Hypercalcemia is a common complication of malignancy, often attributed to elevated PTH-related protein levels mimicking the effects of PTH and promoting bone resorption. We present the case of a 67-year-old Hispanic female with a history of hypertension, type 2 diabetes mellitus, hyperlipidemia, and chronic kidney disease, who initially underwent parathyroidectomy for suspected primary hyperparathyroidism resulting from persistent hypercalcemia. Despite surgery, the patient continued to experience hypercalcemia and was subsequently diagnosed with diffuse large B-cell lymphoma with hypercalcemia as a rare but life-threatening complication.

We report the case of a 70-year-old woman diagnosed with neural epidermal growth factor-like 1 antigen-positive membranous nephropathy and nephrotic syndrome. Following thorough exclusion of autoimmune diseases, medications, or infections as potential causes, colonoscopy was performed as part of malignancy evaluation, revealing an 18 mm villous adenoma in the sigmoid colon and a 7 mm tubulovillous adenoma in the cecum. Despite the absence of gastrointestinal symptoms initially and the absence of high-grade dysplasia in the pathology report, the patient experienced a remarkable improvement in symptoms and a reduction in nephrotic-range proteinuria following polypectomy, observed within a few months.

In this study, we examined all-cause and HF-specific 30-day readmission rates for patients who were admitted for HF on a weekend vs admitted for HF on a weekday.

Purpose: Evaluate the psychometric properties of the 5-item Very Quick Inventory of Depressive Symptomatology self-report and clinician-rated versions (VQIDS-SR/VQIDS-C), compare their relative performance, create crosswalks between their total scores and other accepted depressive symptom ratings, and define clinically relevant depressive symptom severity thresholds and categorical outcomes for both versions.

Patients And Methods: The Sequenced Treatment Alternatives to Relieve Depression trial obtained baseline and exit 17-item Hamilton Rating Scale for Depression (HRSD) and 30-item Inventory of Depressive Symptomatology - Clinician-rated scores, and baseline and visit-wise QIDS-SR and QIDS-C ratings from the first treatment step (citalopram). The VQIDS-C and the VQIDS-SR items (sad mood, self-outlook, involvement, fatigue, psychomotor slowing) (each rated 0-3), extracted from the corresponding 16-item ratings, were selected to best reflect the 6-item HRSD (HRSD) (exclusive of anxiety).

Occurrence of fulminant myocarditis with allorejection-like immunologic features in patients under anti-PD1 (programmed death cell protein 1) treatment suggests a key role of the PD1/PD-L1 (programmed death ligand 1) axis in cardiac immune homeostasis.

We highlight the presence of a calcified mass in the left main coronary artery without significant atherosclerosis seen in the other coronary arteries or in the peripheral large arteries. In our view, the calcified character of the obstruction and the calcification of the aortic valve are characteristic of a variant type of coronary artery disease (CAD) not associated with the same risk factors as diffuse coronary atherosclerosis, but, in this case, with rheumatic heart disease. This case report also emphasizes the interventional approach for patients with aortic valve stenosis secondary to rheumatic heart disease.

Vulvodynia (vulvar pain syndrome) is a chronic multifactorial disease affecting almost 13 million women in the USA and can lead to morbidity and a reduced quality of life. We hypothesize that an initial microbiological insult in the vagina causes modifications in the biological vaginal milieu and/or an alteration on the lactobacilli flora. Dilauryl fluorescein (DLF) is a lipid soluble molecule that becomes fluorescent when lauric acid is removed by hydrolysis The purpose of these studies was to evaluate DLF as a potential probe for the function of reconstituted high-density lipoproteins (rHDL) as hydrophobic drug transport vehicles. The DLF containing rHDL nanoparticles were characterized regarding their physical/chemical properties, including molecular diameter, molecular weight, chemical composition, and buoyant density. We investigated the uptake of DLF from rHDL in cells that overexpress the scavenger receptor (SR-B1), known to facilitate the selective cellular uptake of cholesteryl esters from HDL.

Acquired methemoglobinemia (MetHb) is a rare complication of exposure to toxic chemicals or drugs, most commonly topical anesthetic agents. This condition occurs when the rate of methemoglobin production exceeds the rate of methemoglobin reduction. Topical anesthetics have been reported to cause MetHb, but this adverse event is extremely rare and is not usually listed as one of the possible complications of transesophageal echocardiography (TEE).
