iScience

October 2024

Division of Immunology and Infectious Disease Biology, INtegrative GENomics of HOst-PathogEn (INGEN-HOPE) laboratory, CSIR-Institute of Genomics and Integrative Biology (CSIR-IGIB), Mall Road, Delhi 110007, India.

Article Synopsis
  • This study explores the variations in T cell receptor (TCR) responses in dengue patients, particularly focusing on those with severe infections.
  • Analysis of RNA-seq data from 112 patients revealed that severe cases had notable clinical signs such as low white blood cells and specific TCR chain diversity, suggesting unique immune responses.
  • The research also found that genetic mutations in the dengue virus corresponded with changes in specific TCR chains, emphasizing the importance of TCR dynamics in understanding dengue disease progression and potential future infections.

Adverse cardiovascular (CV) events have declined in Western countries due at least in part to aggressive risk factor control, including dyslipidemia management. The American and European (Western) dyslipidemia treatment guidelines have contributed significantly to the reduction in atherosclerotic cardiovascular disease (ASCVD) incidence in the respective populations. However, their direct extrapolation to Indian patients does not seem appropriate for the reasons described below.

  • New diagnostic criteria have classified small-for-size syndrome (SFSS) after living donor liver transplantation into three severity groups, and this study focuses on grade A SFSS and its mortality risk.
  • Data from 406 patients were analyzed, categorizing them into up-trend, down-trend, and ascites groups based on their bilirubin levels and ascites presence after surgery.
  • While survival rates were similar for SFSS and non-SFSS patients, those in the up-trend group had significantly higher 90-day mortality and should receive aggressive intervention if they have additional risk factors.

Mirizzi syndrome, although rare, is a potential complication of long-standing gallstone disease, particularly cholecystolithiasis. Due to the nonspecific nature of its symptoms, this condition often remains undiagnosed prior to surgery in most cases. While minimally invasive approaches are generally safe in expert hands, they can be challenging and entail the risk of bile duct injuries, often necessitating conversion to bail-out procedures.

Phytobezoars are solid masses of indigestible plant material and are a common type of gastrointestinal bezoar, with varying incidences globally. These bezoars typically form from the ingestion of high-fiber fruits and vegetables and are associated with factors such as decreased gastric acid production and delayed gastric emptying. We present a case of a 35-year-old healthy man with recurrent upper abdominal pain, nausea, a rolling ball sensation in the abdominal region, and a history of consuming unripe persimmons.

Deciphering the iron enigma: Navigating the complexities of iron metabolism in critical illness.

World J Clin Cases

September 2024

Department of Critical Care Medicine, Max Super Speciality Hospital, New Delhi 110017, India.

Iron is a double-edged sword! Despite being essential for numerous physiological processes of the body, a dysregulated iron metabolism can result in tissue damage, exaggerated inflammatory response, and increased susceptibility to infection with certain pathogens that thrive in iron-rich environment. During sepsis, there is an alteration of iron metabolism, leading to increased transport and uptake into cells. This increase in labile iron may cause oxidative damage and cellular injury (ferroptosis) which progresses as the disease worsens.

The current scenario of employing loop diuretics in combination with guideline-directed medical therapy (GDMT) demonstrates a comprehensive approach to improving clinical outcomes in individuals with heart failure (HF). GDMT uses four types of drugs: angiotensin receptor-neprilysin inhibitors (ARNIs), beta-blockers (BBs), mineralocorticoid receptor antagonists (MRAs), and sodium-glucose cotransporter-2 inhibitors (SGLT2i). Torsemide, furosemide, and bumetanide are common loop diuretics used to control fluid overload in HF and provide symptomatic relief.
