Experiencing pain: perspectives of Patrick D. Wall-founding editor of the journal PAIN.

Pain

January 2025

Department of Cell and Developmental Biology, Silberman Institute of Life Sciences, and the Center for Research on Pain, The Hebrew University of Jerusalem, Givat Ram Campus, Jerusalem, Israel.

Pain is a sensory and emotional experience. How did Pat Wall, founding editor of the journal PAIN and recognized pain guru, view the relation between the brain and the experience of pain? He was certain what it is not. It is not impulses in peripheral nociceptors that light up neurons in a central pain nucleus.

Article Synopsis
  • Research indicates that teaching is a high-risk profession for lateral violence, which involves harmful behaviors among employees of similar or lower power.
  • This study focused on the experiences of sexual minority pre-service teachers in South African township schools, using interviews to gather in-depth insights about their challenges.
  • Findings highlight that these teachers face significant lateral violence mainly from school management and veteran teachers, which negatively affects their mental, physical, and social health, emphasizing the need for professional development around sexuality and gender diversity.

Article Synopsis
  • This systematic review investigates predictors of intradialytic hypotension (IDH) in critically ill patients receiving kidney replacement therapy (KRT) for acute kidney injury (AKI).
  • It analyzed data from 8 studies with 2873 patients, finding that machine learning models, particularly Extreme Gradient Boosting Machine (XGB) and deep neural networks (DNN), had the highest predictive accuracy compared to traditional indicators.
  • The study emphasizes the need for combining predictive models and clinical indicators to enhance prediction of IDH and highlights that further validation in different healthcare settings is necessary.

Article Synopsis
  • The study investigated the effectiveness and safety of medication abortion using mifepristone and misoprostol in women up to 42 days of gestation without confirmed intrauterine pregnancy using ultrasound.
  • A total of 1504 women were randomly assigned to either an early-start abortion group or a standard delayed care group, and the primary outcome measured was the rate of complete abortion.
  • The results showed a nearly identical rate of complete abortion between the two groups (95.2% early-start vs 95.3% standard), indicating that starting medication abortion before confirming intrauterine pregnancy is as safe and effective as standard care.

Buparlisib and ponatinib inhibit aggressiveness of cholangiocarcinoma cells via suppression of IRS1-related pathway by targeting oxidative stress resistance.

November 2024

Department of Biochemistry, Faculty of Medicine, Khon Kaen University, Khon Kaen 40002, Thailand; Cholangiocarcinoma Research Institute, Khon Kaen University, Khon Kaen 40002, Thailand. Electronic address:

Cholangiocarcinoma (CCA) is an oxidative stress-driven liver cancer with bile duct epithelial cell phenotypes and currently lacks effective treatments, making targeted drug therapy urgently needed. Oxidative stress plays a critical role in CCA carcinogenesis, involving cells with oxidative stress resistance via upregulation of the PI3K and MEKK3 signaling pathways. In this study, we investigated the antineoplastic efficacy of a PI3K inhibitor (buparlisib) and a multi-tyrosine kinase inhibitor (ponatinib) on CCA.

Department of Physical Therapy, Faculty of Social Welfare & Health Sciences, University of Haifa, Haifa, Israel.

Background: Physiotherapy has demonstrated significant benefits in enhancing quality of life and managing symptoms of people in need of palliative care (PC). Despite these benefits, palliative physiotherapy is infrequently integrated into community-based care, where most PC services are delivered. The limited prevalence of these treatments highlights the need to explore the factors influencing their delivery in home settings.

  • The Apparent Diffusion Coefficient (ADC) measures how water moves in tissues, helping doctors see changes caused by diseases or treatments.
  • The QIBA has made progress on using ADC measurements in different body parts like the brain, liver, prostate, and breast, by creating guidelines for their use.
  • The report explains the right amounts of change in ADC values that indicate real differences in tissue, helping doctors track patients’ health more accurately.

To estimate children's exposure to family overdose in the United States. We used recent demographic kinship modeling advances and the Centers for Disease Control and Prevention's most recent underlying cause of death estimates to model how many children aged younger than 18 years in 2019 had lost 1 or more parents, siblings, grandparents, aunts or uncles, or cousins to overdose mortality since birth. We calculated the number and proportion of children with such exposures and considered age, cohort, and gender patterning.

  • Rapid diagnosis of melanoma is crucial for better patient outcomes, and this study evaluates the effectiveness of teledermoscopy using the FotoFinder device and Moleanalyzer Pro AI compared to in-person clinical exams.
  • The study involved 30 melanocytic moles from 29 patients, analyzing the sensitivity and specificity of the three diagnosis methods against histopathological results.
  • Results showed that face-to-face exams had the highest sensitivity and specificity, while the teledermoscopy provided better agreement with pathology than the AI method, indicating that AI cannot fully replace dermatologists' expertise.

Background: Long-term oxygen supplementation for at least 15 hours per day prolongs survival among patients with severe hypoxemia. On the basis of a nonrandomized comparison, long-term oxygen therapy has been recommended to be used for 24 hours per day, a more burdensome regimen.

Methods: To test the hypothesis that long-term oxygen therapy used for 24 hours per day does not result in a lower risk of hospitalization or death at 1 year than therapy for 15 hours per day, we conducted a multicenter, registry-based, randomized, controlled trial involving patients who were starting oxygen therapy for chronic, severe hypoxemia at rest.

  • Fibrodysplasia ossificans progressiva (FOP) is a rare disorder causing painful flare-ups and abnormal bone growth, known as heterotopic ossification (HO).
  • In a clinical trial, patients receiving the anti-activin A antibody garetosmab experienced fewer severe and lengthy flare-ups compared to those given a placebo, leading to a notable difference in quality of life.
  • The study found that 71% of placebo patients had flare-ups linked to new HO lesions, while garetosmab reduced the severity, duration, and frequency of these flare-ups throughout the trial.

Wound Care Capacity of the Addiction Workforce in the Setting of Xylazine.

J Addict Med

November 2024

From the Center for Research on Healthcare, University of Pittsburgh School of Medicine, Pittsburgh, PA (RJ, MS); Grayken Center for Addiction Training and Technical Assistance, Boston Medical Center, Boston, MA (RJ, SB, SM, ASV, TH, AW, CLB); and Boston Health Care for the Homeless Program, Boston, MA (TJ).

Objectives: As xylazine increasingly adulterates the unregulated opioid supply, people who use drugs (PWUD) are more likely to experience sequalae from xylazine. Given xylazine exposure is consistently associated with development of wounds which can heal with medically directed wound care, we sought to understand the level of preparedness and ability of front-line addiction professionals who interact with PWUD to provide wound care treatment.

Methods: We administered a 26-item online survey assessing participant and organizational characteristics, level of wound care training, ability to test for xylazine and treat xylazine-associated wounds, and funding and billing characteristics to a national sample of addiction professionals using a listserv of over 11,000 individuals.

Continuous kidney replacement therapy (CKRT) is commonly used to manage critically ill patients with severe acute kidney injury. While recent trials focused on the correct dosing and timing of CKRT, our understanding regarding the optimum dose of net ultrafiltration is limited to retrospective data. The Restrictive versus Liberal Rate of Extracorporeal Volume Removal Evaluation in Acute Kidney Injury (RELIEVE-AKI) trial has been conducted to assess the feasibility of a prospective randomized trial in determining the optimum net ultrafiltration rate.
