Lynch syndrome (LS) results from pathogenic variants in mismatch repair genes and is the most common hereditary cancer syndrome. Some guidelines or studies recommend restricting screening according to endometrial cancer (EC) using endometrial biopsy. The pooled sensitivity and specificity of endometrial cytology for detecting endometrial atypical hyperplasia or cancer have been reported to be as high as the pooled sensitivity and specificity of endometrial biopsy.

Division of Diabetes and Endocrinology, Department of Internal Medicine, Kobe University Hospital, Kobe, 650-0017, Japan.

Article Synopsis
  • Corticotroph tumors, specifically ACTHomas, could be potential targets for immunotherapy due to their susceptibility to immune cell attack, particularly in conditions like hypophysitis.
  • The study analyzed tumor-infiltrating immune cells in different cortisol environments and found that ACTHomas had low levels of CD8+ T cells compared to other pituitary tumors, suggesting they may be "immunologically cold."
  • It was observed that CD4+ T cells and certain macrophages were more abundant in cortisol-exposed tumors, indicating a relationship between cortisol levels and immune response, thus highlighting the need for further research on effective immunotherapy approaches.

Article Synopsis
  • The ONO-4538-52/TASUKI-52 study evaluated the effectiveness and safety of nivolumab combined with chemotherapy and bevacizumab for treating advanced non-squamous non-small cell lung cancer (nsNSCLC) in Japan, Korea, and Taiwan.
  • At the interim analysis, patients receiving nivolumab showed significantly longer progression-free survival compared to those who received a placebo, although overall survival data was still early.
  • An updated analysis revealed that patients in the nivolumab group had a median overall survival of 30.8 months versus 24.7 months for the placebo group, suggesting that the nivolumab regimen could be a standard first-line treatment for this type of lung cancer.

Background: The efficacy and safety of olaparib compared with placebo in the subset of patients from Japan in the phase 3 OlympiA trial (NCT02032823) are reported here and contextualized with reference to the global OlympiA population.

Methods: Patients with germline BRCA1 and/or BRCA2 pathogenic variants and HER2-negative, high-risk early breast cancer who had received neoadjuvant or adjuvant chemotherapy and completed local treatment were eligible. Patients were randomized 1:1 to receive olaparib or placebo for 1 year.

Article Synopsis
  • The ESMO Clinical Practice Guidelines for endometrial cancer were updated in 2022, prompting a virtual meeting in July 2022 to adapt these guidelines for the Asian context.
  • A panel of experts from various Asian oncological societies participated in creating these adapted guidelines, focusing on variations in patient management across different countries.
  • The goal is to optimize and harmonize endometrial cancer treatment in Asia by integrating Western and Asian research while considering local differences in clinical practices and access to treatments.

Lymphomatoid gastropathy (LyGa)/natural killer (NK)-cell enteropathy (NKCE) is recognized as a benign NK-cell lymphoproliferative disease. Due to its histological similarity to NK/T cell lymphoma, it is easy to misdiagnose, leading to unnecessary chemotherapy and poor quality of life. This disease is typically observed in the small and large intestines in North America, whereas almost all cases in Japan occur locally in the stomach.

A non-neoplastic epithelium (NE) often appears in gastric cancer (GC). We explored the histological features of NE in comparison between HP-eradicated and HP-infected GCs. We enrolled 40 HP-eradicated and 40 HP-infected GCs matched by size, macroscopic and histological type.

Article Synopsis
  • Synovial sarcoma is a rare and aggressive type of soft tissue cancer, and this report focuses on an unusual case found in the sigmoid colon of a man in his forties with a genetic predisposition (MSH2 gene deletion).
  • The patient had a history of previous successful treatments for unrelated cancers and was monitored for Lynch syndrome, which led to the discovery of this colorectal tumor during a colonoscopy.
  • The tumor exhibited characteristics consistent with monophasic fibrous synovial sarcoma, including specific cell growth patterns and confirmed genetic markers, while also showing that the common relationship between microsatellite instability and synovial sarcoma was not significant in this case.

Objective: To identify specific ultrasonographic features that differentiate hepatic angiomyolipoma (HAML) from hepatocellular carcinoma (HCC).

Methods: Twelve patients with HAML and 73 patients with HCC, whose diagnosis were pathologically confirmed at a single center in Japan between 2006 and 2016, were included in this study. The HAML and HCC cases were histologically evaluated and their histological growth patterns were compared.

Epstein-Barr virus-positive gastric cancer (EBVGC) comprises approximately 9% of all gastric cancers and is associated with a low prevalence of lymph node metastasis (LNM). Given that limited data concerning LNM in EBV-related early GC are available, EBV status is not considered an indicator for endoscopic submucosal dissection (ESD). In this review, we focused on pT1 EBVGC and on gastric carcinoma with lymphoid stroma (GCLS), and discuss expanded ESD indications and curative resection criteria.

Background And Aim: Gastric neoplasms (GN), including gastric adenoma and carcinoma, are well known as extracolonic manifestations of familial adenomatous polyposis (FAP). We aimed to investigate the clinicopathological features of GN in FAP patients and to clarify their relationship with the endoscopic status of the background mucosa.

Methods: We analyzed the records of 39 patients who were diagnosed with FAP and underwent esophagogastroduodenoscopy between April 2005 and July 2016.

In patients with ulcerative colitis, tumor development occurs with an increase in the duration of the disease. Such lesions, known as ulcerative colitis-associated neoplasia (UCAN), histologically show a broad variety of findings such as low-grade dysplasia, high-grade dysplasia, and invasive carcinoma. For pathologists, however, the histopathological diagnosis of UCAN is occasionally difficult.

Background: Epstein-Barr virus-positive gastric cancer (EBVGC) is associated with a low prevalence of lymph node metastasis (LNM); however, EBV status is not considered in the indication of endoscopic resection (ER). We aimed to clarify the implication of EBV status for ER of pT1b GC.

Methods: Consecutive cases of pT1b GCs treated with surgery between 2005 and 2014 were retrospectively analyzed.

Esophageal cancer is one of the most common cancers worldwide. Esophageal squamous cell carcinoma (ESCC) is the major histological type of esophageal cancer in Eastern Asian countries. Several types of noncoding RNAs (ncRNAs) function as key epigenetic regulators of gene expression and are implicated in various physiological processes.
