Introduction: Evidence-based clinical practice guidelines drive optimal patient care and facilitate access to high-quality treatment. Creating guidelines for rare diseases such as haemophilia, where evidence does not often come from randomized controlled trials but from non-randomized and well-designed observational studies and real-world data, is challenging. The methodology used for assessing available evidence should consider this critical fact.

Article Synopsis
  • * The study used whole exome sequencing (WES) on a family from India to explore the genetic causes of ASDs in siblings displaying clinical features associated with Smith-Magenis syndrome (SMS).
  • * Two inherited variants were found that contribute to the SMS features in the siblings, demonstrating how WES can shed light on the genetic complexity of syndromic ASDs and inform understanding of both syndromic and idiopathic types of ASD.

Background Benign prostatic hyperplasia (BPH) is the most common urological condition affecting older men, leading to lower urinary tract symptoms (LUTS) and complications such as acute urinary retention (AUR), which can significantly impact quality of life. This study aims at comparing the postoperative outcomes and complication rates of monopolar transurethral resection of the prostate (TURP) in BPH patients with and without AUR. By examining the differences in postoperative outcomes, the study seeks to highlight the impact of AUR on complications such as UTIs, prolonged catheterization, and extended hospital stays, and assess the potential clinical implications for improving treatment strategies in these patients.

Hypertonic saline in acute decompensated heart failure (HSHF).

Indian Heart J

December 2024

Division of Epidemiology, Biostatistics and Population Health, St. John's Research Institute, Bangalore, India.

Hypertonic saline with high dose furosemide improves refractory heart failure. In this case series of 10 patients of refractory acute decompensated heart failure, effect of HSS with furosemide was compared with furosemide alone. Patients responded to the therapy better, with faster decongestion, without significant adverse effects or worsening renal function.

Objective: Dengue has emerged as the most widely spread mosquito-borne disease, hyperendemic in India. Although severe dengue occurs only in a small proportion of cases, delays in the recognition and management can result in significant morbidity and mortality. Risk stratification of children in the emergency care and identifying those at risk for worsening during hospital stay facilitates optimum utilization of health care resources.

Background Anterior cruciate ligament (ACL) injuries are prevalent, especially among active individuals, and are increasingly observed even in Indian women who are not active in sports. ACL reconstruction (ACLR) is commonly performed using various graft options, but optimal choices for women remain debated. The aim of our study was to examine the use of the peroneus longus tendon as an autograft for ACLR in Indian female patients.

  • A 39-year-old female with a history of fever and weight loss presented with altered mental status and aphasia, leading to an MRI that revealed an acute stroke.
  • Thrombolysis with alteplase was initiated, but the patient experienced a parenchymal bleed, subsequently diagnosed as a stroke caused by infective endocarditis from undiagnosed mitral valve prolapse.
  • The case emphasizes the need for emergency physicians to consider infectious causes in young stroke patients before administering thrombolysis, as it can result in harmful outcomes.

Mammalian colonic contribution of amino acids to whole-body homeostasis.

Curr Opin Clin Nutr Metab Care

January 2025

Department of Physiology, St. John's Medical College, St. John's National Academy of Health Sciences, Bengaluru, India.

Purpose Of Review: The colon is an effective bioreactor with auxotrophic microbiota that can serve by hydrolyzing dietary and endogenous protein, as well as by synthesizing essential amino acids through nitrogen salvage. Due to assumed negligible amino acid absorption, this colonic contribution was thought to be minimal, but this may not be true.

Recent Findings: Several studies that examined the colonic environment in health and disease, show the presence of proteases in the colonic lumen, which are of both host and microbial origin, along with indirect evidence of amino acid transporters in the colonic epithelium.
