Current global status of male reproductive health.

Hum Reprod Open

April 2024

Instituto de Biología y Medicina Experimental, Consejo Nacional de Investigaciones Científicas y Técnicas de Argentina-Fundación IBYME, Buenos Aires, Argentina.

Background: The widespread interest in male reproductive health (MRH), fueled by emerging evidence, such as the global decline in sperm counts, has intensified concerns about the status of MRH. Consequently, there is a pressing requirement for a strategic, systematic approach to identifying critical questions, collecting pertinent information, and utilizing these data to develop evidence-based strategies. The methods for addressing these questions and the pathways toward their answers will inevitably vary based on the variations in cultural, geopolitical, and health-related contexts.

Potential risk of gestational diabetes mellitus in females undergoing in vitro fertilization: a pilot study.

Clin Diabetes Endocrinol

April 2024

Department of Internal Medicine; Unit of Diabetes Lipidology & Metabolism, Faculty of Medicine, Alexandria University, 17 Champollion Street Azarita, Alexandria, Egypt.

Article Synopsis
  • The study found that women who underwent in vitro fertilization (IVF) had a significantly higher incidence of gestational diabetes mellitus (GDM) compared to those who conceived spontaneously.
  • At week 28, the incidence of GDM was 20% in the IVF group versus 5.5% in the spontaneous conception (SC) group, with a p-value of 0.022 indicating statistical significance.
  • Early screening at week 20 also showed a higher GDM occurrence in the IVF group (16.4%) compared to the SC group (3.6%), underscoring the need for vigilant screening practices for GDM in IVF pregnancies.

Background: Systematic reviews and meta-analyses are instrumental in shaping clinical practice. However, their findings can sometimes be marred by discrepancies and potential biases, thereby diluting the strength of the evidence presented. Umbrella reviews serve to comprehensively assess and synthesise these reviews, offering a clearer insight into the quality of the evidence presented.

Purpose: To reach a consensus among obstetrics and gynecology experts on the effects of micronutrient supplementation on fertility and pregnancy to aid clinicians in decision-making and create a unified approach to managing micronutrient deficiencies in women, by performing a modified Delphi study.

Methods: A three-round modified Delphi process was conducted among a Delphi panel of 38 Egyptian experts to define recommendations regarding the role of supplementation on fertility and pregnancy in women of reproductive age. A literature review was performed and supporting evidence was graded to help guide the recommendations based on available evidence.

Assisted reproductive technologies in Africa: The African Network and Registry for ART, 2018 and 2019.

Reprod Biomed Online

May 2023

Department of Obstetrics and Gynaecology, Groote Schuur Hospital and Faculty of Health Sciences, University of Cape Town, Cape Town, South Africa; African Network and Registry for Assisted Reproductive Technology.

Research Question: What were the utilization, practices and outcomes of assisted reproductive technology (ART) in Africa in 2018 and 2019?

Design: Cycle-based data (CBD) and retrospective summary data were collected cross-sectionally from voluntarily participating ART centres.

Results: During 2018, 43,958 ART procedures were reported by 67 centres in 16 countries, increasing to 45,185 procedures reported by 70 centres in 18 countries in 2019. Autologous fresh procedures predominated at 70%, whereas autologous frozen embryo transfers (FET) increased from 21.

This work aimed to determine the effect of cysteamine (25, 50, 100 and 200 μM) incorporated during dilution on frozen thawed buffalo semen quality. Semen was collected twice weekly for 7 consecutive weeks from three Egyptian buffalo bulls using an artificial vagina. Semen samples were pooled and extended with a Tris-based extender, cooled, equilibrated and finally frozen in liquid nitrogen.

Article Synopsis
  • In 2014, approximately 1.93 million assisted reproductive technology (ART) cycles were conducted in 76 countries, accounting for about 66% of global ART activity, with notable increases in success rates and the use of frozen embryo transfer (FET).
  • There was a continued rise in single embryo transfer (SET), which has helped decrease multiple birth rates, alongside a higher adoption of preimplantation genetic testing and stabilized use of intracytoplasmic sperm injection (ICSI).
  • The study highlighted the global disparities in ART practices and outcomes, emphasizing the importance of the International Committee for Monitoring Assisted Reproductive Technologies (ICMART) for tracking trends and providing relevant data to stakeholders.

The impact of religion and culture on medically assisted reproduction in the Middle East and Europe.

Reprod Biomed Online

September 2021

International Islamic Center for Population Studies and Research, Al-Azhar University, Al-Darrasah Cairo, Egypt. Electronic address:

Infertility is a medico-socio-cultural problem associated with gender-based suffering. Infertility treatment, including assisted reproductive technology (ART), is a human right. Culture and religion were among the stumbling blocks to early acceptance of ART, particularly in the Middle East and to a lesser extent in Europe.

The unpredictable variability in patients' responses to gonadotropins represents one of the most intractable IVF treatment problems. Identifying the genetic variants associated with ovarian responses to gonadotropins is an important step towards developing individualised pharmacogenetics protocols for ovarian stimulation. The purpose of the current study was to evaluate correlations between FSHR rs6165, FSHR rs616, and ESR1 rs2234693 gene variants and the degree of ovarian response to gonadotropin in Egyptian women undergoing ICSI treatment.

Purpose: Frozen embryos transfer (ET) may improve the live-birth and reduce rates of ovarian hyperstimulation in polycystic ovary syndrome (PCOS) patients. Morphological criteria are the classical way for embryo selection, yet recently, many biochemical and genetic markers have been developed. This study aimed to compare fresh and frozen ET using the mtDNA/gDNA ratio of embryo secretome and the possibility of using this ratio as a predictive marker of PCOS pregnancy rate.

The uterine junctional zone represents the juncture between endometrium and myometrium. The junctional zone is hormonally dependent and displays continuous peristaltic activity throughout the menstrual cycle in the nonpregnant state which is concerned with sperm transport and embryo implantation. Peristalsis may be observed using various invasive and noninvasive modalities, of which ultrasound is the most readily applied in the clinical setting.

How and Why to Define Unexplained Infertility?

Semin Reprod Med

January 2020

Department of Obstetrics and Gynecology, Kasr Al-Ainy Hospital, Faculty of Medicine, Cairo University, Cairo, Egypt.

Unexplained infertility represents up to 30% of all cases of infertility. It is a diagnosis of exclusion, where no cause for infertility may be identified in the investigation of the couple, be it anovulation, fallopian tube blockage, or severe male factor. Unexplained infertility therefore cannot be considered a diagnosis to which a specific treatment is directed, rather that it indicates a failure to reach a diagnosis of the true cause of infertility.

Article Synopsis
  • In 2012, there was a nearly 20% increase in the total number of assisted reproductive technology (ART) cycles compared to 2011, with notable trends in frozen embryo transfers, oocyte donation, and single embryo transfers.
  • Although ART usage and techniques evolved, pregnancy and delivery rates remained stable while the number of multiple deliveries decreased.
  • The data was gathered from 69 countries and 2600 clinics, indicating a global participation in ART practices, highlighting ongoing disparities in access and effectiveness across different regions.
