The aim of this study is to validate a uniform method for measuring perineal descent which can be used for different imaging methods, to establish cut-off values for this measurement, and to assess diagnostic test accuracy (DTA) of imaging techniques using these cut-off values. Secondly, the study aims to correlate perineal descent to symptoms, signs and imaging findings in women with obstructed defaecation syndrome (ODS) to assess its clinical relevance. Cross-sectional study of 131 women with symptoms of ODS.

Fungal infections are a major contributor to mortality for people with HIV/AIDS. Recently, researchers, clinicians, industry experts, and policymakers convened for the triennial AIDS-Related Mycoses Meeting to address critical gaps in the management of AIDS-related mycoses. Delegates highlighted several scientifically driven milestones that have significantly reduced fungal-related deaths, and the need to address outstanding challenges in diagnosis, treatment, and research.

Article Synopsis
  • * A new technique called polymerase error rate sequencing (PER-seq) was developed to measure DNA polymerase error rates, revealing that a common Pol ε mutant (P286R) significantly increases CpG>TpG errors similar to those seen in cancer tumors.
  • * Wild-type Pol ε shows a much higher error rate when replicating 5mCpG as opposed to regular cytosines, indicating that replication errors represent a crucial factor in CpG>TpG mutagenesis and altering the

Aims: To determine whether obesity and malnutrition have a synergistic effect on outcomes from skeletal trauma or elective orthopaedic surgery.

Methods: Electronic databases including MEDLINE, Global Health, Embase, Web of Science, ScienceDirect, and PEDRo were searched up to 14 April 2024, as well as conference proceedings and the reference lists of included studies. Studies were appraised using tools according to study design, including the Oxford Levels of Evidence, the Institute of Health Economics case series quality appraisal checklist, and the CLARITY checklist for cohort studies.

  • Muslims fast from dawn until dusk during Ramadan, which may increase the risk of accidents, especially for drivers.
  • A study analyzed 17 studies with over 964,000 subjects to assess if trauma incidence rises during this month, finding that road traffic accidents (RTAs) and occupational injuries (OIs) were notably higher, particularly around sunset.
  • Despite some evidence suggesting specific injury types don't increase, overall conclusions are limited due to methodological issues and a lack of studies from the Americas and Europe.

Chronic consequences of accidental dural puncture and postdural puncture headache in obstetric anaesthesia - sieving through the evidence.

Curr Opin Anaesthesiol

October 2024

Department of Anesthesiology and Intensive Care Medicine, Women's Wellness and Research Centre, Hamad Medical Corporation, Doha, Qatar.

Purpose Of Review: Accidental dural puncture (ADP) and postdural puncture headache (PDPH) are relatively common complications of neuraxial anaesthesia and analgesia in obstetrics. Both may result in acute and chronic morbidity. This review intends to discuss the chronic implications of ADP and PDPH and raise awareness of severe and potentially life-threatening conditions associated with them.

Background: Digital self-management platforms have been proposed as a solution to better support people manage their musculoskeletal (MSK) condition. However, research studies often explicitly exclude people who can't access internet connected devices. An important consideration is that digital exclusion (for people without access to an internet connected device) from these new digital options will worsen already existing inequality and inequity in healthcare.

In response to the growing global threat of fungal infections, in 2020 the World Health Organisation (WHO) established an Expert Group to identify priority fungi and develop the first WHO fungal priority pathogen list (FPPL). The aim of this systematic review was to evaluate the features and global impact of invasive infections caused by Pichia kudriavzevii (formerly known as Candida krusei). PubMed and Web of Science were used to identify studies published between 1 January 2011 and 18 February 2021 reporting on the criteria of mortality, morbidity (defined as hospitalisation and length of stay), drug resistance, preventability, yearly incidence, and distribution/emergence.

Cryptococcosis is a major worldwide disseminated invasive fungal infection. Cryptococcosis, particularly in its most lethal manifestation of cryptococcal meningitis, accounts for substantial mortality and morbidity. The breadth of the clinical cryptococcosis syndromes, the different patient types at-risk and affected, and the vastly disparate resource settings where clinicians practice pose a complex array of challenges.

Embedding genomics across the NHS: a primary care perspective.

Future Healthc J

November 2023

St George's University of London, London, UK, and joint clinical champion in genomics medicine, Royal College of General Practitioners.

Primary care remains the point of access to the NHS as well as having key roles in care coordination and prescribing. Therefore, embedding of genomic medicine in the NHS relies on successful implementation into the primary care landscape. Primary care is currently facing considerable challenges, including increasing numbers of patients and consultations per GP, multiple health conditions and polypharmacy, all contributing to increasing workload within a resource-constrained system.

HIV-associated cryptococcal meningitis remains a key driver of AIDS-related mortality. Mortality is twice as high in those who present later to care and with severe symptoms such as confusion. We embedded a qualitative methods study within a randomised controlled trial in Gaborone, Botswana and Kampala, Uganda with the aim of understanding pathways to care.

Physiotherapy

March 2024

Department of Physical Therapy, Faculty of Rehabilitation Medicine, University of Alberta, Edmonton, Canada; Tufts University, School of Medicine, Department of Public Health & Community Medicine, Division of Physical Therapy, Phoenix, AZ, USA.

Introduction: The need to address increasing numbers of people seeking care, insufficient numbers of physicians, and providing high-value and sustainable care has contributed to changing physiotherapy practice across the world, often referred to as advanced practice physiotherapy. Currently, there is no internationally standardized competency and capability framework to support advanced practice physiotherapy.

Objectives: This scoping review has two aims; 1) To identify and map out the competencies of advanced practice physiotherapy available in the literature.

  • Pregnant women infected with SARS-CoV-2 face increased risks, including preterm birth and higher rates of stillbirth or neonatal admissions, highlighting ongoing concerns even after the WHO declared an end to the COVID-19 pandemic in May 2023.
  • A systematic review was performed to analyze the prevalence of early pregnancy loss in women with SARS-CoV-2 and compare these results to those of non-infected pregnant women.
  • The review included various studies that reported pregnancy losses before 20 weeks, as well as data on ectopic pregnancies and pregnancy terminations, ensuring a comprehensive examination of the impact of SARS-CoV-2 on early pregnancy outcomes.
