Why are organisational approvals needed for low-risk staff studies in the UK? Procedures, barriers, and burdens.

BMC Health Serv Res

November 2024

International Observatory On End of Life Care, Division of Health Research, Lancaster University, Lancaster, LA1 4AT, UK.

Background: Health care staff should be given the opportunity to participate in research, but recruiting clinicians via their employing organisation is not always straightforward or quick in the UK. Unlike many countries outside the UK, very low-risk survey, interview or focus group studies can be subject to some of the same governance approval procedures as interventional studies. An exemplar study carried out by the NIHR funded Palliative Care Research Partnership North West Coast is used to highlight the challenges still faced by researchers and health care organisations when setting up a low-risk staff study across multiple NHS and non-NHS sites.

Liverpool Head & Neck Centre, Department of Molecular and Clinical Cancer Medicine, University of Liverpool, William Henry Duncan Building, 6 West Derby Street, Liverpool, L7 8TX, UK; Liverpool Head and Neck Centre, Liverpool University Hospitals NHS Foundation Trust, Aintree Hospital Site, Longmoor Lane, Fazakerly, Liverpool, L9 7AL, UK.

Unlabelled: There is a high burden of socioeconomic deprivation across Merseyside and, along with this, poorer cancer outcomes. The incidence of head and neck squamous cell carcinoma (HNSCC) within this region is higher than the national average and there are often additional complexities to individual treatment pathways such as poor health literacy, lack of social support and transport options which can impact on adherence to prescribed treatments.

Aims: This work aims to understand the impact of deprivation on patients diagnosed with HNSCC undergoing chemoradiotherapy (CRT) or radiotherapy (RT) treatment by identifying barriers to adherence.

Head and neck cancer management requires multidisciplinary approach in which radical surgery with or without flap reconstructions and neck dissection, along with radiotherapy (RT)/chemoradiotherapy (CRT) serve as the key components. Neoadjuvant chemotherapy and immunotherapy are used in selected cases based on the institutional preference. Knowledge of expected post-treatment changes on imaging is essential to differentiate it from recurrence.

Palliative and End of Life Care: An Important Component of Supportive Oncology.

Br J Hosp Med (Lond)

September 2024

Department of Symptom Control and Palliative Care, The Royal Marsden NHS Foundation Trust, London, UK.

Palliative care, an integral component of supportive oncology, enhances the quality of life for patients living with cancer. Whilst palliative care has historically been synonymous with the provision of care at the end of life, it is increasingly playing a role earlier in a patient's cancer journey; frequently in conjunction with administration of anticancer treatment. Although early integration has been shown to improve patient outcomes, service development remains in its infancy and consideration of challenges bears relevance.

Article Synopsis
  • - The study evaluates how well computed tomography (CT) can predict extrathyroidal extension (ETE) and tracheal invasion (TI) in patients with differentiated papillary thyroid carcinoma (PTC), comparing its accuracy to that of ultrasound (US).
  • - Data was collected from 83 patients who had both US and CT scans before surgery, focusing on the diagnostic performance based on sensitivity, specificity, and predictive values for detecting ETE and TI.
  • - Results indicate that while CT is fairly accurate and specific for ETE, its sensitivity is lower than that of US, implying that CT alone may not be sufficient for conclusive assessments in some cases.

Immune checkpoints refer to mechanisms entrusted with the modulation of immune responses in peripheral tissues and are required for minimising collateral damage. Immune checkpoint inhibitors (ICPi) work through numerous pathways, including the anti-CTLA-4 (cytotoxic T-lymphocyte-associated protein 4), anti-PD-1 (programmed cell death protein 1) and the PD-L1 (protein cell death protein-ligand-1) pathways. They are proving to be an exciting therapeutic avenue in the attempt to activate anti-tumour activity.
