Predictive Scores for Identifying Chronic Opioid Dependence After General Anesthesia Surgery.

Purpose: To address the prevalence and risk factors of postoperative chronic opioid dependence, focusing on the development of a predictive scoring system to identify high-risk populations.

Methods: We analyzed data from the Taiwan Health Insurance Research Database spanning January 2016 to December 2018, encompassing adults undergoing major elective surgeries with general anesthesia. Patient demographics, surgical details, comorbidities, and preoperative medication use were scrutinized.

Aims: To assess the association between metformin use and the risk of sudden sensorineural hearing loss (SSNHL) in patients with Type 2 diabetes (T2D), a population at elevated risk for SSNHL.

Materials And Methods: This cohort study utilized data from Taiwan's National Health Insurance Research Database, following T2D patients from 2008 to 202 database's baseline. Metformin use was defined as achieving ≥80% of the medication possession ratio (MPR) and ≥28 cumulative defined daily doses (cDDD) within three months.

Micro-CT evaluation of a nanodiamond irrigant for hard-tissue debris removal during sonic agitation.

Aim: To investigate the efficacy of nanodiamond irrigation solution with sonic agitation for removing of hard-tissue debris (HTD) within the isthmus-containing mesial roots of human mandibular molars using micro-CT analysis.

Methodology: Forty mesial roots of extracted human mandibular molars were selected based on micro-computed tomography scans (9-μm resolution). The mesial canals were mechanically prepared using ProTaper® Gold nickel-titanium rotary instruments and divided into four groups (n = 10 each) according to the final irrigation protocol: sonic agitation with nanodiamond irrigation solution for three 20-s cycles, sonic agitation with 17% EDTA for three 20-s cycles, sonic agitation with 3% NaOCl for three 20-s cycles and manual syringe irrigation with NaOCl using a 30-G needle syringe for 60 s.

Purpose: To compare the oncologic outcomes between proton therapy and intensity-modulated radiation therapy (IMRT) for head and neck squamous cell carcinoma (HNSCC) patients undergoing curative radiotherapy (RT).

Experimental Design: We studied HNSCC patients who underwent curative-intent RT from 2015 to 2019, comparing the oncologic outcomes of proton therapy and IMRT. Our national retrospective HNSCC cohort study involved three institutes with proton therapy and 17 institutes (medical center levels) with IMRT in Taiwan.

Metformin Use and Risk of Delirium in Older Adults With Type 2 Diabetes.

Objective: Delirium is a precursor and risk factor for dementia, emphasizing the urgency of effective prevention and management strategies in older adults with type 2 diabetes (T2D). Identifying long-term, safe, and effective medications to prevent diabetes-related delirium is crucial because of its significant impact on this population. This study aimed to evaluate the protective effects of metformin against delirium in older adults with T2D, using a competing risk analysis of death to provide a more accurate assessment.

Actively Targeting Redox-Responsive Multifunctional Micelles for Synergistic Chemotherapy of Cancer.

Stimuli-responsive polymeric micelles decorated with cancer biomarkers represent an optimal choice for drug delivery applications due to their ability to enhance therapeutic efficacy while mitigating adverse side effects. Accordingly, we synthesized a digoxin-modified novel multifunctional redox-responsive disulfide-linked poly(ethylene glycol--poly(lactic--glycolic acid) copolymer (Bi(Dig-PEG-PLGA)-S) for the targeted and controlled release of doxorubicin (DOX) in cancer cells. Within the micellar aggregate, the disulfide bond confers redox responsiveness, while the presence of the digoxin moiety acts as a targeting agent and chemosensitizer for DOX.

This study aimed to evaluate the impact of different pre-transplant local treatments on the survival of liver transplantation (LTx) recipients with BCLC Stage A Hepatocellular Carcinoma (HCC). We analyzed data from the Taiwan Cancer Registry and National Health Insurance Research Databases spanning 2012 to 2018. Employing propensity score matching, patients were categorized into three groups: those receiving local treatments (180 patients), hepatectomy (179 patients), and combined treatments (180 patients).

Purpose: Studies have reported conflicting findings on the relationship between smoking and surgical site infection (SSI) post fixation for ankle and calcaneal fractures. This meta-analysis explored the effect of smoking on SSI incidence following open reduction and internal fixation (ORIF) of these fractures.

Methods: Full-text studies on smoking's influence on post-ORIF SSI rates for closed ankle and calcaneal fractures were sourced from the PubMed, Embase, and Cochrane databases, with no consideration given to language or publication date.

Statin therapy enhances survival in unresectable stage III lung squamous cell carcinoma with concurrent chemoradiotherapy.

To evaluate the impact of statin use on overall survival and lung cancer-specific survival in patients with unresectable stage III lung squamous cell carcinoma (LSCC) undergoing standard concurrent chemoradiotherapy (CCRT). Using data from the Taiwan Cancer Registry Database and National Health Insurance Research Database, this propensity score matching cohort study analyzed the influence of statin use during CCRT on overall survival and lung cancer-specific survival. Statin use during CCRT was independently associated with significant improvements in overall survival and lung cancer-specific survival.

Metformin monotherapy versus predominantly older non-metformin antidiabetic medications for cerebrovascular risk in early type 2 diabetes management.

Aim: Choosing the initial treatment for type 2 diabetes (T2D) is pivotal, requiring consideration of solid clinical evidence and patient characteristics. Despite metformin's historical preference, its efficacy in preventing cerebrovascular events lacked empirical validation. This study aimed to evaluate the associations between first-line monotherapy (metformin or non-metformin antidiabetic medications) and cerebrovascular complications in patients with T2D without diabetic complications.

Patients And Methods: We categorize elderly patients receiving major surgery into two groups according to the presence or absence of preoperative sarcopenia that were matched at a 1:4 ratio through propensity score matching (PSM). We analyzed 30-day or 90-day adverse postoperative outcomes and mortality in patients with and without sarcopenia receiving major surgery.

Associations of novel complete blood count-derived inflammatory markers with psoriasis: a systematic review and meta-analysis.

Psoriasis is an immune-mediated disorder which primarily affects skin and has systemic inflammatory involvement. Neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), and monocyte-to-lymphocyte ratio (MLR) are novel complete blood count (CBC)-derived markers which can reflect systemic inflammation. This study aimed to systematically investigate the associations of NLR, PLR, SII, and MLR with psoriasis.
