The incidence of venous thromboembolism (VTE) in cancer patients may have changed in the past decade, possibly due to novel cancer therapies, improved survival, and high-resolution imaging. Danish medical registries were used to identify 499 092 patients with a first-time cancer diagnosis between 1997 and 2017, who were matched to 1 497 276 comparison individuals without cancer from the general population. We computed cumulative incidences of VTE 6 and 12 months after the diagnosis/index date.

Objectives There is a global increase in infections caused by Gram-negative bacteria. The majority of research is on bacteremic Gram-negative infections (GNI), leaving a knowledge gap on the burden of non-bacteremic GNI. Our aim is to describe characteristics and determine the burden of bacteremic and non-bacteremic GNI in hospitalized patients in the Netherlands.

Article Synopsis
  • The study aimed to evaluate the effectiveness of chest CT and RT-PCR tests for COVID-19 in asymptomatic surgical patients.
  • The research involved 2,093 patients across 14 centers, revealing a low positive yield of COVID-19 cases—1.5% for combined testing, 0.7% for CT, and 1.1% for RT-PCR.
  • Results indicated that while preoperative screening helped ensure safety in surgeries, the value of chest CT testing was minimal compared to RT-PCR, particularly as community COVID-19 prevalence increased.

Article Synopsis
  • This study evaluated the effectiveness of adding extra rituximab during the first four cycles of R-CHOP, a standard treatment for diffuse large B-cell lymphoma (DLBCL), to see if it improved patient outcomes.
  • The trial included 574 patients and measured success by the rate of complete remission and long-term survival outcomes, showing no significant difference between those receiving standard R-CHOP and those receiving rituximab intensification.
  • It was found that older patients experienced more side effects with the intensified regimen, leading to the conclusion that early rituximab intensification does not enhance treatment outcomes for DLBCL.

Article Synopsis
  • The study investigates the impact of antibiotic resistance on mortality from Gram-negative infections in the Netherlands, as previous research highlighted its serious consequences globally.
  • Researchers analyzed data from eight hospitals between 2013 and 2016, comparing 1954 Gram-negative infections with matched control patients to look for differences in 30-day mortality rates due to antibiotic resistance.
  • The findings indicate that antibiotic resistance did not significantly raise the risk of 30-day mortality among infected patients in the Netherlands, contrasting with international studies suggesting higher risks.

Diagnostic and Therapeutic Management of Upper Extremity Deep Vein Thrombosis.

J Clin Med

July 2020

Department of Vascular Medicine, Amsterdam Cardiovascular Science, Amsterdam UMC, University of Amsterdam, 1105 AZ Amsterdam, The Netherlands.

Upper extremity deep vein thrombosis (UEDVT) accounts for 5% of all deep vein thromboses (DVTs). UEDVT may be complicated by post thrombotic syndrome and pulmonary embolism, and early recognition and prompt start of anticoagulant treatment are key. Primary UEDVT, also known as Paget-von Schrötter syndrome, is associated with repeated or sudden physical activity of the upper arm and venous outflow obstruction due to anatomical variations.

Thromboembolic and bleeding complications in patients with oesophageal cancer.

Br J Surg

September 2020

Department of Vascular Medicine, Amsterdam Cardiovascular Science, Amsterdam, the Netherlands.

Background: In patients who undergo curative treatment for oesophageal cancer, risk estimates of venous thromboembolism (VTE), arterial thromboembolism and bleeding are needed to guide decisions about thromboprophylaxis.

Methods: This was a single-centre, retrospective cohort study of patients with stage I-III oesophageal cancer who received neoadjuvant chemoradiation followed by oesophagectomy. The outcomes VTE, arterial thromboembolism, major bleeding, clinically relevant non-major bleeding and mortality were analysed for four consecutive cancer treatment stages (from diagnosis to neoadjuvant chemoradiotherapy, during neoadjuvant treatment, 30-day postoperative period, and up to 6 months after postoperative period).

Objective: Short-course aminoglycosides as adjunctive empirical therapy to β-lactams in patients with a clinical suspicion of sepsis are used to broaden antibiotic susceptibility coverage and to enhance bacterial killing. We quantified the impact of this approach on 30-day mortality in a subset of sepsis patients with a Gram-negative bloodstream infection.

Methods: From a prospective cohort study conducted in seven hospitals in the Netherlands between June 2013 and November 2015, we selected all patients with Gram-negative bloodstream infection (GN-BSI).

Primary Thromboprophylaxis in Ambulatory Cancer Patients: Where Do We Stand?

Cancers (Basel)

February 2020

Department of Vascular Medicine, Amsterdam Cardiovascular Science, Amsterdam UMC, University of Amsterdam, 1105 AZ Amsterdam, The Netherlands.

Venous thromboembolism (VTE), comprising deep-vein thrombosis and pulmonary embolism, is a frequent complication in ambulatory cancer patients. Despite the high risk, routine thromboprophylaxis is not recommended because of the high number needed to treat and the risk of bleeding. Two recent trials demonstrated that the number needed to treat can be reduced by selecting cancer patients at high risk for VTE with prediction scores, leading the latest guidelines to suggest such an approach in clinical practice.

Background: Serum levels of markers of endothelial cell activation are associated with bacteremia and mortality in sepsis in adults, children, and newborns with early onset sepsis. We hypothesize that levels of these markers are associated with these outcomes in hospitalized newborns with suspected late onset neonatal sepsis (LONS).

Methods: In this prospective cohort study, newborns admitted to the tertiary neonatal care facility of Suriname were included upon clinical suspicion of LONS and before start of antibiotic treatment, between April 1, 2015 and May 31, 2016.

Childhood asthma in the new omics era: challenges and perspectives.

Curr Opin Allergy Clin Immunol

April 2020

Department of Respiratory Medicine, Amsterdam University Medical Centers, University of Amsterdam, Amsterdam, The Netherlands.

Purpose Of Review: Childhood asthma is a heterogeneous inflammatory disease comprising different phenotypes and endotypes and, particularly in its severe forms, has a large impact on the quality-of-life of patients and caregivers. The application of advanced omics technologies provides useful insights into underlying asthma endotypes and may provide potential clinical biomarkers to guide treatment and move towards a precision medicine approach.

Recent Findings: The current article addresses how novel omics approaches have shaped our current understanding of childhood asthma and highlights recent findings from (pharmaco)genomics, epigenomics, transcriptomics, and metabolomics studies on childhood asthma and their potential clinical implications to guide treatment in severe asthmatics.

Respiratory syncytial virus (RSV) lower respiratory tract infection (LRTI) causes significant morbidity and mortality among young infants worldwide. It is currently widely accepted that neutrophil influx into the airways is a hallmark of the pathophysiology. However, the exact mechanism of neutrophil migration from the vasculature into the alveolar space in RSV LRTI has received little attention.

The neonatal early onset sepsis (EOS) calculator is a novel tool for antibiotic stewardship in newborns, associated with a reduction of empiric antibiotic treatment for suspected EOS. We studied if implementation of the EOS calculator results in less healthcare utilization and lower financial costs of suspected EOS. For this, we compared two single-year cohorts of hospitalizations within 3 days after birth in a Dutch nonacademic teaching hospital, before and after implementation of the EOS calculator.
