Extracorporeal shock wave lithotripsy (SWL) can be used in conjunction with endoscopic retrograde cholangiopancreatography (ERCP) for the treatment of large (≥5 mm) or impacted pancreatolithiasis. We report our experience demonstrating the safety and effectiveness of urologist-directed SWL for pancreatolithiasis. We performed a retrospective review of all patients undergoing SWL for pancreatolithiasis at our institution between 2016 and 2020.

Article Synopsis
  • European laparoscopic urological skills (LUSs2) curriculum is being developed to fill the gap in intermediate and advanced training for laparoscopic surgical procedures in urology, promoting uniformity and proficiency among practitioners.
  • A Delphi survey with international experts helped create this curriculum; it achieved consensus on 85% of the evaluated statements regarding crucial laparoscopic procedures.
  • The new curriculum aims to enhance surgical education standards, improve surgical skills, and ultimately provide better patient care, although some limitations and potential biases in the process were noted.

Article Synopsis
  • Nephroureterectomy is the standard treatment for high-grade upper tract urothelial carcinoma (UTUC), but exceptions may exist based on individual patient factors.
  • A case is presented where a patient with horseshoe kidney and significant kidney issues was successfully treated using a minimally invasive, nephron-sparing method instead of the standard approach.
  • This highlights that current treatment guidelines for UTUC may not fit every patient scenario and that personalized treatment plans should be considered in specialized medical centers.

Aims: Urogynecology and Reconstructive Pelvic Surgery (URPS) fellowship can be pursued after completion of either a urology (URO) or obstetrics and gynecology (GYN) residency. Our aim is to determine differences in graduating fellow cohort (GFC) case logs between URO- and GYN-based URPS programs.

Methods: Accreditation Council for Graduate Medical Education case logs for URPS GFCs in both GYN- and URO-based programs were analyzed for the 2019-2023 academic years (AY).

Extrapolations from the adult population have suggested that opioids should be avoided in the management of pediatric urolithiasis, but the literature is sparse with regards to actual practice patterns and the downstream implications. We sought to investigate the rate of oral opioid administration for children presenting to the emergency room (ER) with urolithiasis and to identify associations between opioid administration and return visits and persistent opioid use. The TriNetX Research and Diamond Networks were used for retrospective exploratory and validation analyses, respectively.

Percutaneous nephrolithotomy (PCNL) is the gold-standard treatment for large and complex renal stones. Though associated with higher stone-free rates compared to other minimally invasive stone procedures, this comes at the expense of increased morbidity including postoperative pain and discomfort. We describe our enhanced recovery after surgery (ERAS) protocol for PCNL with emphasis on the use of erector spinae plane blocks to improve patient satisfaction and reduce postoperative opioid use and bother.

Background: Intravesical instillation of analgesic and anticholinergic drugs have shown efficacy in the treatment of pain and voiding symptoms. Unfortunately, drug loss with urination and dilution in the bladder limit their durability and clinical usefulness. We have recently developed and tested in vitro, a sustained delivery system (TRG-100) of fixed-dose combination of lidocaine and oxybutynin designed to allow for a longer exposure of the urinary bladder to the drugs.
