Very Distal Femoral Periprosthetic Fractures: Replacement Versus Fixation: A Systematic Review.

J Orthop Trauma

November 2021

Division of Orthopaedic Surgery, Center for Evidence Based Orthopaedics, McMaster University, Hamilton, ON, Canada.

Article Synopsis
  • The study aims to analyze reoperations, complications, and clinical outcomes in elderly patients (65+) with very distal femur periprosthetic fractures (vDFPFs) treated with either a distal femoral locking plate (DFLP) or distal femoral replacement (DFR).
  • Researchers examined 25 studies, totaling 649 vDFPFs, using data from MEDLINE, Embase, and Web of Science, focusing on specific fracture classifications.
  • The findings suggest that patients treated with DFR had fewer reoperations and a shorter time to weight-bearing, but DFLP knees showed better functional outcomes and range of motion post-surgery, highlighting the need for further prospective studies.

Background: Inguinal disruption and femoroacetabular impingement (FAI) are well-recognized sources of groin pain in athletes; however, the relationship between inguinal disruption and FAI remains unclear. In cases of dual pathology, where both entities coexist, there is no definitive consensus regarding which pathology should be prioritized for treatment in the first instance.

Purpose: (1) To examine the 2-year effectiveness and clinical outcome in athletes presenting with dual pathology in which the FAI component alone was treated with arthroscopic deformity correction.

Hum Reprod Open

October 2020

Acute Gynaecology, Early Pregnancy, and Advanced Endosurgery Unit, Nepean Hospital, Kingswood, 2747 NSW, Australia.

Study Question: Is expectant management (EM) of tubal ectopic pregnancy (EP) an effective and safe treatment strategy when compared to alternative interventions?

Summary Answer: There is insufficient evidence to conclude EM yields a difference in the resolution of tubal EP, the avoidance of surgery or time to resolution of tubal EP when compared to intramuscular methotrexate in stable patients with β-hCG <1500 IU/l.

What Is Already Known: The utilisation of medical and surgical management for EP is well established. EM aims to allow spontaneous resolution of the EP without intervention.

Article Synopsis
  • This systematic review and meta-analysis aimed to determine if a high CO2 gap is a predictor of mortality in critically ill adults with circulatory shock.
  • The analysis included data from 21 studies with over 2,100 adult ICU patients, revealing that a high CO2 gap is associated with a significant increase in mortality, particularly in medical and surgical ICU patients.
  • Additionally, the study found a correlation between a high CO2 gap and worse physiological outcomes, such as higher lactate levels and lower cardiac index, but it did not correlate with longer ICU stays or increased use of certain therapies.
