Outcomes of Orthopaedic Infections in Recreational Intravenous Drug Users Requiring Long-term Antibiotic Treatment.

J Am Acad Orthop Surg Glob Res Rev

June 2022

From the Department of Orthopaedic Surgery & Rehabilitation, University of Nebraska Medical Center (Dr. Stockwell, Pietrok, and Dr. Hartman); the Indiana Hand to Shoulder Center (Dr. Rinehart); the Department of Orthopaedic Surgery, University of Utah (Dr. Boes); the Department of Internal Medicine, University of Nebraska Medical Center (Dr. Hewlett); and the Cleveland Clinic Florida (Dr. Streubel).

Patients who participate in recreational injection drug use (RIVDU) have an increased risk of orthopaedic infections requiring prolonged treatment with intravenous antibiotics. This study reviews clinical outcomes and complications in RIVDU and have orthopaedic infections requiring long term antibiotic therapy (>4 weeks) and compares these outcomes to non-RIVDU patients. In this retrospective review, patients were divided into cohorts based on RIVDU history; the RIVDU cohort was further divided into subcohorts based on treatment location.

Endothelial junctional membrane protrusions serve as hotspots for neutrophil transmigration.

Elife

August 2021

Molecular Cell Biology Lab at Dept. Molecular Hematology, Sanquin Research and Landsteiner Laboratory, Amsterdam, Netherlands.

Upon inflammation, leukocytes rapidly transmigrate across the endothelium to enter the inflamed tissue. Evidence accumulates that leukocytes use preferred exit sites, alhough it is not yet clear how these hotspots in the endothelium are defined and how they are recognized by the leukocyte. Using lattice light sheet microscopy, we discovered that leukocytes prefer endothelial membrane protrusions at cell junctions for transmigration.

Background: Common bile duct injuries (BDI) during laparoscopic cholecystectomy (LC) continue to be the source of morbidity and mortality. The reason for BDI is mostly related to the misidentification of the extrahepatic bile duct structures and the anatomic variability. Near-infrared fluorescent cholangiography (NIFC) has proven to enhance visualization of extrahepatic biliary structures during LCs.

Background: Since obtaining FDA approval for postsurgical pain management in 2011, a number of well-designed studies have reported favorably on the safety and efficacy of liposomal bupivacaine (LB). However, the literature lacks adequate reports of patient perception of postoperative pain and subjective satisfaction.

Methods: A telephone survey of patients who received LB injection at time of operation at a single institution was contacted.

The perioperative management of Parkinson's disease revisited.

Neurol Clin

May 2004

Department of Neurology, The Cleveland Clinic-Florida, Weston (Fort Lauderdale), FL 3331, USA.

The perioperative management of patients who have PD requires knowledge of the potential complications of various types of surgery. Neurologists should be able to balance patient comfort against the potential complications of antiparkinsonian therapy. The authors believe that the approach outlined in the Appendix increases patient comfort, facilitates nursing care, and can potentially reduce some of the complications seen in the postoperative period.

Association between apolipoprotein E alleles and calcific valvular heart disease.

Circulation

October 2003

Department of Cardiology, Desk A-23, The Cleveland Clinic Florida, 2950 Cleveland Clinic Blvd, Weston, Fla 33331, USA.

Background: Studies on apolipoprotein E (apoE) alleles have reported an increased risk of coronary heart disease in patients with the apoE4 allele. Given the risk factor and histological similarities between coronary and calcific valvular heart disease (aortic stenosis [AS] and mitral annular calcification [MAC]), we postulated that apoE alleles might be associated with the development of these valvular lesions.

Methods And Results: We evaluated the association between apoE alleles and calcific valvular lesions in 802 patients undergoing transthoracic echocardiography using logistic regression analyses.

Catechol-O-methyltransferase inhibitors in the management of Parkinson's disease.

Semin Neurol

September 2001

Parkinson's Disease Clinic and Movement Disorders Program, The Cleveland Clinic Florida, Fort Lauderdale 33309, USA.

Parkinson's disease is the most common neurodegenerative disease in which the chemical pathology is known and effective symptomatic treatment, levodopa, is available. Therapy in the initial years after initiation with dopa decarboxylase inhibitors, carbidopa or benserazide, combined with levodopa results in favorable, stable responses. However, by 5 years after the initiation of treatment, over two thirds of patients experience motor fluctuations beginning initially with a "wearing-off" effect followed by more complex fluctuations including dyskinesias and "on-off" responses.

Background: One of the difficulties associated with surgery for rectal villous tumors (RVT) is the finding of invasive adenocarcinoma after transanal excision (TAE) and the possible need for more radical procedures or adjuvant therapy. Improved preoperative evaluation may eliminate this dilemma. The aim of our study was to evaluate the role of transrectal ultrasound (TRUS) in establishing the correct diagnosis of RVT.
