Hydatid disease is endemic in Tunisia. Whereas uncomplicated pulmonary hydatid cysts are easily diagnosed on radiological findings, complicated and atypical forms may be misdiagnosed and confused with other pulmonary lesions, mainly lung malignancies. We report a case of a 47-year-old woman, who presented with a 3-month history of hemoptysis.

Introduction: hydatid disease, caused by Echinococcus granulosus, primarily affects the liver and lungs, with splenic involvement being exceedingly rare, occurring in only 0.5 % of cases. This case report highlights a 17-year-old male presenting with a complicated hydatid cyst in the lung, along with an incidental splenic cyst, emphasizing the importance of single-stage surgery in treating multi-organ hydatid disease.

Introduction: The lung is the second most commonly affected organ by hydatidosis, and the bilateral involvement is rare even in endemic regions.

Case Presentation: We report the case of a 27-year-old patient who presented with right basithoracic pain and cough evolving for three months. Thoracic CT scan revealed two homogeneous, rounded cystic formations enhancing after injection of the contrast media, located in the lateral basal segments of the lower lobe.
