Article Synopsis
  • - The study analyzed the effectiveness and safety of pembrolizumab in 608 older patients with advanced urothelial carcinoma, comparing outcomes between those under 75 and those 75 and older using retrospective data.
  • - Results showed the median overall survival for those under 75 was around 7.8 months, while for those 75 and older, it was approximately 10.4 months; however, this difference wasn't statistically significant, and older patients reported more adverse events overall.
  • - The findings suggest that age doesn't significantly impact the treatment's effectiveness or safety, indicating that elderly patients should not be discouraged from receiving pembrolizumab but should be monitored for possible side effects.

Neutrophil-to-lymphocyte ratio (NLR) was reported to be associated with prognosis of urothelial cancer (UC) patients receiving systemic chemotherapy or immunotherapy. However, it has not been elucidated how preceding first-line chemotherapy affects NLR and subsequent second-line pembrolizumab treatment. This multicenter study analyzed 458 patients with metastatic UC who received first-line chemotherapy and second-line pembrolizumab with regard to pre-chemotherapy and pre-pembrolizumab NLR in association with the efficacy of chemotherapy and pembrolizumab treatment.

Article Synopsis
  • The study investigates the potential of weighted diffusion subtraction (WDS) imaging to improve the diagnosis of clinically significant prostate cancer (csPCa) compared to the traditional PI-RADS method.
  • WDS images, created by combining high and low b-value diffusion-weighted images, aim to provide a more objective assessment of prostate lesions by highlighting areas with low apparent diffusion coefficient (ADC) values.
  • Results indicated that WDS scored significantly better in terms of diagnostic ability, sensitivity, and positive predictive value than the standard DWI/ADC scores.

Predicting pathogenic germline variants (PGVs) in breast cancer patients is important for selecting optimal therapeutics and implementing risk reduction strategies. However, PGV risk factors and the performance of prediction methods in the Japanese population remain unclear. We investigated clinicopathological risk factors using the Tyrer-Cuzick (TC) breast cancer risk evaluation tool to predict BRCA PGVs in unselected Japanese breast cancer patients (n = 1,995).

International consensus on initial screening and follow-up of asymptomatic SDHx mutation carriers.

Nat Rev Endocrinol

July 2021

INSERM, PARCC, Equipe Labellisée par la Ligue contre le Cancer, Paris, France.

Approximately 20% of patients diagnosed with a phaeochromocytoma or paraganglioma carry a germline mutation in one of the succinate dehydrogenase (SDHx) genes (SDHA, SDHB, SDHC and SDHD), which encode the four subunits of the SDH enzyme. When a pathogenic SDHx mutation is identified in an affected patient, genetic counselling is proposed for first-degree relatives. Optimal initial evaluation and follow-up of people who are asymptomatic but might carry SDHx mutations have not yet been agreed.

Article Synopsis
  • Many aldosterone-producing micro-adenomas go undetected through imaging, leading to adrenal venous sampling (AVS) being performed in primary aldosteronism (PA) patients who show no visible adrenal tumors.
  • A study analyzed 1,586 PA patients without apparent adrenal tumors to determine characteristics that differentiate unilateral PA from bilateral cases and assessed adrenalectomy rates.
  • Findings suggest that AVS may not be necessary for all PA patients without visible tumors, especially for young, female, normokalemic patients with low aldosterone-to-renin ratios, while it is recommended for hypokalemic males with high ratios due to their higher likelihood of having unilateral PA.
