Mapping of functional erectogenic nerves on the rat prostate.

J Sex Med

December 2024

The James Buchanan Brady Urological Institute and Department of Urology, Johns Hopkins University School of Medicine, Baltimore, MD 21287, United States.

Background: Preservation of erectogenic nerves during radical prostatectomy (RP) is hampered by limited understanding of their precise localization, due to their complex, intertwined paths, and the inherent individual variations across patients. Because erection utilizes a subset of cavernous nerves (CNs) that in response to sexual stimuli reveal phosphorylation of neuronal nitric oxide synthase (nNOS) on its stimulatory site Ser-1412, we hypothesized that delineation of nerves containing phosphorylated (P)-nNOS on Ser-1412 would establish the location of functional erectogenic nerves within the periprostatic region.

Aim: To identify the distribution and quantity of functional erection-relevant ([P-nNOS]-containing) nerves in the periprostatic area and discriminate them among the CNs distribution.

Priapism, a prevalent complication in sickle cell disease (SCD) patients, manifests as prolonged and painful erections unrelated to sexual arousal. The detailed mechanisms contributing to this condition, especially regarding sympathetic function in the corpus cavernosum that maintains penile flaccidity, remain to be elucidated. In this study, it was hypothesized that the pathways of the sympathetic nervous system would be down-regulated, thereby contributing to the development of ischemic priapism in sickle cell disease.

Introduction: Performing surgery for renal cell carcinoma (RCC) in patients with horseshoe kidneys presents unique anatomic challenges. The complexities of clinical decision-making and technical challenges are further compounded by the presence of both IVC tumor thrombus extension and metastatic disease.

Case Presentation: We encountered the unique case of a 66-year-old woman with a horseshoe kidney who presented with a 14cm mass in her right kidney with level II tumor thrombus extension into the IVC, retroperitoneal lymphadenopathy, and pulmonary nodules that were biopsy-proven to be consistent with metastatic clear cell RCC (cT3aN1M1).

Purpose Of Review: To describe patient experiences of transurethral resection of bladder tumor (TURBT) and review recent advances in enhancing clinical outcomes.

Recent Findings: High rates of recurrence and progression of non-muscle invasive bladder tumors expose patients to multiple TURBT procedures throughout their disease process. Understanding the impact of TURBT on quality of life and patient experiences is crucial for shared decision-making, thus enhanced recovery protocol trials are being explored to improve patient outcomes.

November 2024

Department of Chemical and Biomolecular Engineering, Johns Hopkins University, Baltimore, MD, 21218, USA.

Untethered microrobots offer the possibility to perform medical interventions in anatomically complex and small regions in the body. Presently, it is necessary to access the upper urinary tract to diagnose and treat Upper Tract Urothelial Carcinoma (UTUC). Diagnostic and treatment challenges include ensuring adequate tissue sampling, accurately grading the disease, achieving completeness in endoscopic treatment, and consistently delivering medications to targeted sites.

Article Synopsis
  • Indeterminate renal masses are often discovered incidentally through imaging, and Tc-sestamibi scans can help differentiate between oncocytomas and certain types of renal cell carcinomas, particularly chRCC, which may often be misclassified as false positives.
  • A review of 18 patients with solitary tumors revealed that 72.2% of the tumors remained classified as chRCC, though some were reclassified into different categories, indicating a potential for misinterpretation in imaging.
  • The findings suggest that the false-positive rate for chRCC on renal sestamibi scans may have been exaggerated, highlighting the need for further research to improve non-invasive assessment of indeterminate renal masses.

Urol Oncol

September 2024

The James Buchanan Brady Urological Institute and Department of Urology, Johns Hopkins University School of Medicine, Baltimore, MD; Department of Oncology, The Johns Hopkins University School of Medicine, Baltimore, MD. Electronic address:

Article Synopsis
  • - The study aimed to analyze survival rates and genetic factors in testis cancer specifically among Hispanic men, using data from a national cancer registry between 2000 and 2020.
  • - Results showed that Hispanic patients were younger at diagnosis but had more advanced disease, leading to poorer cancer-specific survival for non-seminomatous germ cell tumors compared to Non-Hispanic Whites, while survival rates for seminomas were similar.
  • - Genetic analysis indicated that while there were no significant differences in mutation frequencies between ethnic groups, there was diverse ancestral background among Hispanic patients, highlighting variations in genetic heritage.

Laboratory of Pharmacology, São Francisco University Medical School, Bragança Paulista, SP, Brazil (D.A.P., F.H.S.); Universidade Federal de São Paulo, Escola Paulista de Medicina, Department of Pharmacology, São Paulo, SP, Brazil (F.B.C.); Hematology and Hemotherapy Center, University of Campinas, Campinas, SP, Brazil (F.F.C.); and The James Buchanan Brady Urological Institute and Department of Urology, The Johns Hopkins School of Medicine, Baltimore, Maryland (A.L.B.)

Patients with sickle cell disease (SCD) display priapism, a prolonged penile erection in the absence of sexual arousal. The current pharmacological treatments for SCD-associated priapism are limited and focused on acute interventions rather than prevention. Thus, there is an urgent need for new drug targets and preventive pharmacological therapies for this condition.

Article Synopsis
  • Patients with sickle cell disease (SCD) have high levels of a substance called heme, which may cause painful erections known as priapism.
  • The study looked at how heme affects smooth muscle relaxation in mouse penis tissue, discovering that heme helps these muscles relax.
  • Blocking certain pathways stopped this relaxing effect, suggesting that targeting heme could lead to new treatments to prevent priapism in men with SCD.
