Progress in identification of the recurrent laryngeal nerve in thyroid and parathyroid surgery has lagged behind the identification of other head and neck nerves. Attempts to use electronic pulsed nerve stimulators with evoked response electromyography have failed due to the absence of a convenient and effective method of monitoring the larynx. A Postcricoid Laryngeal Surface Electrode as herein described shows excellent initial results as an electrode for electromyographic monitoring of the larynx for recurrent laryngeal nerve identification.

This is a case report of a 21-year-old man who had a gradually progressive Class III malocclusion and crepitation of the right temporomandibular joint. Routine TMJ tomograms revealed a radiopaque-radiolucent lesion of the right mandibular condyle with a compensatory anteroinferior displacement of the left mandibular condyle. Nuclear bone scans confirmed increased asymmetric technetium 99 tracer uptake in the region of the right temporomandibular joint.

Twenty patients underwent bilateral sagittal ramus osteotomy for the correction of mandibular retrognathia. A condylar positioning device (CPD) was used intraoperatively in 10 patients to maintain preoperative condyle-proximal segment position, while the CPD was not used in the other 10 patients. Postoperatively, the condyle-proximal segment positions in both groups were compared and evaluated for vertical, horizontal, and rotational changes.

Twenty-nine patients (58 joints) were tomographically evaluated for condylar position and assessed for temporomandibular joint function following superior maxillary repositioning with mandibular autorotation. Seventy-six percent of the condyles showed postoperative posterior repositioning, whereas tomographic measurement of the joint spaces revealed significantly posterior condylar position (P less than .01) relative to the preoperative position.

One hundred consecutive patients who had coronary artery bypass grafting using both internal thoracic arteries (ITAs) and saphenous veins, operated on during a 3-year period between 1972 and 1975, have been compared retrospectively with a series of 100 patients operated on during the same period who had one ITA graft along with saphenous vein grafts. The two groups were similar with respect to age, sex, risk factors for coronary artery disease, angina class, extent of coronary artery disease, left ventricular function, number of coronary bypass grafts performed, and completeness of revascularization. Single ITA operative mortality was 2% and double ITA, 9% (p = NS).

Congenital diaphragmatic hernia complicating pregnancy is a rarity, accounting for only six out of 17 cases of diaphragmatic hernia reported in the English literature. This case report describes the first successful repair of an acutely symptomatic foramen of Bochdalek hernia during pregnancy, with maternal and fetal survival. In the asymptomatic patient, surgery should be performed promptly on an elective basis in the first and second trimesters.
