A 30 degrees arthroscope is introduced via the posterior soft spot portal, and an anterosuperior portal is created with the use of a 7-mm disposable cannula. Objective: To determine whether 123 I can be used as a safe and effective alternative tracer to 131 I for imaging remnant tissue and for searching for metastatic lesions of well-differentiated thyroid cancer.

Methods: We studied a series of 16 patients (10 women and 6 men; 29 to 73 years of age) who had well-differentiated thyroid cancer and had undergone thyroidectomy and subsequent radioiodine treatment. Diagnostic 10 mCi (either preablation or postablation) were compared with the 5- to 7-day 131 I posttherapy scans (doses, 75 to 200 mCi).

Objective: To determine whether a 5-mCi dose of 123I can be used as an effective radiotracer for assessing the presence of remnant thyroid tissue and for searching for metastatic lesions in patients with well-differentiated thyroid cancer as well as to attempt to ascertain whether a scan performed only at 4 hours is sufficient for accurate diagnosis and might replace the conventional protocol of scanning at both 4 hours and 24 hours.

Methods: We prospectively studied 27 patients who had undergone near-total thyroidectomy and had a documented diagnosis of well-differentiated thyroid carcinoma. Patients underwent scanning after receiving a 5-mCi dose of 123I, at a time when they had discontinued thyroid replacement therapy and had a thyrotropin level in excess of 30 mIU/mL.

A 28-yr-old woman who sustained a mild traumatic brain injury had an 18F-fluorodeoxyglucose (FDG) PET brain study using coincidence imaging performed on a dual-head gamma camera (MCD version; ADAC, Milpitas, CA) followed 24 hr later by 99mTc-hexamethyl propyleneamine oxime SPECT brain perfusion imaging using a triple-head gamma camera (TRIONIX, Twinsburg, OH). The results of the SPECT brain perfusion study demonstrated decreased frontoparietal cortical brain perfusion, whereas 18F-FDG cerebral uptake was normal. Neuropsychological evaluation suggested frontal lobe involvement.

Postmortem examination of the lungs of a patient with advanced AIDS who had developed pulmonary arterial hypertension late in the course of the illness demonstrated extensive cytomegalovirus (CMV) infection in endothelial cells of the lung microvasculature. Enlarged CMV-infected endothelial cells were present in virtually all histologic sections of the lungs, protruded into and compromised the lumens of the small vessels they lined, and were estimated by image cytometry of immunohistochemically stained sections to comprise 0.8% of the total lung tissue volume.

The purpose of this atlas is to present a review of the literature showing the advantages of SPECT brain perfusion imaging (BPI) in mild or moderate traumatic brain injury (TBI) over other morphologic imaging modalities such as x-ray CT or MRI. The authors also present the technical recommendations for SPECT brain perfusion currently practiced at their center. For the radiopharmaceutical of choice, a comparison between early and delayed images using Tc-99m HMPAO and Tc-99m ECD showed that Tc-99m HMPAO is more stable in the brain with no washout over time.

Recognition of brain injury in asymptomatic carotid artery injuries with conventional methods can be difficult. We present a case of angiographically proven asymptomatic left internal carotid artery occlusion with normal CT after a gunshot wound. The SPECT brain perfusion imaging showed mild generalized supratentorial hypoperfusion of the bilateral cerebral cortices on the left side and severe left temporal lobe hypoperfusion.

To examine the relationship between acyclovir use and survival in AIDS, we performed a retrospective analysis of data collected through an observational cohort of the 17-site Community Program for Clinical Research on AIDS (CPCRA), under the sponsorship of the National Institute of Allergy and Infectious Diseases. Data were analyzed regarding 2,368 patients with CD4+ lymphocyte counts of < 500/mm3, and 7,836 follow-up visits were conducted from September 1990 to July 1994. Factors associated with use of acyclovir were studied by stratified analysis of variance and Mantel-Haenzel chi 2 tests.

A 79-yr-old woman with asymptomatic subclavian vein thrombosis associated with transvenous pacemaker electrode and congestive heart failure is reported. The subclavian thrombosis was discovered accidentally from the first-pass radionuclide angiogram that is routinely performed with the intravenous bolus injection of the radiopharmaceutical for a gated blood-pool study. It demonstrated venous obstruction at the level of the subclavian vein and abnormal collateral circulation over the chest wall consistent with subclavian thrombosis.

The indications, advantages, complications, and benefits of peripheral neurectomy in patients with trigeminal neuralgia were studied in detail in 40 patients treated between 1982 and 1991. Twenty-eight patients had previously received radiofrequency thermocoagulation: peripheral neurectomy was performed for pain recurrence. These patients had excellent or good pain relief for at least 5 years postsurgery.

A case fulfilling the criteria for the diagnosis of diabetic amyotrophy is reported. Based on the clinical and electrodiagnostic features, it is concluded that diabetic amyotrophy is a recognizable clinical entity that can be differentiated from other diabetic neuropathies. The site of the lesion and the pathogenesis in diabetic amyotrophy remain controversial.
