The endoscopic endonasal approach (EEA) is a rapidly growing, minimally invasive discipline applied to a broad set of skull base tumors. The introduction of the endoscopic endonasal approach for the management of lesions of the skull base has produced a paradigm shift in the way these complicated lesions are managed. The extended endonasal approach provides the most direct route to the anterior cranial base including sella, cribriform plate, planum sphenoidale, suprasellar cistern, clivus and foramen magnum.

Purpose: The extent to which postoperative hypotension contributes to renal injury remains unclear, much less what the harm thresholds might be. We therefore tested the primary hypothesis that there is an absolute hypotensive arterial pressure threshold for acute kidney injury during the initial seven days after noncardiac surgery.

Methods: We conducted a single-centre historical cohort analysis of adults who had noncardiac surgery and had creatinine recorded preoperatively and postoperatively.

Ewing sarcoma (EWS) is a highly aggressive cancer with a survival rate of 70%-80% for patients with localized disease and under 30% for those with metastatic disease. Tumor-infiltrating neutrophils (TIN) can generate extracellular net-like DNA structures known as neutrophil extracellular traps (NETs). However, little is known about the presence and prognostic significance of tumor-infiltrating NETs in EWS.

Article Synopsis
  • Lennox-Gastaut syndrome (LGS) is a severe epilepsy disorder in children that is hard to treat; when medications fail, surgeries like vagus nerve stimulation (VNS) and corpus callosotomy (CC) may be options, with CC often being more effective.
  • This study reviewed the outcomes of 127 children with LGS who had undergone CC after unsuccessful VNS, focusing on their seizure types and surgery results.
  • The findings showed that about 83% of patients experienced at least a 50% reduction in drop attacks after CC, demonstrating that CC is an important option for improving seizure control with low surgical risks.

Background Context: Spinal pathologies are diverse in nature and, excluding trauma and degenerative diseases, includes infectious, neoplastic (either extradural or intradural), and inflammatory conditions. The preoperative diagnosis is made with clinical judgment incorporating lab findings and radiological studies. When the diagnosis is uncertain, a biopsy is almost always mandatory since the treatment is dictated by the type of pathology.

Chiari type 1 malformation (CIM) is defined as tonsillar ectopia of >5 mm, while syringomyelia (SM) is defined as a cerebrospinal fluid (CSF)-filled cavity larger than 3 mm dissecting the spinal cord. Over the last decades, our understanding of these pathologies has grown; however, many controversies still exist almost in every aspect of CIM and SM, including etiology, indication for treatment, timing of treatment, surgical technique, follow-up regime, and outcome. This chapter provides a comprehensive overview on different aspects of CIM and SM and on the still existing controversies, based on the evidence presently available.
