A hierarchical self-attention-guided deep learning framework to predict breast cancer response to chemotherapy using pre-treatment tumor biopsies.

Med Phys

December 2023

Department of Electrical Engineering and Computer Science, Lassonde School of Engineering, York University, Toronto, Ontario, Canada.

Background: Pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) has demonstrated a strong correlation to improved survival in breast cancer (BC) patients. However, pCR rates to NAC are less than 30%, depending on the BC subtype. Early prediction of NAC response would facilitate therapeutic modifications for individual patients, potentially improving overall treatment outcomes and patient survival.

Article Synopsis
  • This study investigated the effectiveness of postoperative radiation therapy for low and intermediate grade cancers in the parotid and submandibular glands.
  • A retrospective analysis of 621 patients treated between 2010 and 2020 showed that those who received radiation had a lower rate of locoregional recurrence (LRR).
  • The findings suggest that while postoperative radiation can benefit certain cancer patients, it is not necessary for early-stage, low-grade cases without nodal disease and negative margins.

Article Synopsis
  • - Patients with diffuse glioma, especially those with glioblastoma, have a high risk of developing venous thromboembolism (VTE), with incidences reaching up to 30%, and ongoing research is aiming to identify risk factors, though prophylactic measures beyond surgery haven't been established.
  • - Certain genetic markers, like IDH wild-type glioma, are linked to a higher VTE risk, and mutations in IDH may interfere with the production of blood clotting factors.
  • - Recommended treatment for VTE includes low molecular weight heparin (LMWH) or direct oral anticoagulants (DOACs), but due to the risk of intracranial hemorrhage (ICH), particularly in glioblastoma

The main objective of this study is to develop a robust deep learning-based framework to distinguish COVID-19, Community-Acquired Pneumonia (CAP), and Normal cases based on volumetric chest CT scans, which are acquired in different imaging centers using different scanners and technical settings. We demonstrated that while our proposed model is trained on a relatively small dataset acquired from only one imaging center using a specific scanning protocol, it performs well on heterogeneous test sets obtained by multiple scanners using different technical parameters. We also showed that the model can be updated via an unsupervised approach to cope with the data shift between the train and test sets and enhance the robustness of the model upon receiving a new external dataset from a different center.

Introduction: Contrast-enhanced MRI is routinely performed as part of preoperative work-up for patients with Colorectal Cancer Liver Metastases (CRLM). Radiomic biomarkers depicting the characteristics of CRLMs in MRI have been associated with overall survival (OS) of patients, but the reproducibility and clinical applicability of these biomarkers are limited due to the variations in MRI protocols between hospitals.

Methods: In this work, we propose a generalizable radiomic model for predicting OS of CRLM patients who received preoperative chemotherapy and delayed-phase contrast enhanced (DPCE) MRIs prior to hepatic resection.
