Klinik für Urologie und Kinderurologie, Helios Klinikum Wuppertal, Wuppertal, Deutschland.

Ureteral strictures are uncommon complications of radiotherapy which are often recognized late. Their consequences range from harmless dilatation of the ureter to loss of renal function and potential life-threatening urosepsis.Therapy of radiogenic ureteral stricture is a challenging task for every urologist.

[Noninvasive Mechanical Ventilation in Acute Respiratory Failure].

Pneumologie

December 2015

Lungenklinik, Kliniken der Stadt Köln gGmbH, Köln.

The non-invasive ventilation (NIV) is widespread in the clinical medicine and has attained meanwhile a high value in the clinical daily routine. The application of NIV reduces the length of ICU stay and hospitalization as well as mortality of patients with hypercapnic acute respiratory failure. Patients with acute respiratory failure in context of a cardiopulmonary edema should be treated in addition to necessary cardiological interventions with continuous positive airway pressure (CPAP) or NIV.

[Purple urine bag syndrome in a patient with bilateral nephrostomy].

Med Klin Intensivmed Notfmed

November 2016

Klinik für Innere Medizin und Internistische Intensivmedizin, St.-Antonius-Hospital Eschweiler, Dechant-Deckers-Str. 8, 52249, Eschweiler, Deutschland.

This article presents a case of purple urine bag syndrome (PUBS) with purple discoloration of urine in a patient with bilateral nephrostomy and acute renal failure. The discoloration of the urine is a product of bacterial metabolism. In principle, PUBS is benign and not of relevance for prognosis.

Tyrosine kinase inhibitors represent today's treatment of choice in chronic myeloid leukemia (CML). Allogeneic hematopoietic stem cell transplantation (HSCT) is regarded as salvage therapy. This prospective randomized CML-study IIIA recruited 669 patients with newly diagnosed CML between July 1997 and January 2004 from 143 centers.

Major route additional cytogenetic aberrations (ACA) at diagnosis of chronic myeloid leukaemia (CML) indicate an increased risk of progression and shorter survival. Since major route ACA are almost always unbalanced, it is unclear whether other unbalanced ACA at diagnosis also confer an unfavourable prognosis. On the basis of 1348 Philadelphia chromosome-positive chronic phase patients of the randomized CML study IV, we examined the impact of unbalanced minor route ACA at diagnosis versus major route ACA on prognosis.

Increased cerebral water content in hemodialysis patients.

PLoS One

March 2016

Department of Neurology, RWTH Aachen University Hospital, Germany; Institute of Neuroscience and Medicine (INM-4), Research Centre Jülich GmbH, Jülich, Germany; Jülich Aachen Research Alliance (JARA)-Translational Brain Medicine, Jülich and Aachen, Germany.

Little information is available on the impact of hemodialysis on cerebral water homeostasis and its distribution in chronic kidney disease. We used a neuropsychological test battery, structural magnetic resonance imaging (MRI) and a novel technique for quantitative measurement of localized water content using 3T MRI to investigate ten hemodialysis patients (HD) on a dialysis-free day and after hemodialysis (2.4±2.

The Montreal Cognitive Assessment (MoCA) - a sensitive screening instrument for detecting cognitive impairment in chronic hemodialysis patients.

PLoS One

June 2015

Department of Neurology, RWTH Aachen University Hospital, Aachen, Germany; Jülich Aachen Research Alliance (JARA) - Translational Brain Medicine, Aachen and Jülich, Germany; Institute of Neuroscience and Medicine (INM-4), Research Center Jülich GmbH, Jülich, Germany.

Background: Chronic kidney disease (CKD) patients undergoing hemodialysis (HD) therapy have an increased risk of developing cognitive impairment and dementia, which are known relevant factors in disease prognosis and therapeutic success, but still lack adequate screening in clinical routine. We evaluated the Montreal Cognitive Assessment (MoCA) for suitability in assessing cognitive performance in HD patients in comparison to the commonly used Mini-Mental State Examination (MMSE) and a detailed neuropsychological test battery, used as gold standard.

Methods: 43 HD patients and 42 healthy controls with an average age of 58 years, were assessed with the MoCA, the MMSE and a detailed neuropsychological test battery, covering the domains of memory, attention, language, visuospatial and executive functions.

[Citrate anticoagulation in acute renal replacement therapy : Method of choice].

Med Klin Intensivmed Notfmed

June 2014

Klinik für Innere Medizin, St.-Antonius-Hospital Eschweiler, Dechant-Deckers-Str. 8, 52249, Eschweiler, Deutschland,

Background: Anticoagulation is prerequisite for efficient continuous renal replacement therapy (CRRT). Premature clotting of the extracorporeal system leads to therapy interruptions, is costly, and causes relevant blood losses.

Regional Anticoagulation: Regional citrate anticoagulation (RCA) achieves reliable coagulation inhibition and is clearly superior to heparin with regard to filter survival time.

Electronic address:

Article Synopsis
  • Cognitive impairment is common in hemodialysis patients and this study aimed to understand how cognitive function varies during the dialysis cycle, focusing on specific clinical factors.
  • A cross-sectional observational study included 47 HD patients and 40 controls, measuring cognitive performance related to attention and executive functions before and after dialysis.
  • Results indicated that while most patients showed some cognitive decline post-dialysis, significant changes were noted only in a minority, with associations found between cognitive fluctuations and intradialytic hypotensive episodes, psychoactive medication use, and some laboratory values.

The task of physicians is to maintain life, to protect and re-establish health as well as to alleviate suffering and to accompany the dying until death, under consideration of the self-determination rights of patients. Increasingly more and differentiated options for this are becoming available in intensive care medicine. Within the framework of professional responsibility physicians must decide which of the available treatment options are indicated.

January 2013

Klinik für Urologie und Kinderurologie, St.-Antonius-Hospital Eschweiler, Deutschland.

A primary megaureter is an anomaly with a prevesical or overall dilated ureter of more than 6 mm in diameter. It is important to distinguish between cases of primary non-refluxing and primary obstructive-refluxing megaureters, as the treatment of both is completely different. The basic diagnostic work-up includes ultrasonography and voiding cystourethrography.

Article Synopsis
  • - Cholesterol embolization occurs when cholesterol crystals are released into the bloodstream during vascular procedures, leading to small artery blockage and inflammation, often resulting in renal failure and skin symptoms like blue toes.
  • - Diagnosis is confirmed through biopsies or fundoscopy, as classic symptoms may not always be present, and vasculitis must be ruled out as a differential diagnosis.
  • - Treatment focuses on managing symptoms, with statins recommended for their protective effects; however, prognosis is poor, with a significant portion of affected patients requiring renal replacement therapy and a high mortality rate within the first year.

Can urology overcome the socioeconomic changes in the coming decade? Important is the establishment of networks and connections in order to cover the highest number of branches of urology possible. The responsibility of urologists necessitates changes and personnel placements in research, clinical and private practice. The author explains this exemplified by five selected aspects.
