Article Synopsis
  • The study explored the association between a 70-gene risk score and the likelihood of locoregional recurrence (LRR) in early-stage breast cancer patients who underwent breast-conserving surgery (BCS).
  • Out of 6,693 patients, 5,470 had BCS, with an 8-year LRR incidence of 3.2%, and a lower 2.7% for those with a low-risk 70-gene signature.
  • The findings suggest that while the 70-gene signature is not an independent predictor for LRR at this time, the overall low recurrence rate indicates potential for reducing the intensity of local therapy in future trials.

Background: This study aims to (1) investigate the prevalence of anxiety, depression and severe fear of cancer recurrence or progression in gastrointestinal stromal tumor (GIST) patients treated in a curative or palliative setting, (2) compare their prevalence with a norm population, (3) identify factors associated with anxiety, depression and severe fear, and (4) study the impact of these psychological symptoms on health-related quality of life (HRQoL).

Methods: In a cross-sectional study, GIST patients completed the Hospital Anxiety and Depression Scale, Cancer Worry Scale, and EORTC QLQ-C30.

Results: Of the 328 patients, 15% reported anxiety, 13% depression, and 43% had severe fear.

Introduction: The European Association of Urology (EAU) and the American Society of Clinical Oncology (ASCO) recently issued updated guidelines on penile cancer, emphasising dynamic sentinel node biopsy (DSNB) as the preferred method for surgical staging among patients with invasive penile tumours and no palpable inguinal lymphadenopathy. This paper outlines the rationale behind this new recommendation and describes remaining challenges, as well as strategies for promoting DSNB worldwide.

Main Text: DSNB offers high diagnostic accuracy with the lowest postoperative complications compared to open or minimally invasive inguinal lymph node dissection (ILND), prompting its preference in the new guidelines.

Uncertainty remains about the best level of intraoperative positive end-expiratory pressure (PEEP). An ongoing RCT ('DESIGNATION') compares an 'individualized high PEEP' strategy ('iPEEP')-titrated to the lowest driving pressure (ΔP) with recruitment maneuvers (RM), with a 'standard low PEEP' strategy ('low PEEP')-using 5 cm HO without RMs with respect to the incidence of postoperative pulmonary complications. This report is an interim analysis of safety and feasibility.

Non-therapeutic plasma levels in individuals utilizing curcumin supplements in daily life.

Front Nutr

November 2023

Department of Pharmacy and Pharmacology, Amsterdam UMC location AMC, Amsterdam, Netherlands.

Introduction: The spice curcumin and its metabolites are widely used by cancer patients but have not shown proven health benefits in clinical studies, likely due to low plasma concentrations after oral intake. However, public interest in curcumin continues to grow, and companies claim enhanced absorption in their formulations. This study aims to determine if daily oral intake of curcumin leads to sufficient plasma concentrations for health effects.

The majority of patients with resected stage II-IIIA non-small cell lung cancer (NSCLC) are treated with platinum-based adjuvant chemotherapy (ACT) in a one-size-fits-all approach. However, a significant number of patients do not derive clinical benefit, and no predictive patient selection biomarker is currently available. Using mass spectrometry-based proteomics, we have profiled tumour resection material of 2 independent, multi-centre cohorts of in total 67 patients with NSCLC who underwent ACT.

Extragonadal androgens play a pivotal role in prostate cancer disease progression on androgen receptor signaling inhibitors (ARSi), including abiraterone and enzalutamide. We aimed to investigate if germline variants in genes involved in extragonadal androgen synthesis contribute to resistance to ARSi and may predict clinical outcomes on ARSi. We included ARSi naive metastatic prostate cancer patients treated with abiraterone or enzalutamide and determined 18 germline variants in six genes involved in extragonadal androgen synthesis.

Purpose: To assess the effects of contemporary treatment of ductal carcinoma in situ (DCIS) on the risk of developing an ipsilateral invasive breast cancer (iIBC) in the Dutch female population.

Methods: Clinical data was obtained from the Netherlands Cancer Registry (NCR), a nationwide registry of all primary malignancies in the Netherlands integrated with the data from PALGA, the Dutch nationwide network and registry of histo- and cytopathology in the Netherlands, on all women in the Netherlands treated for primary DCIS from 2005 to 2015, resulting in a population-based cohort of 14.419 women.

Objective: Correct referral of women with an ovarian tumor to an oncology department remains challenging. The International Ovarian Tumor Analysis (IOTA) consortium has developed models with higher diagnostic accuracy than the alternative Risk of Malignancy Index (RMI). This study explores the uptake of the IOTA models in Dutch hospitals and factors that impede or promote implementation.

Article Synopsis
  • Cisplatin-based neoadjuvant chemotherapy (NAC) is commonly used in muscle-invasive bladder cancer patients but can lead to renal toxicity, and radical cystectomy (RC) itself is a risk factor for renal injury.
  • An analysis of 234 patients showed a significant decrease in estimated glomerular filtration rate (eGFR) after receiving NAC, with a 17% decline and an increase in chronic kidney disease stage ≥3 from 27% to 50% one year after surgery.
  • In comparison, a control group that underwent RC without NAC had a smaller eGFR decline of 6% and a baseline higher incidence of stage ≥3 CKD, highlighting the renal risks associated with NAC prior to surgery

The value of sentinel lymph node biopsy (SLNB) for LMM patients remains to be determined, as the reported average yield of positive lymph nodes is less than 10%. In this nationwide cohort study, we wanted to identify LMM patients with an increased risk of SLNB-positivity.

Nivolumab plus ipilimumab in advanced salivary gland cancer: a phase 2 trial.

Nat Med

December 2023

Head and Neck Service and Immunogenomic Oncology Platform, Department of Surgery, Memorial Sloan Kettering Cancer Center, New York, NY, USA.

Article Synopsis
  • Salivary gland cancers (SGCs) are rare and aggressive, often lacking effective treatments when they spread, prompting a phase 2 trial of nivolumab and ipilimumab in 64 metastatic SGC patients.
  • Results showed some success in "other SGCs" cohort (16% response) but limited efficacy in adenoid cystic carcinoma (6% response), with notable adverse events occurring in 38% of patients.
  • Genetic and immune cell analyses indicated that responding tumors had active T cell responses and certain neoantigens, suggesting a potential path for treatment in non-ACC SGCs like salivary duct carcinomas.
