Long-term outcomes after resection of extra-ampullary duodenal adenocarcinomas: single-center experience.

J Gastrointest Surg

November 2024

Division of Hepatopancreatobiliary Surgical Oncology, Department of Surgical Oncology, Tata Memorial Hospital, Homi Bhabha National Institute, Parel, Mumbai, India. Electronic address:

Article Synopsis
  • Extra-ampullary duodenal adenocarcinoma is a rare cancer, and this study examined the long-term outcomes and survival factors in patients who underwent surgery for it.
  • The analysis included 59 patients, primarily males aged around 60, with most tumors located in the second part of the duodenum, and followed them for a median of 32 months post-surgery.
  • Key findings revealed that factors like having clear surgical margins, the type of adenocarcinoma, and the lymph node ratio significantly impacted overall survival rates, while the effectiveness of adjuvant chemotherapy is still uncertain.

Article Synopsis
  • The report details the clinical journey of a 35-year-old male kidney transplant patient suffering from post-transplant fever and suspected acute-on-chronic graft dysfunction with no clear infectious source identified through various diagnostic tests.
  • Initial treatments included antibiotics for persistent symptoms, leading to emergency hospitalization due to acute urinary retention, high creatinine levels, and proteinuria.
  • Further imaging with [F]FDG-PET/CT revealed prostatitis and abscess complications, but despite improvement in symptoms after treatment, the patient ultimately experienced worsened graft function and required dialysis after eight months.*

We herein present a patient initially suspected of multiple lytic skeletal metastasis of unknown primary on anatomical imaging. Metabolic imaging by [18F]-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) detected focal [18F]FDG uptake in the right thyroid nodule, mild [18F]FDG uptake in soft tissue lesion in the left inferior parathyroid region, and multiple nonavid osteolytic skeletal lesions. Fine-needle aspiration cytology of the right thyroid nodule showed papillary thyroid carcinoma (PTC).

Article Synopsis
  • The study focuses on evaluating the effectiveness of levofloxacin and levonadifloxacin against a newly identified pathogen that has shown increasing resistance to levofloxacin.
  • 116 clinical isolates were analyzed, revealing that approximately 40% were resistant to levofloxacin, but both antibiotics had a 98% inhibition rate against the tested isolates.
  • Although both antibiotics showed similar susceptibility, levonadifloxacin had lower minimum inhibitory concentrations (MICs), suggesting it might be a better option; further research is needed to confirm its clinical use.

Desmoid Fibromatosis of the Oesophagus Creating an Oesophageal Diverticulum in a 2-year-old Girl.

Afr J Paediatr Surg

July 2024

Department of Surgical Oncology, Division of Paediatric Surgical Oncology, Tata Memorial Hospital and Advanced Centre for Training Research and Education in Cancer, Tata Memorial Centre, Mumbai, Maharashtra, India.

Article Synopsis
  • - The case involves a 2-year-old girl who has a rare condition called extra-abdominal desmoid fibromatosis, which is linked to an esophageal tumor and also has traction diverticula.
  • - This situation presented a surgical challenge because it required the treatment of multiple health issues at once.
  • - The surgery successfully targeted both the desmoid fibromatosis and the diverticulum, ultimately helping to achieve clear margins in the surgical removal.

Purpose: Standard-dose immune checkpoint inhibitors (SD-ICIs) are the standard of care as initial therapy in microsatellite instable-high (MSI-H) advanced/metastatic colorectal adenocarcinomas (mCRC), but there are preclinical data to suggest that low-dose ICIs (LD-ICI) might also have similar efficacy.

Materials And Methods: A retrospective study of patients with MSI-H mCRC receiving ICIs between June 2017 and January 2023 was conducted. The primary end point of the study was 12-month progression-free survival (PFS), which was computed using the Kaplan-Meier method.

Uniqueness of lung cancer in Southeast Asia.

Lancet Reg Health Southeast Asia

August 2024

Department of Medical Oncology, Tata Memorial Hospital, Tata Memorial Centre, Mumbai, India.

Lung cancer varies between Caucasians and Asians. There have been differences recorded in the epidemiology, genomics, standard therapies and outcomes, with variations according to the geography and ethnicity which affect the decision for optimal treatment of the patients. To better understand the profile of lung cancer in Southeast Asia, with a focus on India, we have comprehensively reviewed the available data, and discuss the challenges and the way forward.

Background: The objective of this systematic review and meta-analysis was to pool randomized trials of patients undergoing left-sided colorectal anastomosis, comparing the use of fluorescence perfusion imaging versus visual inspection in reducing anastomotic leaks.

Methods: Databases searched included PubMed, Cochrane Library, Scopus, CINHAL (EBSCO), and Google Scholar based on the concepts: randomized, colorectal, anastomotic leak, and fluorescence imaging. The risk of bias was assessed using RoB2 and the certainty of the evidence with the GRADE Pro tool.

Tamoxifen, a selective estrogen receptor modulator (SERM), is a hormone therapy used for the treatment of estrogen receptor (ER)-positive breast cancer. We report the case of a 29-year-old premenopausal lady with a history of infertility treatments who was diagnosed with ER-positive infiltrating ductal carcinoma (IDC) of the breast. Following a modified radical mastectomy (MRM) and adjuvant systemic chemotherapy, tamoxifen was recommended as part of her adjuvant hormonal therapy.

Robotic Radical Cholecystectomy: Demonstrating Technical Equivalence to Open Surgery in Gallbladder Cancer.

Ann Surg Oncol

November 2024

Hepatobiliary and Retroperitoneal Sarcoma Division, Department of Surgical Oncology, Tata Memorial Hospital, Homi Bhabha National Institute, Mumbai, India.

Background: Open radical cholecystectomy is the current "gold standard" for the management of gallbladder cancer. In well-selected patients, robotic radical cholecystectomy (RRC) can be a suitable alternative offering immediate postoperative benefits, such as less blood loss, shorter hospital stay, and fewer complications, while being oncologically equivalent. However, it requires a longer learning curve.

Validation and Optimisation of the ISGPS Risk Classification for Postoperative Pancreatic Fistula after Pancreatoduodenectomy for Periampullary Tumours.

Ann Surg

August 2024

Gastrointestinal and Hepato-Pancreato-Biliary Service, Department of Surgical Oncology, Homi Bhabha National Institute, Tata Memorial Hospital, Parel, Mumbai, Maharashtra, 400012, India.

Article Synopsis
  • This study aims to validate the International Study Group of Pancreatic Surgery (ISGPS) classification for predicting clinically relevant pancreatic fistula (CRPF) specifically in patients with periampullary tumors (P-amps).
  • The authors tested the ISGPS model on data from 1,422 patients and found a CRPF rate of 22.2%, which is higher (25.8%) for P-amps, revealing the need for a tailored prediction model.
  • By optimizing the classification for P-amps, the adjusted ISGPS showed improved predictive ability (AUC=0.672) compared to the original model, highlighting the need for further research with larger, multicenter studies.

Oral malignancy that presents at a locally advanced stage needs complex surgical resections in which the maxillary cavity is usually left open. The constant maxillary secretions lead to problems like poor healing, fistula formation, and flap necrosis, causing longer hospital stays, delayed adjuvant therapy, and additional surgeries. Several methods have been tried to ameliorate this, each faced with its own difficulties.
