Open axillary arterial injury is life-threatening, and upper-extremity reperfusion must be performed within approximately 6 h. We present the case of a patient who underwent reperfusion of the upper limb and nerve reconstruction of the post-ganglionic brachial plexus injury in one stage while maintaining stable vital signs. The injury was an avulsion with no fracture.

Article Synopsis
  • The study evaluated various ANCA detection methods in a group of 147 patients with untreated ANCA-associated vasculitis, focusing primarily on myeloperoxidase (MPO)-ANCA.
  • Different testing methods were compared, including immunofluorescence (IIF) and several antigen-specific immunoassays (ELISA, CLEIA, FEIA, LTIA), revealing varying sensitivities and specificities for diagnosing AAV.
  • Results indicated that while antigen-specific immunoassays performed well overall, using dual IIF and these immunoassays had limited benefits, and standardizing ANCA measurements across different methods remained challenging.

Background And Aims: The impact of hepatitis C virus (HCV) eradication via direct-acting antiviral (DAA) therapy on overall mortality, particularly non-liver-related mortality, is understudied.

Methods: We recruited 4180 patients with chronic HCV infection who achieved sustained virological response (SVR) (HCV eradication) through DAA therapy (n = 2501, SVR group) or who did not receive antiviral therapy (n = 1679, non-SVR group); 1236 from each group were chosen using propensity score matching. Causes of death and all-cause mortality, including non-liver-related diseases, were investigated.

Article Synopsis
  • - This study explores the effectiveness of combining immune checkpoint inhibitors durvalumab and tremelimumab (DT) for patients with hepatocellular carcinoma (HCC) who previously received treatments like atezolizumab plus bevacizumab (AB).
  • - Results showed objective response rates (ORRs) were highest in first-line treatment (44%), followed by second-line without prior AB (54%), and significantly lower in patients who had prior AB (5%), indicating that prior treatment heavily affects success.
  • - The findings suggest that while DT may be less effective after AB treatment, it can still yield similar results to first-line therapy in patients without prior AB, highlighting the importance of treatment history when planning HCC therapy.

Article Synopsis
  • Immunoglobulin A vasculitis (IgAV) is a rare systemic condition that can cause myalgia, as demonstrated in a reported pediatric case involving a five-year-old boy.
  • The patient's symptoms included abdominal pain, vomiting, and skin rashes, leading to a diagnosis of IgAV after imaging revealed issues in the digestive tract.
  • Treatment involved steroid therapy which successfully managed his myalgia and other symptoms, highlighting that fasciitis can complicate IgAV and requires consideration in diagnosis, especially when muscle pain occurs but enzyme levels are normal.

Aim: Fracture-related femoral neck displacement is more likely in patients with schizophrenia because of delayed diagnosis, as these patients frequently have less severe fracture-associated subjective symptoms. This study aimed to investigate the association of schizophrenia with the risk of fracture-related femoral neck displacement in hospitalized patients.

Methods: We retrospectively analyzed the medical records of patients with femoral neck fractures treated between April 2013 and March 2018 at a single institution.

Bexarotene-induced hypothyroidism and dyslipidemia; a nation-wide study.

Endocr J

August 2024

Division of Nephrology and Endocrinology, Graduate School of Medicine, The University of Tokyo, Tokyo 113-8655, Japan.

Central hypothyroidism and dyslipidemia are well-known adverse events (AEs) of bexarotene therapy. Although hypothyroidism is known to cause dyslipidemia, no study has examined the association between hypothyroidism and dyslipidemia in patients undergoing bexarotene therapy. The aim of this study is to examine this association.

Article Synopsis
  • The study investigates how esophageal varices change in patients with cirrhosis after achieving sustained virological response (SVR) using direct-acting antivirals (DAAs).
  • 243 patients were examined before and after DAA treatment, revealing that 14.8% experienced worsened varices post-SVR and 11.9% showed improvement.
  • The ALBI score at SVR12 was identified as a significant predictor for both worsening varices and the development of new varices, with specific cut-off values highlighting the risk.
  • Despite achieving SVR, patients with high ALBI scores remain at risk for negative changes in their esophageal varices.

  • Abdominal angiography techniques like transarterial chemoembolization (TACE) are vital for treating liver cancer, but traditional transfemoral access (TFA) can lead to issues like postoperative pain and extended bed rest, impacting patient satisfaction.
  • A study involving 1,601 patients showed that using transradial access (TRA) for TACE achieved a high success rate of 99.4% and a low complication rate of 1.2%, with 98.2% of patients wanting the radial method for future treatments.
  • TRA is shown to be a safe and effective alternative to TFA for abdominal angiography, potentially easing the patient experience and improving clinical outcomes.

Aim: An association between hepatitis B core-related antigen (HBcrAg) kinetics and hepatocarcinogenesis during nucleoside (t)id analog (NA) treatment has recently been reported. HBcrAg kinetics and factors associated with HBcrAg response during tenofovir alafenamide (TAF) administration remain unclear. In this multicenter retrospective study, we aimed to clarify the efficacy and safety of TAF in treatment-naïve patients with chronic hepatitis B, focusing on the reduction in HBcrAg levels.

Background Aims: Tacrolimus (TAC) plus short-term methotrexate (stMTX) is used for graft-versus-host disease (GVHD) prophylaxis after allogeneic hematopoietic stem cell transplantation (allo-HSCT). TAC blood concentrations are frequently adjusted to enhance the graft-versus-leukemia/lymphoma effect or attenuate severe GVHD. Limited information is available on the clinical impact of these adjustments and the optimal time to perform them in order to achieve good clinical outcomes.

  • The TACTICS-L trial studied the combination of transarterial chemoembolization (TACE) and lenvatinib (LEN) for treating unresectable intermediate-stage hepatocellular carcinoma (HCC), aiming to improve outcomes since recurrence is common after TACE alone.
  • Patients with specific criteria were given LEN before and after TACE, with primary results showing a median progression-free survival of 28.0 months and high overall response rates (79% with 53% complete response 4 weeks post-TACE).
  • The trial included 62 participants, mostly men aged around 72, and results indicated that combining LEN with TACE could be beneficial across different patient subgroups.

Aberrant DNA Methylation of NPTX2 as an Indicator of Malignant Behavior in Thymic Epithelial Tumors.

Cancers (Basel)

January 2024

Department of Thoracic, Endocrine Surgery and Oncology, Graduate School of Biomedical Sciences, Tokushima University, Tokushima 770-8503, Japan.

Thymic epithelial tumors (TET) consist of thymomas, thymic carcinoma (TC), and neuroendocrine tumors of the thymus (NECTT). Genetic and epigenetic alterations in TET have been the focus of recent research. In the present study, genome-wide screening was performed on aberrantly methylated CpG islands in TET, and this identified neuronal pentraxin 2 (NTPX2) as a significantly hypermethylated CpG island in TC relative to thymomas.
