Background: Routine group and save (G&S) testing is frequently performed prior to cholecystectomy, despite growing evidence that a targeted approach is safe and avoids unnecessary investigations. This retrospective cohort study explored frequency of testing in our unit, rates of peri-operative blood transfusion and pre-operative risk factors for requiring transfusion.

Methods: Health records of 453 consecutive adults who underwent cholecystectomy in a UK NHS trust were reviewed for blood transfusion up to 30 days post-operatively.

National Heart and Lung Institute, Imperial College London, London, UK; Department of Cardiology, Imperial College Healthcare NHS Trust, London, UK; Department of Cardiology, Chelsea and Westminster Hospital NHS Foundation Trust, London, UK. Electronic address:

Article Synopsis
  • - The AI-ECG risk estimator (AIRE) platform was developed to improve predictions of future disease and mortality risks from electrocardiograms (ECGs), addressing limitations in existing models related to individual actionability and biological plausibility.
  • - AIRE utilizes deep learning and survival analysis on a massive dataset of over 1.16 million ECGs to predict patient-specific mortality risks and timelines, validated across diverse international cohorts.
  • - The platform demonstrated high accuracy for predicting various health risks, such as all-cause mortality and heart failure, and identified biological pathways linked to cardiac health, making it a promising tool for clinical use globally.

Aims: Concerns exist that intravenous (IV) iron might increase the risk of infections. The IRONMAN trial provided an opportunity to investigate whether giving IV ferric derisomaltose (FDI) to patients with heart failure and iron deficiency alters the rate of hospitalization or death due to infections.

Methods And Results: IRONMAN was a randomized trial of IV FDI versus usual care in patients with symptomatic heart failure, left ventricular ejection fraction (LVEF) ≤45%, and transferrin saturation (TSAT) <20% or ferritin <100 μg/L.

The use of extracorporeal life support (ECLS) throughout the perioperative phase of lung transplantation requires nuanced planning and execution by an integrated team of multidisciplinary experts. To date, no multidisciplinary consensus document has examined the perioperative considerations of how to best manage these patients. To address this challenge, this perioperative utilization of ECLS in lung transplantation consensus statement was approved for development by the International Society for Heart and Lung Transplantation Standards and Guidelines Committee.

Unfractionated heparin (UFH) is commonly used as an anticoagulant in sepsis treatment and has recently been found to have non-anticoagulant effects, but underlying mechanisms remain unclear. This retrospective clinical data showed that UFH has significant protective effects in sepsis compared to low-molecular-weight heparin and enoxaparin, indicating potential benefits of its non-anticoagulant properties. Recombinant protein chip screening, surface plasmon resonance, and molecular docking data demonstrated that UFH specifically bound to the cytoplasmic Drp1 protein through its zone 2 non-anticoagulant segment.

J Thorac Dis

September 2024

Copenhagen Academy for Medical Education and Simulation (CAMES), Rigshospitalet, University of Copenhagen, the Capital Region of Denmark, Copenhagen, Denmark.

Background: Bronchoscopy is a challenging technical procedure, and assessment of competence currently relies on expert raters. Human rating is time consuming and prone to rater bias. The aim of this study was to evaluate if a bronchial segment identification system based on artificial intelligence (AI) could automatically, instantly, and objectively assess competencies in flexible bronchoscopy in a valid way.
