Background: Nearly one-third of the patients with interstitial lung disease (ILD) require surgical biopsy for a definite diagnosis. Video-assisted thoracoscopic surgical (VATS) biopsy has replaced open lung biopsy, but the number of biopsy required to achieve an accurate diagnose is controversial.

Objectives: Our study aims to show that a well-planned single VATS biopsy is as effective as multiple biopsies for the accurate diagnosis of ILD by reduced days of hospital stay.

University of Health Sciences, Ankara Atatürk Chest Diseases and Thoracic Surgery Training and Research Hospital, Anesthesiology and Reanimation Clinic, Ankara, Turkey.

The Assess Respiratory Risk in Surgical Patients in Catalonia (ARISCAT) risk index, which is frequently used in nonthoracic surgery, may not be sufficient to predict postoperative pulmonary complications (PPCs). We aimed to evaluate the effectiveness of the ARISCAT risk index, ASA, preoperative albumin level, neutrophil/lymphocyte ratio (NLR), and other parameters in predicting PPCs after thoracic surgery. Patients undergoing elective thoracic surgery with 1-lung ventilation (OLV) were prospectively analyzed.

Article Synopsis
  • The study compares four diagnostic criteria systems for allergic bronchopulmonary aspergillosis (ABPA): ISHAM, Rosenberg-Patterson, ABPA-S, and ABPA-CB, to determine which is most effective.
  • In a retrospective evaluation of 10 ABPA patients, ISHAM criteria diagnosed the most cases (7), while no patients met the ABPA-S and ABPA-CB criteria when assessed independently.
  • The findings suggest ISHAM criteria are superior for diagnosing patients with high total IgE levels (above 1000 IU/mL), but all criteria may be adequate for diagnosing those with lower levels.

Article Synopsis
  • - The study aimed to compare pain relief after video-assisted thoracoscopic surgery (VATS) using three methods: ultrasound-guided erector spinae plane block (ESPB), thoracic paravertebral block (TPVB), and a combination of both (comb-group), with 75 patients participating.
  • - Results showed that patients receiving TPVB experienced higher pain levels (measured by Visual Analog Scale) and required more morphine and rescue analgesics compared to those in the ESPB or comb-group.
  • - The findings suggest that ESPB alone and the combination of ESPB and TPVB were more effective in managing acute pain post-surgery than TPVB alone, indicating the potential benefits of using ESPB for

Purpose Of Review: This expert opinion, prepared by a panel of chest disease specialists, aims to review the current knowledge on practice patterns in real-life management of mild asthma and to address the relevant updates in asthma treatment by The Global Initiative for Asthma (GINA) to guide clinicians for the best clinical practice in applying these new treatment paradigms.

Recent Findings: On the basis of the emerging body of evidence suggesting the non-safety of short-acting β2-agonists (SABA)-only therapy and comparable efficacy of the as-needed inhaled corticosteroids (ICS)-formoterol combinations with maintenance ICS regimens, GINA recently released their updated Global Strategy for Asthma Management and Prevention Guide (2019). The new GINA 2019 recommendations no longer support the SABA-only therapy in mild asthma but instead includes new off-label recommendations such as symptom-driven (as-needed) low-dose ICS-formoterol and "low dose ICS taken whenever SABA is taken.

Effects of hydrodissection on anesthesia characteristics in ultrasound guided infraclavicular brachial plexus blockade.

Medicine (Baltimore)

June 2022

University of Health Sciences, Ankara Atatürk Chest Diseases and Thoracic Surgery Training and Research Hospital, Anesthesiology and Reanimation Clinic, Ankara, Turkey.

Background: The development of ultrasonography technology and its widespread application have increased peripheral nerve block applications, especially in limb surgeries, since it reduces complication rates and increases success rates in peripheral nerve block applications. However, even experienced physicians need to direct a large number of needles and injections for adequate local anesthetic spread, which can cause accidental vascular puncture and local anesthetic systemic toxicity. Such complications can be prevented by hydrodissection and a safer and successful anesthesia can be provided to patients in this study, it was aimed to investigate the effects of hydrodissection on anesthesia characteristics in the infraclavicular brachial plexus blockade.
