Background: Endogenous glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) regulate islet cell function. GLP-1 receptor agonists (GLP-1RAs) have been associated with an elevated risk of acute pancreatitis. Data on the pancreatic safety of tirzepatide (a dual GLP-1 and GIP agonist) and its effects on islet cell function in randomized controlled trials (RCTs) are scarce.

Background: The global health concern regarding the low prevalence of diagnosed diabetes mellitus (DDM) is evident, but the prevalence of DDM is increasing. This is attributed to the frequent underestimation of undiagnosed diabetes mellitus (UDM). Given the limited research on this matter in Bangladesh, there is a need to investigate sex differences in both the prevalence and risk factors of DDM among Bangladeshi adults.

Rheumatic heart disease (RHD) is one of the leading causes of valvular heart disease worldwide and still persists in the USA, particularly among vulnerable populations with limited healthcare. Depending on the risk, severity, and types of valve involvement, treatment includes guideline-directed medical therapy (GDMT) and surgical interventions like valve repair or replacement. Here, we present a unique case of a patient in his late fifties who presented with worsening heart failure symptoms and several heart murmurs.

Background: Traumatic intestinal perforation by foreign bodies is rare, with cases involving live fish being exceedingly uncommon, with only one reported case to date. We present a unique case of a 55-year-old fisherman who presented to the Emergency Department with traumatic intestinal perforation due to an eel fish accidentally entering his rectum. Despite initial reluctance to seek medical attention, prompt intervention was crucial to addressing peritonitis.

Background: Both dengue and Leptospira infections are endemic to tropical and subtropical regions, with their prevalence increasing in recent decades. Coinfection with these pathogens presents significant diagnostic challenges for clinicians due to overlapping clinical manifestations and laboratory findings. This case report aims to elucidate two clinical scenarios where the coinfection of dengue and leptospirosis complicates the disease course, creating a diagnostic conundrum.

Article Synopsis
  • A liver biopsy is an important diagnostic procedure for liver diseases, and while it's usually done using ultrasound guidance (PC-LB), there's a newer option called endoscopic ultrasound-guided liver biopsy (EUS-LB) that may offer advantages.
  • A study analyzed data from nine research papers involving 785 patients, focusing on diagnostic adequacy, accuracy, and the safety of EUS-LB versus PC-LB, finding that both methods performed similarly in most respect.
  • However, PC-LB outperformed EUS-LB in obtaining higher-quality specimens, which is essential for accurate diagnosis, leading the authors to suggest that clinicians should weigh specimen quality, procedural preferences, and expertise when choosing a biopsy
