Introduction: Venous thromboembolic events (VTE) occur in more than 5% of hospitalized patients with acute CMV infection. In immunocompetent patients, splanchnic vein thrombosis (SVT) and splenic infarction have been suggested to represent approximately half of the published cases of CMV-associated VTE. We performed a systematic review of the literature with the aim to describe epidemiology, clinical characteristics, treatment, and natural history of CMV-associated SVT in immunocompetent patients.

Department of Obstetrics, Gynecology and Gynecologic Oncology, St. Stephen Hospital, Nagyváárad Tér 1, Budapest, Hungary.

Introduction: Recurrence originating from the pelvic lymph node containing fibro-fatty tissue has consistently been identified as the most frequent pattern of treatment failure in early-stage cervical cancer. A surgical technique for the complete removal of the connective tissue content of the pelvis was introduced at St. Stephen Hospital in 1993 to improve oncological outcome by reducing the risk of recurrence from the pelvis.

Department of Obstetrics, Gynecology and Gynecologic Oncology, St. Stephen Hospital, Budapest, Hungary.

Introduction: Since 1993 an operative technique without adjuvant therapy (laterally extended parametrectomy, the LEP procedure) has been in use at our institution for the treatment of Stage IIB cervical cancer and for patients with pelvic lymph node metastases in Stages IA-IIA. Iliac/femoral artery embolic occlusion in the cohort of LEP operated patients was studied in an 11 years long period.

Methods: The LEP-Wertheim procedure was used in 320 patients between 1994 and 2005.

Surgical treatment of lymph node metastases in stage IB cervical cancer: the laterally extended parametrectomy (LEP) procedure.

Int J Gynecol Cancer

February 2004

Department of Obstetrics, Gynecology and Gynecologic Oncology, St. Stephen Hospital, Budapest, Hungary.

A surgical method has been introduced for the treatment of early stage cervical cancer patients with pelvic lymph node metastases. The procedure was used without any adjuvant treatment in 31 stage IB cervical cancer patients, where pelvic lymph node metastases were proven by intraoperative histology. Two patients were lost for follow-up.

A new surgical method was introduced for the treatment of Ib lymph node positive and IIb cervical cancer patients. The lateral resection plane corresponds to the true pelvic side wall, the plane represented by the internal obturator muscle, the linea arcuata, and the piriformis muscle with the convergent branches of the sacral plexus. The LEP procedure overcomes the limitations of the standard class III-IV radical hysterectomy, which leaves in situ the gluteal superior, inferior and pudendal nodes, thus improving local control and survival.

An alternative method to laparoscopic surgery has been developed for urological procedures. The surgery is minimal access because the length of the single skin incision ranges from 3-6 cm depending on the type of operation. It is direct access because the surgeon sees the operative area directly and stereoscopically by eye without video-optical support.

A simplified version of the "Indiana pouch" technique for urinary diversion. Experience of the first 18 cases.

Eur J Gynaecol Oncol

June 1995

Department of Obstetrics, Gynaecology and Gynaecological Oncology, St. Stephen Hospital, Budapest.

During the last 3 years, a modified technique of the continent urinary diversion, known as the "Indiana pouch" has been performed in 18 patients as part of pelvic exenteration. The narrowing of the ileum and/or the site of Bauchin valve was excluded from the procedure. In order to achieve anti reflux effect, ureters were implanted to the urinary reservoir by the "split cuff nipple" technique instead of tunnelling ureters.

Serum elastase-type activity, elastase inhibitory capacity and their relation to lipids were examined in 140 male patients with ischaemic vascular disease (coronary, cerebral, peripheral) and in 60 control subjects. Serum elastase-type activity was found to be significantly lower, inhibitory capacity significantly higher, in the groups of patients than in the controls. HDL- and HDL2 cholesterol as well as apo A concentration showed significant negative correlation with elastase inhibitory capacity both in atherosclerotic and in control subjects.
