A 71-year-old-man was referred to our hospital because of jaundice of the skin. On biochemical examination of blood, we identified an elevation in the levels of AST and ALT, the serum level of biliary enzymes, and the serum levels of tumor markers. We found pancreatic head cancer with invasion to the main blood vessels and duodenum, with liver metastases, on abdominal CT.

Background: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) attempts to improve the management of out-of-hospital cardiac arrest by laypersons who are unable to recognize cardiac arrest and are unfamiliar with CPR. Therefore, we investigated the sensitivity and specificity of our new DA-CPR protocol for achieving implementation of bystander CPR in out-of-hospital cardiac arrest victims not already receiving bystander CPR.

In two patients with frequent supraventricular tachycardia, echocardiograms revealed remarkable left ventricular systolic and diastolic dysfunction, associated with an absence of left ventricular filling during the early diastolic phase. This dysfunction was detected during normal sinus rhythm as well as tachycardia. By treatment with propafenone, both patients showed dramatic and rapid improvement of symptoms and the left ventricular dysfunction.
