Introduction: Chronic ocular hypotony (low intraocular pressure (IOP)) is a potentially sight-threatening condition. Although there is some controversy as to what constitutes non-physiological IOP, generally it is accepted as an IOP of 6.5 mm Hg or less, measured on three separate occasions and lasting for at least 3 months.

Keratoplasty to restore vision in trachomatous corneal opacity: A literature review.

PLoS Negl Trop Dis

November 2024

Global Neglected Tropical Diseases Programme, World Health Organization, Geneva, Switzerland.

Article Synopsis
  • Trachoma is the primary infectious cause of blindness, and individuals with trachomatous corneal opacity (TCO) are considered at high risk for graft failure in corneal transplantation.
  • A review of studies from 1992 to 2022 found that 71% of grafts were clear at follow-up, but there was significant variability in study quality and reporting.
  • Although evidence is limited, keratoplasty appears to improve vision in TCO patients, suggesting potential benefits for this underserved group.

Facedown Positioning in Macular Hole Surgery: A Systematic Review and Individual Participant Data Meta-Analysis.

Ophthalmology

August 2024

Biosciences Institute, Newcastle University, Newcastle upon Tyne, United Kingdom; Sunderland Eye Infirmary, Sunderland, United Kingdom. Electronic address:

Topic: To assess the anatomic and visual effects of facedown positioning (FDP) advice in patients undergoing vitrectomy with gas tamponade for idiopathic full-thickness macular holes (FTMHs) and to explore differential treatment effects by macular hole size and FDP duration.

Clinical Relevance: The necessity and duration of FDP for FTMH closure remain contentious, with no consensus guidelines.

Methods: Prospectively registered systematic review and individual patient data (IPD) meta-analysis of randomized controlled trials comparing FDP with no FDP (nFDP) across the MEDLINE, Embase, and Cochrane Library databases and clinical trial registries from January 2000 to March 2023 (CRD42023395152).

Background: The purpose of this study was to quantify how much of the burden of visual impairment (VI) and unmet need in Talagang, identified by Rapid Assessment of Avoidable Blindness (RAAB) survey data, has been addressed by Community Eye Health (CEH) programme efforts.

Methods: A RAAB survey was carried out in November 2018, with 2,824 participants in Talagang Tehsil, Punjab, Pakistan, aged 50 and over. Census data were used to extrapolate survey data to the population.

November 2024

From the Tennent Institute of Ophthalmology, Gartnavel General Hospital, Glasgow, United Kingdom (Henry, Lockington); James Weir Fluids Laboratory, Department of Mechanical and Aerospace Engineering, University of Strathclyde, Glasgow, United Kingdom (Castro, Rycroft, Syntouka, Oliveira); Advanced Materials Research Laboratory, University of Strathclyde, Glasgow, United Kingdom (Belkessa, Sillars); Edinburgh Complex Fluids Partnership (ECFP), SUPA and School of Physics & Astronomy, The University of Edinburgh, Edinburgh, United Kingdom (Silva).

Purpose: To evaluate how temperature affects the rheology of common ophthalmic viscosurgical devices (OVDs) and clinical implications.

Setting: Tennent Institute of Ophthalmology, Glasgow, with Department of Mechanical and Aerospace Engineering, and Advanced Materials Research Laboratory, University of Strathclyde, Glasgow, United Kingdom.

Design: Laboratory pilot study.

Purpose: Complications associated with intravitreal anti-VEGF therapies are reported inconsistently in the literature, thus limiting an accurate evaluation and comparison of safety between studies. This study aimed to develop a standardized classification system for anti-VEGF ocular complications using the Delphi consensus process.

Design: Systematic review and Delphi consensus process.

Importance: Fluoroquinolone use has been associated with increased risk of uveitis and retinal detachment in noninterventional studies, but the findings have been conflicting and causality is unclear.

Objective: To estimate the association of systemic fluoroquinolone use with acute uveitis or retinal detachment, using multiple analyses and multiple databases to increase the robustness of results.

Design, Setting, And Participants: This cohort study used data from the Clinical Practice Research Datalink Aurum and GOLD UK primary care records databases, which were linked to hospital admissions data.

Background: Recent estimates of global prevalence of uncorrected presbyopia range from 510 to 826 million. There is a shortage of primary data regarding Near Visual Impairment (NVI) magnitude.

Methods: Near visual acuity (NVA) and NVI data was collected from over 388,000 people aged 35 or over across 9 countries, within Community Eye Health programmes between January 2022 and June 2023.

Introduction: Proton beam therapy (PBT) is an effective treatment option for uveal melanomas. Following treatment, it may take many months for the tumour to respond and it may initially enlarge. We reviewed our PBT patients to determine when they showed a radiological response to treatment.

Lisch Epithelial Corneal Dystrophy Is Caused by Heterozygous Loss-of-Function Variants in MCOLN1.

Am J Ophthalmol

February 2024

Department of Pediatrics I, Medical University of Innsbruck, 6020 Innsbruck, Austria (D.G.-A., T.M., A.R.J.); Division of Human Genetics, Medical University of Innsbruck, 6020 Innsbruck, Austria (A.R.J.). Electronic address:

Article Synopsis
  • The study investigates the genetic causes of Lisch epithelial corneal dystrophy (LECD) through a multicenter cohort involving 27 individuals from 17 families.
  • Researchers found nine rare variants of the MCOLN1 gene in 23 patients, indicating that mutations in this gene lead to LECD and potentially other conditions like mucolipidosis IV (MLIV).
  • The findings suggest that while MCOLN1 haploinsufficiency is the main contributor to LECD, there is variability in how strongly the condition manifests, highlighting a complex relationship between genetics and disease expression.

Purpose: We present a novel approach for managing retinal detachment in a patient with aniridia related corneal scarring precluding fundal view via a deep anterior lamellar keratoplasty.

Methods: A 30-year-old female patient who had bilaterally opaque cornea, due to advanced aniridia related keratopathy presented with, experiencing flashing of light in her right eye. Examination of the retinal fundus was not possible due to the dense corneal scarring and underlying cataract.

We report a man in his 80s who described sudden unilateral loss of vision, resulting in extensive urgent investigations by the accident and emergency department team. Subsequent evaluation of visual acuity with pinhole demonstrated significant improvement, triggering further questioning. It transpired that he was mistakenly wearing his wife's glasses, which accounted for his visual symptoms.
