Article Synopsis
  • Researchers studied how axillary surgery practices have changed over time following neoadjuvant chemotherapy (NAC) in breast cancer patients enrolled in the I-SPY2 trial from 2011 to 2021.
  • The findings showed a significant decrease in axillary lymph node dissection (ALND) procedures, particularly in patients who were clinically node-positive (cN+) at diagnosis, with a corresponding increase in the use of sentinel lymph node (SLN) surgery.
  • These trends indicate a shift in surgical practices, with less extensive surgery being performed after NAC, particularly for patients showing node positivity before treatment, reflecting evolving clinical approaches prior to further research outcomes.

Does spinal deformity affect adolescents' quality of life before we tell them it should?

Spine Deform

September 2023

Pediatric Orthopedic Surgery Department, Stanford Hospital and Clinics, 300 Pasteur Dr. Edwards Building R107, Stanford, CA, 94305, USA.

Article Synopsis
  • The study aimed to understand if knowledge about scoliosis and counseling by orthopedic surgeons affects the health-related quality of life (HRQoL) in adolescents with scoliosis.
  • Patients aged 10-18 were evaluated using the SRS-22r questionnaire before and after their first orthopedic visit, with 52 participants showing no significant changes in their overall HRQoL scores.
  • Findings suggest that surgeon counseling has minimal impact on adolescents’ perceptions of their HRQoL concerning scoliosis.

Mandibular distraction osteogenesis (DO) is mediated by skeletal stem cells (SSCs) in mice, which enact bone regeneration via neural crest re-activation. As peripheral nerves are essential to progenitor function during development and in response to injury, we questioned if denervation impairs mandibular DO. C57Bl6 mice were divided into two groups: DO with a segmental defect in the inferior alveolar nerve (IAN) at the time of mandibular osteotomy ("DO Den") and DO with IAN intact ("DO Inn").

Patients treated with cytotoxic therapies, including autologous stem cell transplantation, are at risk for developing therapy-related myeloid neoplasms (tMN). Preleukemic clones (ie, clonal hematopoiesis [CH]) are detectable years before the development of these aggressive malignancies, although the genomic events leading to transformation and expansion are not well defined. Here, by leveraging distinctive chemotherapy-associated mutational signatures from whole-genome sequencing data and targeted sequencing of prechemotherapy samples, we reconstructed the evolutionary life-history of 39 therapy-related myeloid malignancies.

Objective: Tongue base and hypopharynx are the major sites of obstruction in OSA patients with failed palatal surgery. In recent years, several minimally invasive procedures have been developed to address tongue base obstruction. However, the research focus has consistently been on the effectiveness of surgery in reducing obstructive sleep apnoea rather than on postoperative complications.

Thinking Outside the Operating Room: Guidance on Designing a Safe and Effective Minor Procedure Room.

J Hand Surg Am

January 2023

Division of Plastic and Reconstructive Surgery, Stanford Hospital and Clinics, Palo Alto, CA; Department of Surgery, US Department of Veterans Affairs, Palo Alto Healthcare System, Palo Alto, CA.

The minor procedure room (MPR) offers numerous advantages over the traditional operating room for performing many common hand surgeries. MPRs require less space, are subject to more practical architectural design standards, and facilitate more judicious use of disposable materials and unnecessary instruments than common hand surgeries. MPRs reduce costs to the system and patient at every step of the surgical workflow and improve efficiency by removing preoperative and postoperative monitoring requirements.

Unlabelled: Necrosis of the nipple-areolar complex (NAC) is a major complication of breast surgery that most commonly results from venous congestion. Several conservative rescue therapies have been proposed for relieving NAC congestion, but each carries certain drawbacks, including cost and side effect profile. In this study, we evaluated the effectiveness of topical dimethyl sulfoxide (DMSO), an inexpensive compound with vasodilatory, free radical scavenging, and antiinflammatory properties in rescuing congested NACs.

Sexually dimorphic tissues are formed by cells that are regulated by sex hormones. While a number of systemic hormones and transcription factors are known to regulate proliferation and differentiation of osteoblasts and osteoclasts, the mechanisms that determine sexually dimorphic differences in bone regeneration are unclear. To explore how sex hormones regulate bone regeneration, we compared bone fracture repair between adult male and female mice.

Background: Preclinical models have provided key insights into the response of local tissues to radiofrequency (RF) renal denervation (RDN) that is unobtainable from human studies. However, the anatomic translatability of these models to the procedure in humans is incompletely understood.  Aims: We aimed to compare the renal arterial anatomy in normotensive pigs treated with RF-RDN to that of human cadavers to evaluate the suitability of normotensive pigs for determining the safety of RF-RDN.

Dual antiplatelet therapy (DAPT) is a management cornerstone for intracranial aneurysms treated with flow diversion. However, combined dual antiplatelet plus anticoagulation (triple therapy) can be indicated in some patients with important associated risks. Here we present the case of a 72-year-old woman with prior history of subarachnoid hemorrhage who was started on triple therapy (enoxaparin and DAPT) following successful flow diversion of an enlarging but unruptured left fetal posterior communicating artery aneurysm.

Introduction: The Accreditation Council for Graduate Medical Education (ACGME) offers descriptions of competencies and milestones but does not provide standardized assessments to track trainee competency. Entrustable professional activities (EPAs) and special assessments (SAs) are emerging methods to assess the level of competency obtained by regional anesthesiology and acute pain medicine (RAAPM) fellows.

Methods: A panel of RAAPM physicians with experience in education and competency assessment and one medical student were recruited to participate in a modified Delphi method with iterative rounds to reach consensus on: a list of EPAs, SAs, and procedural skills; detailed definitions for each EPA and SA; a mapping of the EPAs and SAs to the ACGME milestones; and a target level of entrustment for graduating US RAAPM fellows for each EPA and procedural skill.
