of diet-induced obesity. Treatment of mice with CXCR4 antagonist inhibited CXCL12-induced signaling pathways, tumor growth and EMT in HMVP2 allograft tumors.
